Book excerpt: Failure to Thrive (FTT) is a multifactorial illness requiring the systematic application of evidence in conjunction with a multidisciplinary team-based approach. Evidence suggests most cases of FTT can be diagnosed using key elements of the patient history and physical examination including appropriate growth assessment, and that laboratory testing is usually unhelpful in determining an etiology. Despite this, there is wide variation in the definition, evaluation, treatment, costs, and outcomes of FTT patients, with subsequent burdens on the healthcare system. A suggested approach to the differential diagnosis and management of FTT is to review calories – inadequate caloric intake, excessive calorie losses, and/or increased caloric intake (i.e. increased metabolic demand) – while acknowledging the available resources along the continuum of patient care. Implementing evidence-based practice for FTT provides improvement opportunities for providers and potentially has significant impact on the healthcare system. This book is designed to present a practical and concise handbook on the diagnosis and management of failure to thrive (FTT)/ malnutrition for quick reference by trainees and practitioners by using case examples and available evidence in the current literature. This book is divided into 7 chapters in a logical progression of defining, diagnosing, and managing FTT with cases in each chapter to further illustrate evidence-based care.

Book excerpt: Failure to thrive affects the lives of many infants and young children at critical times in their development and represents a significant public health problem in the United States. Moreover, this condition is invisible and can affect children for long periods of time before it is recognized. The long-term psychosocial sequelae of failure to thrive have only begun to be recognized but may be more severe than first realized. We do know that the costs to society in terms of acute pediatric hospitalization and long-term rehabilitation, foster care, and mental health treatment of young children who present with failure to thrive are considerable. Children who are diagnosed with failure to thrive represent a special challenge and opportunity for intervention, especially preventive intervention, because it is quite possible that many of the long-term consequences of this condi tion on psychological development can be lessened via early recognition and intervention. However, the potential for preventive intervention in failure to thrive has been limited by the state of the art in scientific knowledge and practice. Despite the frequency with which failure to thrive is encounter ed in ambulatory and inpatient settings, there is little scientific infor mation to guide practitioners. Research on the causes and consequences of failure to thrive has been very much limited by small sample sizes, lack of common definitions, and short follow-up periods. Uncertainties in the science of failure to thrive coincide with the considerable practical difficulties involved in diagnosis and inte~vention.

Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. In this third report, the committee provides its final analyses, recommendations, and the supporting rationale.
