Book excerpt: Contexte : La consommation de benzodiazépines et apparentés au long cours chez les personnes âgées est un problème de santé publique Les médecins généralistes sont en première ligne pour conduire le sevrage chez ces patients. L'objectif de ce travail est d'identifier les facteurs facilitant et limitant le sevrage en benzodiazépines et apparentés chez la personne âgée. Méthode : Une étude qualitative et analysée selon l'outil managérial SWOT a été réalisée. Les données étaient recueillies par la réalisation d'entretiens semi-dirigés auprès de médecins généralistes installés de la métropole lilloise. Résultats : Les forces principales étaient le respect strict des règles de prescription, la nécessité d'une sensibilisation répétée auprès du patient et la discussion autour des effets indésirables potentiels. Les faiblesses étaient la banalisation de l'utilisation des benzodiazépines, la forte demande de prescription et le manque de temps à consacrer au sevrage. Les opportunités étaient consacrées au rôle décisif de l'hôpital, au rôle positif du ROSP et à la prise de conscience du patient. Les menaces comportaient le poids du contexte socio-économique actuel, le manque de volonté politique et le manque de sensibilisation externe des patients et des médecins. Conclusion : Les médecins généralistes mettent en évidence le manque de temps nécessaire à consacrer au sevrage, le manque de volonté politique et la réponse inadaptée les pouvoirs publies au mésusage des benzodiazépines. Susciter la prise de conscience à travers une sensibilisation accrue et d'une meilleure coopération avec le milieu hospitalier sont des pistes à envisager.

The unique needs of dying children are particularly evident across Africa, where the scale of the problem is overwhelming, and the figures so enormous that they are barely comprehensible: over 400,000 children in Africa died from AIDS in 2003, and out of the 166,000 children a year diagnosed with cancer, 85% of these are in the developing world. Despite the enormous need, provision of children's palliative care in Africa is almost non-existent, with very few health workers trained and confident to provide care for dying children. The challenges of providing palliative care in this setting are different to those in more developed countries, contending with the shortage of physical and human resources in addition to the vast scope of the care needed.
