In the field, the probe was cut into four sections, wrapped, and placed in a 55-gallon drum, This drum was delivered to the 222-S Laboratory. The 222 S Laboratory unpackaged the sections of the AN-107 electrochemical noise probe and examined the material for evidence of corrosion. Each of the four sections contained three C-ring and three bullet specimens. The specimens were examined for pitting corrosion, crevice corrosion, and stress corrosion cracking. No evidence of stress corrosion cracking was found in the stressed C-ring specimens. Minor pitting was evident on some surfaces. Crevice corrosion was the dominant type of corrosion observed.

Book excerpt: An Integrated Multi-function Corrosion Probe (IMCP) was installed in Tank 241-AN-107 on September 20, 2006. A portion of the probe was retrieved on June 8, 2010 and the sections holding the detectors were delivered to the 222-S Laboratory for analysis. The examination and disassembly of the probe sections encountered a number of challenges. However, disassembly and relevant analyses were successfully completed. The following summarizes our observations. Brittle failure of the fiberglass probe in the middle of detector 2 resulted in the recovery of only three vapor space C-rings and six supernatant bullet specimens. The design of the bullets and how they were attached to the probe made the recovery of the components more difficult. The use of glue/epoxy on the bullets and the attachment of the flat bottom of the bullets to the curved surface of the fiberglass probe body meant that weight loss on cleaning and surface area of the specimens could not be determined with acceptable accuracy. Macrophotography of all specimens reveals that corrosion was slight in the vapor space and extremely slight in the supernatant. The one pre-cracked C-ring recovered from the vapor space still had the stress bulge visible on the polished surface, indicating that crack propagation had not occurred in the tank. No photographs were taken of the C-ring before deployment. No further analysis was conducted on this specimen. A detailed discussion and photographic documentation are provided in this report.

Book excerpt: This test plan describes the methods to be used in the forensic examination of the Multi-probe Corrosion Monitoring System (MPCMS) installed in the double-shell tank 241-AY-101 (AY-101). The probe was designed by Applied Research and Engineering Sciences (ARES) Corporation. The probe contains four sections, each of which can be removed from the tank independently (H-14-107634, AY-101 MPCMS Removable Probe Assembly) and one fixed center assembly. Each removable section contains three types of passive corrosion coupons: bar coupons, round coupons, and stressed C-rings (H-14-l07635, AY-101 MPCMS Details). Photographs and weights of each coupon were recorded and reported on drawing H-14-107634 and in RPP-RPT-40629, 241-AY-101 MPCMS C-Ring Coupon Photographs. The coupons will be the subject of the forensic analyses. The purpose of this examination will be to document the nature and extent of corrosion of the 29 coupons. This documentation will consist of photographs and photomicrographs of the C-rings and round coupons, as well as the weights of the bar and round coupons during corrosion removal. The total weight loss of the cleaned coupons will be used in conjunction with the surface area of each to calculate corrosion rates in mils per year. The bar coupons were presumably placed to investigate the liquid-air-interface. An analysis of the waste level heights in the waste tank will be investigated as part of this examination.

There are five principle objectives of this task: (1) Acquire data on the corrosive characteristics of the existing waste in 241-AN-105. (2) Provide supporting data to the Integrity Assessment program. (3) Acquire data to support a proposed eventual change in the operation of double shell tanks to monitor and control tank corrosion through the use of corrosion probes rather than waste sampling and analysis. (4) Demonstrate that corrosion monitoring by evaluation of electrochemical noise data is possible in waste tank systems, particularly with regard to the detection of general corrosion and (if present) pitting and stress corrosion cracking. (5) Demonstrate the durability of the design of the corrosion monitoring equipment. The long term advantage of corrosion monitoring in double shell tanks is expected to be extended tank life and reduced annual operations cost.

Analytical results are presented that show supernatant was within the limits while the interstitial liquid remained below the limits for the analytical cores. Applicable AC 5.16 chemistry control limits for AN-107 are presented.

Book excerpt: Corrosion product from the annulus side of the double-shell tank 241-AY-102 primary wall was retrieved and analyzed. The analyses indicated that the tank wall was wetted with water leading to corrosion. The water source was quite likely the ingress of water from an external source and condensation of moisture during the long annulus ventilation system shutdowns. Corrosion of the primary tank wall in the annulus of double-shell tank (DST) 241-AY-102 was observed during the review of annulus videos of the tank in year 2001. This resulted in the issuance of an occurrence report, RP-CHG-TANKFARM-2001-0106. The occurrence report required collection and evaluation of the corrosion product to determine the cause of such corrosion. Therefore, a corrosion product sample was collected from the annulus side of the primary wall through Riser 80 in February 2003. The sample was scanned by Operational Health Physics and found to contain no radioactivity. The sample was then released and transmitted to ChemMet, Ltd, PC, for analyses. The present report discusses the types of analyses performed and the results of these analyses from ChemMet, Ltd, PC (Divine 2003) along with conclusions and recommendations. The letter report from ChemMet, Ltd, PC is attached.

Three grab samples were collected from riser 1 on September 11, 1997. Analyses were performed on samples 7AP-97-1, 7AP-97-2 and 7AP-97-3 in accordance with the Compatibility Grab Sampling and Analysis Plan (TSAP) (Sasaki, 1997) and the Data Quality Objectives for Tank Farms Waste Compatibility Program (DQO) (Rev. 1: Fowler, 1995; Rev. 2: Mulkey and Nuier, 1997). The analytical results are presented in the data summary report (Table 1). A notification was made to East Tank Farms Operations concerning low hydroxide in the tank and a hydroxide (caustic) demand analysis was requested. The request for sample analysis (RSA) (Attachment 2) received for AP-107 indicated that the samples were polychlorinated biphenyl (PCB) suspects. Therefore, prior to performing the requested analyses, aliquots were made to perform PCB analysis in accordance with the 222-S Laboratory administrative procedure, LAP-101-100. The results of this analysis indicated that no PCBs were present at 50 ppm and analysis proceeded as non-PCB samples. The results and raw data for the PCB analysis will be included in a revision to this document. The sample breakdown diagrams (Attachment 1) are provided as a cross-reference for relating the tank farm customer identification numbers with the 222-S Laboratory sample numbers and the portion of sample analyzed.

Book excerpt: NASA commissioned the Columbia Accident Investigation Board (CAIB) to conduct a thorough review of both the technical and the organizational causes of the loss of the Space Shuttle Columbia and her crew on February 1, 2003. The accident investigation that followed determined that a large piece of insulating foam from Columbia's external tank (ET) had come off during ascent and struck the leading edge of the left wing, causing critical damage. The damage was undetected during the mission. The Columbia accident was not survivable. After the Columbia Accident Investigation Board (CAIB) investigation regarding the cause of the accident was completed, further consideration produced the question of whether there were lessons to be learned about how to improve crew survival in the future. This investigation was performed with the belief that a comprehensive, respectful investigation could provide knowledge that can protect future crews in the worldwide community of human space flight. Additionally, in the course of the investigation, several areas of research were identified that could improve our understanding of both nominal space flight and future spacecraft accidents. This report is the first comprehensive, publicly available accident investigation report addressing crew survival for a human spacecraft mishap, and it provides key information for future crew survival investigations. The results of this investigation are intended to add meaning to the sacrifice of the crew's lives by making space flight safer for all future generations.
