Book excerpt: One particular area of concern has been physician referrals for services that finan- cially benefit the physician because the physician has an ownership interest in the practice or facility that provides the referred service. [...] At the same time, the ability to provide self-referred services continues to rise with expanded in-office ancil- lary capacity and steady increases in the number of ASCs. [...] Focusing on the in-office ancillary exception, the Com- mission examined three types of options to address the growth of an- cillary services: limiting the services or physician groups exempted from the Stark law, reducing payments or improving payment accu- racy for ancillary services, and adopting a limited prior authorization requirement.6 More broadly, MedPAC and others have endorsed more enco. [...] Rita Redberg, cardiologist and professor of medicine at the University of Califor- nia, San Francisco, will discuss some of the latest innovations in the practice of cardiology, their incorporation into office practice, and the utilization of services. [...] KEY QUESTIONS • How have the exceptions under the Stark law contributed to in- novation in health care technology? Health care delivery? What are the advantages to having a growing share of ancillary services and surgeries performed out of the hospital setting? What are the disad- vantages? • How has the mix of patients at ASCs and hospital outpatient de- partments changed with the growth in ASCs?.

Book excerpt: SeSSion The ability of physicians to refer patients to facilities in which they have a financial interest was curtailed with the Ethics in Patient Referrals Act, commonly known as the Stark law.1 The Act prevents a physician with an ownership or financial interest in a facility from referring a Medicare patient to that facility and it prevents the facility from billing for the referred service. [...] changeS in the health care lanDScape Changes in health care delivery since the enactment of the Stark law have contributed to the use of new sites of care and the continuing rise in the volume of and spending for physician services. [...] The referring physician pays the center for each imaging service or "per click" for the use of the facility and then bills Medicare for the technical or facility portion of the fee, which is higher than the per-click rental payment. [...] In fact, evidence indicates that specialty hospitals tend to care for the least costly subgroup of patients receiving a particular procedure.12 If this results in the concentration of the costlier patients in community hospitals, the growth in physician-owned specialty hospitals may impair the financial performance of hospitals that accept a wider mix of patients. [...] "Medicare Program; Revisions to Payment Policies Under the Physician Fee Schedule, and Other Part B Payment Policies for CY 2008; Revisions to the Payment Policies of Ambu- lance Services Under the Ambulance Fee Schedule for CY 2008; and the Amendment of the E-Prescribing Exemption for Computer-Generated Facsimile Transmissions," CMS-1385- FC, on display at the Federal Register on November 1, 2007.

Book excerpt: In addition, one physician will document how his provider group has used profiling to improve quality of care, and another phy- sician will present information about the use of profiling and concerns about the accuracy of physician comparisons. [...] A recent analysis by the Congressional Budget Office supports earlier stud- ies that indicate that the volume and intensity of physicians' services, not the fee paid for each service, has caused the rise in Medicare physician spending.1 Although Medicare can set the fee for each service, controlling the volume and intensity of physician care has remained elusive. [...] Health plans may provide the comparative information to physicians to illustrate differences in practice styles to encourage the adoption of more efficient options or the best practice. [...] A goal of resource use measurement is to provide information that is mean- ingful, actionable, and fair to physicians in order to reduce inefficient practice patterns."4 In comments on the GAO report, CMS Acting Ad- ministrator Leslie Norwalk wrote, "...given the role of physicians in driving total Medicare spending, there is opportunity to increase the efficiency of the Medicare program by measurin. [...] This may be due to the complex interplay of factors that affect health care costs and the growth in service use and the shift to more intensive prac- tice styles.

Available in PDF, EPUB and Kindle. Book excerpt: This review incorporates the views and visions of 2,000 clinicians and other health and social care professionals from every NHS region in England, and has been developed in discussion with patients, carers and the general public. The changes proposed are locally-led, patient-centred and clinically driven. Chapter 2 identifies the challenges facing the NHS in the 21st century: ever higher expectations; demand driven by demographics as people live longer; health in an age of information and connectivity; the changing nature of disease; advances in treatment; a changing health workplace. Chapter 3 outlines the proposals to deliver high quality care for patients and the public, with an emphasis on helping people to stay healthy, empowering patients, providing the most effective treatments, and keeping patients as safe as possible in healthcare environments. The importance of quality in all aspects of the NHS is reinforced in chapter 4, and must be understood from the perspective of the patient's safety, experience in care received and the effectiveness of that care. Best practice will be widely promoted, with a central role for the National Institute for Health and Clinical Excellence (NICE) in expanding national standards. This will bring clarity to the high standards expected and quality performance will be measured and published. The review outlines the need to put frontline staff in control of this drive for quality (chapter 5), with greater freedom to use their expertise and skill and decision-making to find innovative ways to improve care for patients. Clinical and managerial leadership skills at the local level need further development, and all levels of staff will receive support through education and training (chapter 6). The review recommends the introduction of an NHS Constitution (chapter 7). The final chapter sets out the means of implementation.

Book excerpt: This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

Book excerpt: Each year, more than 33 million Americans receive health care for mental or substance-use conditions, or both. Together, mental and substance-use illnesses are the leading cause of death and disability for women, the highest for men ages 15-44, and the second highest for all men. Effective treatments exist, but services are frequently fragmented and, as with general health care, there are barriers that prevent many from receiving these treatments as designed or at all. The consequences of this are seriousâ€"for these individuals and their families; their employers and the workforce; for the nation's economy; as well as the education, welfare, and justice systems. Improving the Quality of Health Care for Mental and Substance-Use Conditions examines the distinctive characteristics of health care for mental and substance-use conditions, including payment, benefit coverage, and regulatory issues, as well as health care organization and delivery issues. This new volume in the Quality Chasm series puts forth an agenda for improving the quality of this care based on this analysis. Patients and their families, primary health care providers, specialty mental health and substance-use treatment providers, health care organizations, health plans, purchasers of group health care, and all involved in health care for mental and substanceâ€"use conditions will benefit from this guide to achieving better care.

Available in PDF, EPUB and Kindle. Book excerpt: To maintain their own health and the health of their families and communities, consumers rely heavily on the health information that is available to them. This information is at the core of the partnerships that patients and their families forge with today's complex modern health systems. This information may be provided in a variety of forms â€" ranging from a discussion between a patient and a health care provider to a health promotion advertisement, a consent form, or one of many other forms of health communication common in our society. Yet millions of Americans cannot understand or act upon this information. To address this problem, the field of health literacy brings together research and practice from diverse fields including education, health services, and social and cultural sciences, and the many organizations whose actions can improve or impede health literacy. Health Literacy: Prescription to End Confusion examines the body of knowledge that applies to the field of health literacy, and recommends actions to promote a health literate society. By examining the extent of limited health literacy and the ways to improve it, we can improve the health of individuals and populations.
