Book excerpt: Contexte : Depuis 2004, avec la restructuration législative des modalités du troisième cycle des études de Médecine Générale, on assiste dans les facultés de médecine de France à une réorganisation des enseignements. A présent, tournée vers un enseignement centré sur les apprentissages, l'approche par compétences devient la norme qui prévaut dans nos programmes de formation. Basée sur les six compétences, qui ont été mises en place par le Collège National des Généralistes Enseignants ainsi que par d'autres instances influentes sur la scène internationale, nous devons mettre en place les enseignements théoriques adaptés à cette approche. Cette réorganisation ne peut se faire qu'au prix d'une évaluation rigoureuse de ces enseignements. Objectifs : Elaboration d'un outil d'évaluation de nos formations facultaires à Poitiers, soumis aux étudiants sous la forme d'un questionnaire de satisfaction après enseignement, ayant pour but de mettre en évidence les points forts et les points faibles de nos cours théoriques, et de permettre ainsi de réaliser les ajustements nécessaires à l'amélioration de de ceux-ci. Méthode : Nous avons réalisé une étude prospective auprès de trois promotions d'internes de médecine générale à Poitiers, par l'intermédiaire d'un questionnaire de niveau 1 de Kirkpatrick, avec une échelle de Likert à 6 niveaux. Les réponses ont été recueillies, via l'environnement numérique de travail de la faculté, et ont permis de visualiser les critères de jugements des étudiants. Résultats : Il ressort que les étudiants sont majoritairement satisfaits des enseignements qui leur sont proposés. Il ne semble pas exister de points négatifs importants ou nettement individualisables. Notre outil a permis de souligner les critères de jugements les plus importants aux yeux des étudiants. Conclusion : Nous avons pu démontrer que notre outil présentait une validité importante, tout en donnant aux enseignants un feedback indispensable aux ajustements nécessaires à tout programme de formation efficient.

Désormais unanimement plébiscitées, celles-ci ont donné lieu à l'élaboration de nouveaux outils pédagogiques. À Saint- Etienne, les études de 3e cycle de médecine générale ont ainsi été progressivement repensées selon une pédagogie centrée sur l'apprenant. Planification. Ce changement de paradigme d'apprentissage a donné naissance à un projet original : la participation d'un étudiant à la création d'une nouvelle unité d'enseignement traitant de l'organisation d'un cabinet de soins premiers. Implantation. Les étudiants de 2e année du DESMG ayant bénéficié de cette demi-journée de formation ont été invité à remplir un questionnaire de satisfaction, évaluant la qualité et l'utilité de cet enseignement. Évaluation. Bien que le taux de réponse ne s'élève qu'à 19%, l'ensemble des étudiants s'est dit satisfait du cours, par son contenu et son côté interactif. Conclusion. Cette étude tendrait à montrer un intérêt à inclure un étudiant dans l'élaboration d'un enseignement destiné à ses pairs. Ce procédé, peu commun, constitue un nouvel outil de pédagogie active, permettant aux enseignants de diversifier les méthodes d'apprentissages.

Dans le processus de certification des internes au sein du Département Universitaire de Médecine Générale de la faculté de Rouen, les ECOS (Examens Cliniques Objectifs Structurés) font partie des épreuves sanctionnantes. L'objectivité est une des qualités requises pour une évaluation. Objectifs : Améliorer l'objectivité des observateurs des ECOS au moyen d'une formation préalable et d'une notice explicative des grilles d'évaluation. Matériel et méthode : Étude quantitative prospective au moyen d'une vidéo d'un étudiant passant une station ECOS diffusée à 140 observateurs. Analyse des notes sur 100 et item par item. Résultats : Les notes sur 100 sont plus homogènes dans les groupes ayant la notice explicative, mais pas systématiquement dans ceux ayant eu la formation. En ce qui concerne les items, certains nécessitent des explications complémentaires pour homogénéiser la notation, et d'autres non. Discussion : La notice explicative améliore l'objectivité des observateurs. En ce qui concerne la formation, cela n'a pas été démontré, tout du moins, dans la forme actuelle délivrée à la faculté de Rouen. Pour ce qui est des items de la grille d'évaluation, plus l'item est précis, moins il nécessite d'explication sur la notice. Conclusions : L'objectivité des observateurs dans les ECOS peut être améliorée, notamment par une notice expliquant les items les moins précis. La formation des observateurs serait très probablement plus bénéfique sous une forme plus longue, plus détaillée, et à distance des épreuves.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

This document offers an approach to assessment that should help you review information that is already available and only collect new data that will be of practical use, depending on your capacity and the phase of the humanitarian crisis. This document is rooted in two policy documents, the IASC Reference Group s (2010) "Mental Health and Psychosocial Support in Humanitarian Emergencies: What Should Humanitarian Health Actors Know?" and the "Sphere Handbook's Standard on Mental Health" (Sphere Project, 2011). It is written primarily for public health actors. As the social determinants of mental health and psychosocial problems occur across sectors, half of the tools in the accompanying toolkit cover MHPSS assessment issues relevant to other sectors as well as the health sector.
