Book excerpt: Introduction : Les accidents vasculaires cérébraux (AVC) sont la lere cause de handicap acquis chez l'adulte. La thrombolyse intraveineuse est un traitement qui a fait ses preuves dans les 3 heures suivant le début des symptômes. En 2007, nous avions mis en évidence, que sur une population de 536 patients admis aux urgences pour un AVC, seuls 31 % d'entre eux avaient été admis avant la 3ème heure. Après des campagnes d'information auprès du public et des professionnels de santé, l'objectif principal de notre travail a été de réévaluer ces délais ainsi que les facteurs les influençant. Matériel et Méthodes : II s'agit d'une étude épidémiologique, prospective s'étant déroulée entre le 8 Octobre 2012 et le 8 Octobre 2013 au CHU de Saint Etienne. L'analyse des résultats préliminaires a porté sur 368 patients présentant un AVC ischémique. Les différents délais médians (appel, intervention, transport et admission) ont été mesurés ainsi que la nature de l'appelant et les modalités de recours au système de soins. Deux groupes de patients ont été réalisés en fonction de l'admission avant ou après 3ème heure. Résultats : 192 (52,1%) patients ont été admis avant la 3ème heure avec un délai médian d'admission de 1hl6 min (groupe I) versus 22h20 min (groupe II). Les facteurs significativement associés à une réduction des délais d'admission ont été : l'appel par l'entourage, sa présence au moment du déficit, un appel vers le centre 15 ou les pompiers et un transport rapide sans intervention médicale préalable. Un déficit moteur, facial ou phasique était également associé au groupe I. Quarante-six (12,5%) patients ont ainsi pu bénéficier d'un traitement thrombolytique. Conclusion : Les délais d'admission des patients aux urgences pour suspicion d'AVC ischémique sont principalement liés à un retard de recours au système de soins et à son mode. En effet, une reconnaissance immédiate des signes de l'accident vasculaire cérébral est nécessaire ainsi qu'un transport immédiat vers une structure adaptée. Un message simple doit être délivré à la population en ce sens.

L'objectif était d'étudier les facteurs influençant les délais d'admission des patients atteints d'AVC ischémique aux urgences de Pau. Méthodologie : il s'agissait d'une étude épidémiologique rétrospective incluant tous les patients admis pour un AVC ischémique aux urgences de Pau de 2018 à 2019. Les données recueillies étaient: heure de survenue du déficit; âge; sexe; adresse; facteurs de risque cardiovasculaire; antécédents d'accident vasculaire cérébral et d'infarctus du myocarde; traitement antiagrégant; évaluation de l'autonomie; mode de recours aux soins; symptômes neurologiques; Score NIHSS. Deux groupes ont été constitués: groupe 1, patients avec un délai d'admission ≤ 4,30 heures; groupe 2, patients avec un délai d'admission> 4,30 heures après l'apparition du déficit. Résultats: parmi 458 dossiers analysés, 381 patients ont été inclus, 217 (52%) admis moins de 4,30 heures après l'apparition des symptômes, 164 (48%) après 4,30 heures. Le délai médian d'admission était de 3,50 heures [1,5-10]. En analyse multivariée, les facteurs associés à un délai d'admission tardif étaient: l'aide à l'autonomie (OR = 2,62), la consultation spontanée aux urgences par un moyen de transport privé (OR = 5,98), le recours à la médecine libérale (OR = 17,66), un déficit ataxique (OR = 2,7) ou visuel (OR = 2). Discussion: ces résultats pourraient permettre de cibler les populations à risque, et de mettre en place des moyens adaptés de prévention et d'éducation à l'AVC.

L'objectif de ce travail est definir le type de prise en charge effectuee dans un tel service. Patients et methode : Il s'agit d'une etude retrospective sur une duree de 12 mois consecutifs incluant les patients dont le diagnostic pose au SAU etait un accident vasculaire cerebral augu (hors AVC par rupture d'anevrisme). Ont ete recueillis, le delai d'admission par rapport a l'apparition des symptomes (DA), les signes cliniques presents a l'admission, le bilan realise, le traitement prescrit, la duree de sejour aux urgences ainsi que le service de mutation. Le diagnostic final de certitude a ete retenu a la sortie du service hospitalier de court sejour. Resultats : 557 dossiers ont ete retenus, 9 ont ete exclus du fait de donnees manquantes. L'age moyen etait de 73 ans ± 13, le DA median etait de 10 heures (2h-20h) avec 27 % des patients admis dans les trois heures. Un scanner cerebral a ete realise aux urgences pour 74 % des patients avec un delai median de 4 heures (2h-7h), une echographie doppler des troncs supra aortique pour 42 % d'entre eux avec un delai median des 3 heures (2h-7h). Une anticoagulation a doses curatives a ete prescrite dans 14 % des cas. La duree mediane de sejour aux urgences etait de 10h30 (6h45-22h15). Les unites prenant habituellement en charge la pathologie neurovasculaire ont pu accueillir 59 % de ces patients. Le diagnostic final d'AVC a ete retenu pour 88 % des patients. Le delai median d'hospitalisation dans les services d'hospitalisation de courte duree a ete de 16 jours. Conclusion : les delais d'admission au SAU, d'obtention des examens complementaires et de transfert dans les unites d'hospitalisation de courte duree, apparaissent longs. L'organisation d'une filiere de soins specifique a cette pathologie devrait permettre une prise en charge plus rapide au SAU.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
