In fact, the term itself comes from the Greek, meaning "wandering womb." We have since learned that this diagnosis had evolved from certain assumptions about women's social roles and mental characteristics, and is no longer in use. The modern equivalent of hysteria, however, may be borderline personality disorder, defined as "a pervasive pattern of instability of self-image, interpersonal relationships, and mood, beginning in early adulthood and present in a variety of contexts." This diagnosis is applied to women so much more often than to men that feminists have begun to raise important questions about the social, cultural, and even the medical assumptions underlying this "illness." Women are said to be "unstable" when they may be trying to reconcile often contradictory and conflicting social expectations. In Women and Borderline Personality Disorder, Janet Wirth-Cauchon presents a feminist cultural analysis of the notions of "unstable" selfhood found in case narratives of women diagnosed with borderline personality disorder. This exploration of contemporary post-Freudian psychoanalytic notions of the self as they apply to women's identity conflicts is an important contribution to the literature on social constructions of mental illness in women and feminist critiques of psychiatry in general. Janet Wirth-Cauchon is an associate professor of sociology at Drake University.

Book excerpt: Borderline personality disorder (BPD) is a major social problem about which little empirical research has been done. Individuals with this diagnosis, who are approximately 75% female, are highly stigmatized in society, do not readily respond to treatment, and are at very high risk for completed suicide. Between 8% and 10% of individuals, who currently have a diagnosis of BPD, will eventually commit suicide (APA, 2000; Gunderson, 2006; Oldham, 2006). This suicide rate is 50 times higher than the rate in the general population (Skodol, 2005; McKeown, Cuffe and Schultz, 2006). There is a near-consensus among treatment providers and researchers that BPD is still not well understood and that further research is needed. The aim of this qualitative study was to examine the subjective experience of women in treatment for BPD. Results indicated that the research participants identified with many of the diagnostic criteria for BPD described in the DSM-IV-TR (APA, 2000), as well as in other professional literature. Part of their narrated experience that has been less emphasized by the BPD treatment literature, however, was their subjective experience of shame. Shame has been defined in professional literature as "a global, negative evaluation of the entire self".

This collection of papers renews long-standing proposals for a dimensional model of personality disorder, describing alternative models, addressing questions about their clinical application and utility, and suggesting that future research seek to integrate such models within a common hierarchical structure. With contributions by preeminent researchers in the field, Dimensional Models of Personality Disorders is drawn from a conference series convened by APA, WHO, and NIH in order to plan for the fifth edition of the DSM. The Nomenclature Work Group concluded that consideration should be given to basing part or all of DSM-V on dimensions rather than categories, and recommended that a dimensional model for personality disorders should serve as a basis for exploring dimensional approaches in other areas. Among the topics addressed are Behavioral and molecular genetic research supporting an etiologically informed dimensional classification of personality disorders The as-yet tenuous associations between dimensional trait measures of personality as contained in the models of Cloninger, Depue, and Siever-Davis, and specific neurobiological measures, as examined in neurotransmitter research Potential links between childhood and adolescent temperament and personality dimensions and adult personality psychopathology Studies examining the covariation of personality dimensions across cultures The continuity of Axis I and Axis II disorders and a proposed hierarchical structure of mental disorders that integrates the psychopathology of Axis I disorders with specific personality traits The dual challenges of coverage and cutoffs that must be addressed if dimensional models are to be considered viable alternatives to the existing categorical diagnostic system Although the editors acknowledge that concerns are certain to be raised regarding conversion to a dimensional classification -- such as the disruption to clinical practice by a radical shift in diagnosing personality disorder -- these papers make a strong case for opening the field to alternative ways of enhancing clinical utility and improving the validity of basic classification concepts. Together, they offer stimulating insight into how we approach personality disorders, with the hope of encouraging a new model of diagnosis for DSM-V.
