(2001), birth rates for United States (U.S.) teenagers continue to be higher than the rates for teens in most other developed countries Furthermore, according to Childtrends (2006, 2001, 2007) the proportion of teen births that were repeat has been relatively consistent (21-19%) from 1999 to 2005. Studies state that a teen can finish high school, develop vocational skills and become self-sufficient by delaying additional pregnancies during the teen years. This study conducted a secondary analysis of restricted data from the National Longitudinal Survey of Adolescent Health (Add Health) to determine which factors are associated with the prevention of a repeat teen pregnancy. The core sample included females from Wave II with at least one birth prior to age 19 (teen mothers). Then, this sample of teen mothers was divided into two sub samples of teen mothers, one who did not have any additional pregnancies prior to age 20 and the second who had additional pregnancies prior to age 20 between Waves II and III. This study assessed the relationship between several demographic, psychosocial and behavioral factors among teen mothers. Discrete time-series analyses using Stata statistical software were performed to examine the factors contributing to the prevention of repeat teen pregnancy. The results of the study showed that having high spirituality and a medium positive attitude towards pregnancy/childbearing were protective and showed a decreased risk for a repeat teen pregnancy. These findings could be used to develop interventions that include spirituality and other protective factors such as: continuing in school, participating in vocational training, addressing teen mothers' attitudes toward pregnancy and childbearing, consistently using contraceptives, initiating the program early in the post partum period and having program staff that is able to connect with the teen mother. Interventions may work collaboratively with faith communities which are good resources that have access to methods to practice spirituality and influence values. insert short pdf.

The United States continues to have the highest adolescent pregnancy rates of any industrialized country (Kost, Henshaw, & Carlin, 2010). While adolescents of all ages (10-21 years) can become pregnant, pregnancy rates are highest among adolescents who are 15 to 19 years of age. Multiple factors have been identified that increase the likelihood of adolescent pregnancy and repeat pregnancy. Often times, adolescent pregnancy is the result of sexual risk-taking, which has been attributed to the changes that occur neurologically, physically, and emotionally (Reyna & Farley, 2006). The antecedents of adolescent pregnancy and repeat pregnancy are often the result of sexual and social decisions made by the adolescent. Purpose: The purpose of this qualitative, descriptive, study is to examine the influences of social and sexual decisions made by adolescent mothers, 16 to 19 years of age, before and after delivery of their first child. Methods: This study utilized a qualitative, descriptive inquiry approach. The participants were recruited from Jefferson County Department of Health. The sample (n = 6) was comprised of adolescent mothers ages 15-19 years of age, within the first year following delivery of their initial child. Semi-structured interviews were utilized for data collection and content analysis was used in order to analyze the data collected. Results: The adolescent mothers' relationships with parents, romantic partners, friends, school, community, and newborn child influenced her decisions. Conclusions: The findings of the current study agree with previous literature supporting the influence of relationships on adolescent mothers' social and sexual decision-making. The impact of adolescent mothers' transition into motherhood as well as the relationship with her newborn child on her decision-making addressed gaps observed in the literature.

Book excerpt: [Truncated abstract] AIMS The primary aim of this thesis was to identify the biological (clinical), psychosocial and behavioural risk factors that influence teenage pregnancy and rapid repeat pregnancy within two years of a first teenage birth (RRP) The secondary aims were to: (1) compare the incidence of RRP among users of Implanon, the combined oral contraceptive pill (COCP) or Depot Medroxyprogesterone Acetate (DMPA), and barrier methods or nothing (barrier/none); (2) assess contraceptive continuation among Implanon users compared with non-users postpartum; and (3) assess the acceptability of Implanon as a contraceptive of choice. METHODS A two phase design was used, the first phase being a retrospective study of 4896 nulliparous pregnant women delivering at the sole tertiary obstetric hospital in Western Australia. Data were obtained from hospital-based computerised records. Maternal risk factors, pregnancy characteristics and obstetric and perinatal outcomes were compared between teenage and adult pregnancies. Statistical analysis included multivariable comparisons of binary outcomes using logistic regression. The second phase was a prospective cohort study drawn from Phase 1, which recruited 187 pregnant teenagers delivering at the same, sole tertiary obstetric hospital in Western Australia. Questionnaires were used to gather data. Statistical analysis included multivariable logistic regression to investigate the simultaneous factors associated with sexual activity and repeat pregnancy, every three months until 24 months postpartum. Time to contraceptive discontinuation and time until repeat pregnancy were estimated using Kaplan-Meier survival probabilities. RESULTS Phase 1 showed that the rate of teenage pregnancy was 11% (560/4896). Compared to adults, teenagers were more likely to be Indigenous (33% vs. 4%). Prevalence of smoking in pregnancy was associated with maternal ethnicity and age. Indigenous women were more likely to smoke than non-Indigenous women with the youngest Indigenous teenagers being the most likely to smoke (OR 6.29; 95% CI 3.99 9.92). Anaemia (14% vs. 6%) and pregnancy-induced hypertension (11% vs. 7%) were more frequent in teenagers. Caesarean delivery in teenagers was lower (17% vs. 33%). Neonatal outcomes were similar between age groups, except for stillbirth (3% vs. 2%, P=0.014), which was higher in teenagers over 16 years of age (OR 1.99; 95% CI 1.03 3.76). Outcome comparisons accounting for maternal age, smoking and ethnicity showed that Indigenous status increased risk of preterm delivery (OR 1.31; 95% CI 1.01 1.71), admission to special care (OR 1.41; 95% CI 1.10 1.81) and low birth weight (OR 1.43; 95% CI 1.10 1.87) without the additional effect of age. Phase 2 showed that of the 147 follow-up participants, 49 (33%) had a RRP. Sexual activity was independently associated with using oral contraception (OR 2.83; CI 1.38 5.82), living with the birthfather (OR 8.43; CI 5.12 13.86), intending to become pregnant (OR 3.20; CI 1.53 6.65), smoking marijuana (OR 2.60; CI 1.38 4.79) and using alcohol (OR 1.93; CI 1.17 3.20.)...
