These side-effects can include fatigue, decreased activities of daily living (ADL) performance, and decreased quality of life (QOL). Despite the interrelationship between ADL performance and QOL, attention to ADL has been limited in the area of cancer rehabilitation. The purpose of this study was to determine the effects of a specifically designed therapeutic exercise regimen on selected functional performance tasks, perceived fatigue, and QOL measures in patients undergoing treatment for cancer. We hypothesized that exercising subjects would experience significantly less decrement in fatigue, ADL performance, and QOL than subjects not participating in structured exercise. Participants in the study were thirty male and female cancer patients (middle aged to elderly) undergoing treatment for cancer (surgery and adjuvant treatment: chemotherapy or radiation). Subjects completed questionnaires regarding fatigue, QOL, and physical activity level and were tested on various functional activities at four distinct time points: (1) at diagnosis; (2) post-surgery; (3) at 9 weeks post-diagnosis; (4) at 9 weeks post-diagnosis. Adjuvant treatment commenced between measurement point 2 and 3. Experimental subjects took part in a 17-week exercise intervention (low to moderate intensity level; commenced at diagnosis) that targeted muscle group and energy systems involved in ADL (low to moderate intensity level), while control subjects did not participate in structured exercise. A factor analysis allowed for the creation of new variables that were the combination of original dependent variables that loaded well (correlated) together. Repeated-measures ANOVA analyses revealed significant improvement in the experimental group on all study variables and significant decline in the control group. These findings indicate that participation in a specifically designed exercise intervention during treatment can elicit improvement in fatigue, QOL, and the performance of functional activities. The results also indicate that a sedentary lifestyle during treatment exacerbates treatment-related side-effects. Cancer patients should be encouraged to take part in a structured exercise program from the point of diagnosis through the end of treatment in order to improve treatment tolerance, shorten recovery time, and maintain or improve quality of life.

Book excerpt: Quality research has been done to further our understanding of the role of physical activity (PA) as a potential therapeutic modality to improve health outcomes of cancer survivors. Yet, there are gaps in our understanding that need to be addressed. As such, the aims of the three distinct studies making up this dissertation project were: 1) to cross-sectionally explore PA behaviors and their relationship to health-related quality of life (HQOL) in a sample of Native American cancer survivors (NACS), 2) to conduct a randomized controlled trial (RCT) to evaluate the feasibility, safety and role of an exercise intervention in improving both prognostic biomarkers and HQOL in a sample of lung cancer survivors, and 3) to utilize a RCT to evaluate the feasibility and role of an exercise intervention to improve the HQOL of informal caregivers of lung cancer survivors. In regards to our first study, our findings suggest that a majority of NACS meet PA recommendations, though this may be the result of over-reporting, and that meeting these recommendations is associated with better physical function in this sample, but of no other HQOL constructs. Our second study found that, while recruitment was challenging, for lung cancer survivors that were interested in and well enough to participate, exercise training appears to be safe and feasible. The majority of our sample was compliant with the intervention and met PA recommendations through a combination of in-clinic and at-home exercise, though our small sample size did not allow for a thorough evaluation of the effectiveness of this intervention as it relates to markers of prognosis and HQOL. Due to the challenges associated with recruiting lung cancer patients to participate in the second study, the sample size in our third study was also quite low, hindering our ability to examine exercise effects on HQOL and exercise capacity. Despite that, the exercise group was able to meet current PA recommendations. When considering this work collectively, this project adds weight to the literature suggesting that moderate-intensity exercise is both feasible and safe in cancer survivors.

The regular, moderate level of exercise seems to be a solution that may help the patients. Any form of exercise can be helpful for the patient in fighting the fatigue .To find evidence based data, In the adult oncology population with cancer diagnosis, what is the effect of the physical activity or different model of exercise in reducing the fatigue compared to the patients not using the physical activity or exercise, over a 3 month period of time, a literature review on the fatigue and the effect of the exercise on the patients with cancer performed. The review of 15 articles provided both qualitative and quantitative analysis of the results including result of literature analysis. The result of this review provides evidence that it is effective in reducing the fatigue and the helps to increase physical functional status. A proposal for, a pilot study with proper assessment and evaluation tools to further assess the effectiveness of a home based personalized exercise program is recommended to help in implementation of the program house wide. The pilot program proposal needs to be approved by the Institutional Review board prior to the implementation. The implementation is incorporated using the trajectory theory to help with the planning and incorporating intervention to improve effect of chronic illness and related side effect management .The Participation of the staff, the physicians, advanced practice nurses, pharmacist, physiotherapist and the patient representatives/community representative are needs to be ensured using the in-services and dissemination plan. The proper assessment of the fatigue and the interventions to reduce the fatigue in any form of exercise will promote quality of life for these patients; this will also ensure the patient satisfaction and quality of care.

It has been suspected that more of these benefits may be obtained in a structured group-based exercise program due to the safe environment and support from instructors/group members. Exploration utilizing the Self-determination Theory was used to determine differences in exercise participation immediate versus long-term post participation in a structured group-based program. Furthermore, fulfillment of the basic psychological needs (autonomy, competence, relatedness) was assessed to determine different factors that influence motivation over time. Method: For both Phase One and Phase Two of this study 10 semi-structured interviews were conducted with women aged 37 years and older, living with cancer, and who were recruited from a structured group-based exercise program. All participants completed the Medical History Assessment form which gathered information regarding lifestyle factors and general health. Using NVivo and the basic psychological needs as a theoretical framework exercise motivation was explored for both Phase One and Phase Two. Results: According to participants interviewed immediate post program (Phase One), fulfillment of autonomy, competence, and relatedness was achieved while participating in a structured group-based exercise program. Participants indicated that the safe and comfortable atmosphere, as well as the support from the group played an important role in fostering their motivation towards exercise. Those participants interviewed long-term post program (Phase Two) had varied outcomes. Four participants continued exercise in a structured group-based exercise program and indicated that fulfillment of the basic psychological needs still remained present and impacted their adherence to exercise. The remaining participants had significantly lower levels of exercise participation and indicated a variety of barriers (e.g., injury, retirement) influencing exercise engagement. The lack of group support negatively impacted exercise outcomes for the less active individuals in this phase. Conclusion: The findings revealed that exercise in a structured group-based exercise program may provide more of the benefits associated with exercise for individuals living with cancer. Fulfillment of all three basic psychological needs did impact exercise motivation, which was evident for those who continued exercise over time. Further investigation into the role of the needs and long-term exercise participation for individuals living with cancer is necessary to gain a better perspective on how exercise programs can be modified appropriately for this population."-- from abstract.
