Les complications chez ces personnes sont plus fréquentes, en partie par l'éloignement au parcours de soins recommandé par la HAS. Cette étude qualitative effectuée en Rhône-Alpes explore le point de vue des professionnels du médico-social quant à l'impact de la précarité sur les ruptures de parcours des patients diabétiques de type 2. Douze entretiens semi-dirigés ont été réalisés après inclusion des professionnels du médico-social participant au parcours de soins du patient diabétique de type 2 en situation de précarité. Les grandes thématiques de rupture de parcours de soin identifiées ont été associées aux différentes étapes du parcours de soin d'un patient. Le non-ressenti du besoin de soins, avec un rapport utilitariste au corps de ces patients, engendrait un retard ou une absence de demande de soins. Un rapport au temps particulier chez les personnes précaires avec le présent qui était privilégié par rapport au futur, ainsi que les différents déterminants socio-culturels freinent la démarche active pour entrer dans le soin. Les déterminants économiques et professionnels, notamment par leur impact sur la santé mentale engendraient un obstacle à l'accès aux soins. Les autres obstacles à l'accès étaient les déterminants structurels du système de santé français et le report de certains objectifs de santé par les professionnels. Dans le vécu de soins, l'incompréhension due à la distance sociale était une des causes des difficultés de la relation professionnel-patient. Les professionnels médico-sociaux souhaitent renforcer leur coopération en formation et en pratique de proximité pour faciliter le ressenti du besoin de soin par les patients et les maintenir dans le parcours de soin. L'approche centrée patient permet un renforcement de l'estime de soi des patients par une écoute privilégiée et une meilleure convergence des représentations soignants-soignés.

Si la santé connectée améliore la santé et la qualité de vie du patient diabétique, son arrivée massive et rapide laisse entrevoir une profonde mutation de la prise en charge future de ces patients mais aussi un impact majeur dans le système de santé de demain. Chacun devra trouver la juste place, le juste rôle dans un environnement en plein essor : patient ; professionnels de santé ; fabricants ; autorités et ce, pour assurer le juste équilibre permettant à la santé connectée de répondre à cet enjeu majeur de santé publique.

Although explicitly articulated by nineteenth-century philosophers like Mill, Whewell and Venn, it has a much older history dating back to Plato and Aristotle. In recent years, essentialism has been the dominant account of natural kinds among philosophers, but the essentialist view has encountered resistance, especially among naturalist metaphysicians and philosophers of science. Informed by detailed examination of classification in the natural and social sciences, this book argues against essentialism and for a naturalist account of natural kinds. By looking at case studies drawn from diverse scientific disciplines, from fluid mechanics to virology and polymer science to psychiatry, the author argues that natural kinds are nodes in causal networks. Instances of these impairments are expected to double by the year 2050, making low vision a significant public health issue. This Practice Guideline includes occupational therapy assessment and intervention guidelines for older adults who have visual acuity impairments, visual field impairments, or both as a result of a low vision diagnosis. Screening, referral, and evaluation are covered for clients for central visual field impairments, such as AMD and cataract; peripheral field impairments, such as glaucoma, hemianopsia, and quadrantanopsia; and mixed visual field loss, such as diabetic retinopathy. This work can help occupational therapists and occupational therapy assistants, as well as individuals who manage, reimburse, or set policy regarding occupational therapy services, understand the contribution of occupational therapy to treating older adults with low vision.
