Background: For the first time in history, the current generation of children is projected to have a shorter life expectancy than their parents primarily because of children's overweight status. Many factors place young children at risk for becoming overweight. The risk factors include: a family history of obesity, family lifestyle, diet sedentary living environments, and a lack of exercise. Purpose: To examine the relationship(s) between a mother's mealtime habits, maternal BMI, the location of maternal meals, and infant weight gain at two months of age and at six months of life. Subjects: Mother and infant dyads (N=160) from three different pediatric clinics participated in the study. Racial and financial considerations were considered in clinic selection. Exclusion criteria required that the infant was: not born younger than 36 weeks, not hospitalized, not diagnosed with a genetic syndrome, and not diagnosed with a gastrointestinal disease or a metabolic disorder. Almost 60% of the mothers of the sample are African American, 34% are white, and 2.5% are Hispanic. The age of the mothers was positively skewed, and ranged from 15 years of age to 42 years of age, with a mean age of 24. Methods: Questionnaires were administered to each maternal participant at the 2 month and 6 month well-child visit. A series of closed-ended questions assessing maternal eating habits were included. It is hypothesized that mothers who eat their main meal at the kitchen or dining room table will have infants at average weight at 6 months of age and mothers who eat their main meal elsewhere would have heavier infants at 6 months of age. Descriptive statistics including frequencies, means and standard deviations as well as Pearson correlations will be conducted. Given the unprecedented environment in the United States in which two-thirds of the adult population meets the criteria for being overweight or obese, the implications for women in the reproductive age period are unique in the history of the country. The concerns for maternal and infant health are real. The questions and answers tackled by committee members and workshop participants were not easy. Nevertheless, having an opportunity to explore what is known, examine the gaps in knowledge, and explore what to do now and in the future build a pathway for further inquiry and action. This report summarizes the workshop proceedings and highlights key themes that deserve further attention. The participants in this workshop describe what is known about recent trends in maternal weight gain and the impact of maternal weight during pregnancy on the health of mothers and their children. The workshop provided a valuable opportunity to assess trends that have occurred since the publication of an earlier study by the Institute of Medicine (IOM), which included guidelines for recommended weight gain during pregnancy.

Book excerpt: A review of 120 studies published since 2006 was undertaken to examine the relationship between food insecurity at the household or individual level and the following nutrition indicators: child stunting, child wasting, low birth weight, exclusive breastfeeding of infants < 6 months of age, anaemia in women of reproductive age, child overweight and adult obesity. While there is some evidence of a direct association between food insecurity and stunting for children in lower-middle and upper-middle income countries, evidence of links between food insecurity and either child wasting or overweight is almost absent, with the exception of an association with overweight among girls in middle- and high-income countries. The obesity–food insecurity link is most predominant among women in high-income countries, while it is almost absent in men. In addition, food insecurity increases the risk for low birth weight in infants and anaemia in women. Methodological concerns that pose challenges for valid comparison of results relate to study design, data analysis techniques, use of different indicators of household/individual food security and malnutrition, and the limited availability of high-quality micro-level data from large-scale surveys. Most studies report correlation rather than causal associations between food insecurity and nutrition indicators; longitudinal micro-level data from large-scale surveys can help establish causal association and capture the dynamic nature of food insecurity. Food insecurity emerges as a predictor of undernutrition as well as overweight and obesity, highlighting the need for multisectoral strategies and policies to combat food insecurity and multiple forms of malnutrition.
