Available in PDF, EPUB and Kindle. Book excerpt: Fertility preservation in children has long only been considered for those facing gonadotoxic treatments in the setting of new diagnoses of cancer. However, this field is now being expanded to include children with other conditions affecting fertility such as rheumatologic/nephrologic disease and even the transgender population, where fertility preservation counseling and discussion is in its infancy. Research regarding the optimal methods, timing, and eventual outcomes of both ovarian and testicular tissue storage in prepubertal girls and boys, respectively, as well as data on outcomes of oocyte and semen cryopreservation in adolescence is lacking. Deriving mainly from literature in adults, ovarian tissue cryopreservation has recently been deemed non-experimental; knowledge gaps still exist in the pediatric population.

Book excerpt: Many technologies exist for men and women to preserve fertility, and advances are being made all the time. Sadly, this information is not always well known in the oncology community and is, therefore, not presented to all eligible cancer patients. Reflecting the latest advances in the field and offering practical information useful to clinicians and scientists, Preservation of Fertility addresses new concepts, progress in reproductive techniques, novel treatment modalities, pregnancy after cancer treatment, patient care issues, and professional responsibilities. Tulandi and Gosden have assembled an outstanding group of internationally recognized specialists to address the entire spectrum of issues regarding fertility preservation. They begin with a discussion of age and fertility, epidemiology of cancer survival and fertility, and the vulnerability of the reproductive system to radiotherapy and chemotherapy. The book includes coverage of methods for preserving fertility, new developments in the laboratory for preserving gametes and gonadal tissue, and the psychosocial, legal, and ethical issues of fertility preservation. A powerful knowledge tool for the medical community, this pioneering book provides a complete understanding of the techniques for preserving fertility.

With modern treatment protocols, 80% of patients can be expected to survive. It has been estimated that up to one in 250 young adults will be a survivor of childhood cancer in the future; infertility, however, may be a consequence. As a wide range of fertility preservation methods are increasingly offered by clinicians, this systematic and comprehensive textbook dealing with the cryobiology, technology and clinical approach to this therapy will be essential reading to infertility specialists, embryologists, oncologists, cryobiologists, ObGyns, andrologists, and urologists with an interest in fertility preservation. Fertility Cryopreservation reviews all the techniques of this increasingly important field within reproductive medicine. It covers the basic principles of pertinent cryobiology, and contains major sections on the different therapies available, written by international specialists combining experience from both academic centers and commercial industries.

Fertility concerns beyond cancer are also discussed, including disorders of sexual differentiation, gender dysphoria and thalassemia. Practical chapters on pediatric oncofertility in the clinic round out the book, covering setting up a practice, counseling and consent, disclosure and insurance considerations. Concluding chapters document fertility preservation techniques and outcomes internationally, with contributions from Portuguese, Brazilian and Japanese authors. Timely and wide-ranging, Pediatric and Adolescent Oncofertility is an ideal resource for reproductive medicine specialists, pediatric oncologists, and primary care physicians treating pediatric and adolescent cancer patients looking to protect fertility options.

Book excerpt: Identifying and Addressing the Needs of Adolescents and Young Adults with Cancer is the summary of a workshop convened by the Institute of Medicine's National Cancer Policy Forum in July 2013 to facilitate discussion about gaps and challenges in caring for adolescent and young adult cancer patients and potential strategies and actions to improve the quality of their care. The workshop featured invited presentations from clinicians and other advocates working to improve the care and outcomes for the adolescent and young adult population with cancer. Cancer is the leading disease-related cause of death in adolescents and young adults. Each year nearly 70,000 people between the ages of 15 and 39 are diagnosed with cancer, approximately 8 times more than children under age 15. This population faces a variety of unique short- and long-term health and psychosocial issues, such as difficulty reentering school, the workforce, or the dating scene; problems with infertility; cardiac, pulmonary, or other treatment repercussions; and secondary malignancies. Survivors are also at increased risk for psychiatric conditions such as anxiety, depression, substance abuse, and suicide and may have difficulty acquiring health insurance and paying for needed care. Identifying and Addressing the Needs of Adolescents and Young Adults with Cancer discusses a variety of topics important to adolescent and young adult patients with cancer, including the ways in which cancers affecting this group differ from cancers in other age groups and what that implies about the best treatments for these cancer patients. This report identifies gaps and challenges in providing optimal care to adolescent and young adult patients with cancer and to discuss potential strategies and actions to address them.

Chronic diseases justify long-term medical, psychological, educational, and social management. The establishment of networks with adult endocrinologists is, therefore, very important to achieving optimal conditions for a successful transition period. In this book, leading experts discuss major public health issues such as diabetes and obesity. They also review genetic diseases, including Turner syndrome and congenital adrenal hyperplasia. All clinical cases are presented in a focused way to highlight any issues which might be discussed by the doctor with patients and family. Psychological approaches are debated as are proposals for defining specific tools to encourage autonomy during this life period. This book is a valuable tool and will be helpful in answering many questions for endocrinologists, adult and pediatric, dedicated to the long-term management of patients with chronic diseases.
