Book excerpt: Cette étude, menée à partir d’une base de données hospitalière, avait pour objectif d’identifier les facteurs d’échec de la transmission mère-enfant du VIH (TME). Dans la période d’étude concernée (01/01/2000 au 31/12/2005), 293 patientes séropositives pour le VIH ont accouché au CHSD. Parmi elles, neuf ont contaminé leur nouveau-né soit un taux de TME de 3%. Le risque de TME est apparu multifactoriel, les principaux facteurs favorisants étant : la prise en charge tardive, l’absence ou la mauvaise observance du traitement ARV pendant la grossesse, l’origine des mères, la prématurité, l’ouverture prolongée de la poche des eaux. Ces facteurs étaient majoritairement en rapport avec des défauts de dépistage précoce ou des échecs de la prise psychosociale. Le risque de TME dans cette cohorte n’était pas corrélé à la parité, à l’âge, à la voie d’accouchement et au poids de l’enfant. Nous avons confronté ces résultats aux données de la littérature et conclu en insistant sur l’intérêt d’un accompagnement personnalisé des patientes tout au long de la gravido-puerpéralité.
