Book excerpt: Ces dernières années, une augmentation régulière de la fréquentation des services d'accueil des urgences (SAU) a été relevée en France. La région Provence-Alpes-Côte d'Azur n'est pas épargnée par ce phénomène, elle relève une augmentation de la fréquentation des SAU de +22,5% entre 2008 et 2017. Nous avons décidé de nous intéresser particulièrement au SAU de la Timone car le temps de prise en charge (PEC) des patients dans ce centre est supérieur aux autres services de la région (5h08 versus 3h33 en moyenne en 2016). Ce délai supplémentaire de PEC peut porter préjudice au patient, nous nous attachons donc à rechercher des facteurs influençant le temps de PEC dans ce service. Leurs identifications permettraient de trouver des solutions permettant de le réduire. Matériel et méthodes : il s'agit d'une étude monocentrique rétrospective, réalisée du 27 mars au 2 avril 2017 au sein du SAU adultes de l'hôpital de la Timone à Marseille. Les dossiers étudiés sont ceux d'un quart des patients ayant un temps de prise en charge supérieur au 75ème percentile. Au total, des chronogrammes regroupant les différentes étapes de la PEC ont été réalisés sur 104 dossiers. Résultats : sur la semaine étudiée, 1610 admissions au SAU ont été réalisées avec une durée moyenne de PEC de 7h04, parmi elles ; 104 dossiers ont été étudiés. 2/3 des patients étudiés avaient un Classification Clinique des Maladies des Urgences ≥3, 93% ont nécessité l'utilisation du plateau technique de l'hôpital (examens biologiques, imagerie médicale et avis spécialisés (nécessaire chez 46% des patients)). On remarque que la durée de PEC est liée à la fréquentation du SAU et que le Médecin d'Accueil et d'Orientation permet de diminuer cette PEC. Conclusion : bien qu'aucun facteur unique n'ait été identifié, nous avons proposé des pistes de réflexion et d'amélioration pour optimiser le temps passé pour chaque étape de la PEC des patients.

Book excerpt: The most recent Ebola epidemic that began in late 2013 alerted the entire world to the gaps in infectious disease emergency preparedness and response. The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats. This publication summarizes the presentations and discussions from the workshop.
