Book excerpt: Introduction : Dans le cadre du stage ambulatoire SASPAS, les internes de médecine générale sont amenés à recevoir en consultation des enfants de 0 à 6 ans avec leur parent. L'objectif de cette étude était de comprendre l'expérience vécue par les parents lorsqu'ils sont reçus par des étudiants et de mieux cerner leurs attentes. Méthodes : Etude qualitative à partir d'entretiens semi dirigés, réalisés auprès de 15 parents d'enfant de 0 à 6 ans, reçus par un interne SASPAS entre janvier 2021 et juillet 2021. L'analyse des données s'est faite par théorisation ancrée, avec triangulation des données. Résultats : L'analyse du vécu des parents montre que ceux-ci accordent une forte importance au comportement de l'interne vis-à-vis de l'enfant notamment au fait qu'il veille à son confort et qu'il cherche à créer une relation personnelle avec l'enfant. L'interne montre des compétences en structurant la consultation, en réalisant un examen exhaustif et en faisant preuve de compétences communicationnelles. Les parents adhèrent à la formation des internes par le compagnonnage. Ils découvrent une pluralité des approches avec la rencontre de différents internes. Le lien de connaissance qui se noue progressivement et les capacités relationnelles de l'interne participent au fait que celui-ci est avant tout perçu comme un médecin. Discussion : Les attentes des parents concernant les compétences de l'interne sont en adéquation avec les objectifs pédagogiques du DES de médecine générale. Ils adhèrent au suivi de leur enfant par des internes dans la mesure où ils construisent un lien de connaissance qui permet d'établir une relation de confiance. Une consultation en supervision directe avec le maître de stage, d'un enfant suivi par l'interne pendant son stage, pourrait permettre d'évaluer les compétences de l'interne dans la gestion de la consultation pédiatrique et du suivi au long cours.

Son rôle lors de cet événement physiologique que constitue la grossesse est d'assurer la prévention, le dépistage, le diagnostic et le traitement des pathologies associées ainsi que l'orientation au sein du réseau de soins en cas de nécessité. Compte tenu de la démographie médicale actuelle et des besoins croissants de la population, les médecins généralistes vont être probablement de plus en plus sollicités pour prendre en charge le suivi des grossesses normales. Il semble donc primordial que les internes y soient préparés. C'est pourquoi le but de ce travail est d'évaluer le ressenti des internes de médecine générale sur leur niveau de compétence dans le suivi de grossesse à faible niveau de risque. Il s'agit d'une étude descriptive, quantitative par questionnaire, adressé aux internes inscrits en troisième année de DES de médecine générale ou étant en première année post internat à Lyon sur l'année universitaire 2020-2021. Nous avons obtenu 59 réponses soit un taux de participation de 20,6%. Le ressenti de la formation globale en GO à l'issue de l'internat est jugé majoritairement comme satisfaisante à 54,2% et à 33,9% comme peu satisfaisante. De multiples raisons ont été évoquées par les internes ayant un avis plutôt négatif. La majeure partie a fait remarquer qu'à leur sens le stage de gynécologie-obstétrique de la maquette du DES n'était pas adapté à la médecine générale. 67,5% des internes percevant leur formation en GO comme satisfaisante en fin de DES de médecine générale se sentent également compétents dans le suivi de grossesse à bas risque (p = 0,01). Malgré cela, il semblerait que des améliorations puissent être apportées afin d'optimiser la formation et de répondre aux attentes des internes. Dans cette perspective, des propositions d'amélioration ont été faites par les internes interrogés. Les plus plébiscitées étaient : proposer davantage de stages ambulatoires en santé de la femme, des formations théoriques complémentaires ciblées sur le sujet, faire un semestre entier en santé de la femme plutôt qu'un semestre couplé GO-pédiatrie, promouvoir dès le début de la formation des outils numériques utiles et fiables comme le site GESTACLIC. La formation au suivi de grossesse à bas risque répond aux besoins des internes de médecine générale mais pourrait être encore améliorée d'un point de vue pratique et théorique.

Déterminer les freins et les facteurs facilitant à lui confier la santé de leur(s) enfant(s) Méthode : Analyse qualitative de 16 entretiens semi-dirigés, réalisés du 01 juin au 03 novembre 2012, de parents d'enfants de 0 à 6 ans dans les Alpes-Maritimes. Résultats : Alors que certains parents ne ressentent aucun frein à confier la santé de leur(s) enfant(s) au médecin généraliste, d'autres évoquent des réticences liées à des conditions particulières (urgence, gravité ou spécificité de la pathologie ressentie, nécessité d'orientation de l'enfant vers un confrère spécialiste, nécessité d'une hospitalisation, enfant d'âge préscolaire, échec de traitement de 1ère intention). Les facteurs facilitant évoqués sont surtout d'ordre organisationnel (proximité, accessibilité, horaires et jours d'ouverture du cabinet médical, possibilité de visites à domicile et tarif conventionné). Lors de la prise en charge de leur(s) enfant(s), les parents ont de multiples attentes, telles que les conditions d'accueil du jeune enfant (espace réservé à l'enfant en salle d'attente, jouets mis à disposition, hygiène et décoration), les spécificités de la consultation pédiatrique elle-même (approche adaptée de l'enfant, examen médical systématique, clarté dans le diagnostic et la prescription) et l'attention particulière à l'établissement d'une relation de confiance entre les trois protagonistes. De nombreuses pistes d'amélioration ont été proposées : réduction du temps d'attente, espace adapté à l'enfant en salle d'attente, amélioration de l'approche de l'enfant, prise en charge de l'angoisse parentale. Conclusion : Afin d'améliorer la consultation de l'enfant chez le médecin généraliste, des solutions peuvent être envisagées passant par une réduction du temps d'attente, une amélioration de l'approche spécifique de l'enfant, une transparence dans les diagnostics et prescriptions et une réponse adaptée à l'angoisse parentale.

Résultats : Les parents expriment une globale satisfaction de la prise en charge des internes de médecine générale pour leur(s) enfant(s). Le principal contentement réside dans la richesse des explications données par les internes sur l'état de santé de l'enfant malade. Perçus dans la plupart des entretiens comme des « étudiant en cours de formation » et manquant d'expérience, la confiance des parents envers les internes reste fragile ; le recours au médecin traitant étant toujours privilégié au moindre doute ou incertitude parentale sur l'état de santé de l'enfant. De fait, la problématique de la pénurie de médecin généraliste dans le territoire sud-Manchois a souvent émergé et ainsi les internes de médecine générale ont fréquemment été assimilés à une offre de soins supplémentaire précieuse et rapide d'accès. Conclusion : La consultation pédiatrique par des internes de médecine générale est globalement bien perçues par les parents interrogés. Leur présence est saluée car permet une augmentation de l'offre de soins au sein des PSLA. Le manque d'expérience souligné par certains parents sera peut-être partiellement jugulé par l'augmentation du temps de formation des internes à un semestre dans les services de pédiatrie.

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
