L'élaboration d'un protocole de bilan a permis de comparer les compétences communicatives et langagières ainsi que les possibilités motrices de la sphère oro-faciale de deux groupes d'enfants I.M.C. âgés de 5 à 11 ans : l'un ayant bénéficié d'une éducation précoce avant 3 ans et l'autre non. Une analyse qualitative des résultats en fonction des différentes variables retenues (âge, niveau socioculturel, pathologie et prise en charge orthophonique précoce) ainsi que celle de plusieurs appariements d'enfants I.M.C. d'âges différents suggèrent que la précocité de l'intervention orthophonique a des répercussions positives sur certains domaines étudiés. Les résultats de notre recherche, même s'ils ne sont pas quantitatifs ni généralisables, nous confortent donc dans l'idée que les enfants I.M.C. tirent bénéfice d'une prise en charge orthophonique avant 3 ans.

Book excerpt: La problématique de l’enfant IMC/IMOC recouvre un champ vaste de domaines théoriques à maîtriser du fait du trouble moteur et de la multiplicité des troubles associés constatés. Les orthophonistes se doivent de savoir reconnaître et repérer les différents troubles exprimés par chaque enfant IMC/IMOC afin de mieux évaluer leur potentielle implication sur le développement de la sphère langagière. Nous avons objectivé, grâce à un questionnaire, la démarche évaluative des orthophonistes ainsi que les moyens d’investigation mis à leur disposition. Nous avons pu relever les réponses de 23,7% des équipes orthophoniques des institutions spécialisées pour déficients moteurs. Nous avons remarqué que le champ d’intervention des orthophonistes s’étend au-delà de l’évaluation de la sphère langagière. L’orthophoniste tient, au sein des établissements spécialisés, un rôle important dans l’évaluation des compétences neuro-motrices, mnésiques, visuo-attentionnelles et visuo-spatiales. Par conséquent, les outils d’évaluation doivent être optimaux. Nous avons pu mettre en évidence que ce n’était pas toujours le cas et nous avons défini les compétences pour lesquelles un manque d’outils évaluatifs est constaté. Nous avons observé que les relations internes à chaque établissement spécialisé entre les différents professionnels qui suivent l’enfant IMC/IMOC permettent de mieux apprécier ses capacités et démontrent ainsi toute la nécessité d’un travail en pluridisciplinarité. Nous avons également recueilli de nombreux commentaires concernant le poids majeur de l’expérience dans les différents temps d’évaluation de l’enfant IMC/IMOC, les insatisfactions liées aux moyens d’évaluation du langage et la diversité des adaptations qui en découle. Au fur et à mesure de notre propos, nous avons détaillé ces différents points, en essayant d’insister sur les pistes de réflexion à poursuivre et sur les axes d’intervention qu’il était nécessaire de développer. Notre travail a ainsi permis de faire un état des lieux de l’évaluation orthophonique de l’enfant IMC/IMOC. De plus, au vu des différents commentaires recueillis, il nous est apparu nécessaire d’ouvrir un forum de discussion concernant la problématique de l’enfant IMC/IMOC afin de faciliter la mise en commun des approches estudiantines et professionnelles. Nous espérons que ces échanges permettront la mise en place d’un travail en partenariat afin de faire évoluer la recherche clinique orthophonique, la prise en charge et l’évaluation autour de la problématique de l’enfant IMC/IMOC.
