Book excerpt: Introduction : la douleur aiguë est insuffisamment prise en charge chez le sujet âgé, du fait de difficultés d'évaluation et d'une tolérance modifiée aux traitements. La kétamine est un antagoniste des récepteurs N-méthyl-D-aspartate (NMDA) ayant des effets amnésiques, psychosensoriels, analgésiques et neuroprotecteurs. L'objectif de ce travail est de synthétiser l'état des connaissances issues de la recherche clinique sur l'efficacité antalgique et la tolérance de la kétamine à dose analgésique dans la douleur aiguë du sujet âgé. Méthode : nous avons effectué une revue systématique de littérature. L'ensemble des articles publiés jusqu'au 01/11/2021 a été recensé dans différentes bases de données en utilisant les mots-clés « aged » et « acute pain » associés au mot-clé « kétamine ». Les articles ont ensuite été triés et analysés à l'aide de l'outil Covidence. L'évaluation du risque de biais de chaque article a été réalisée avec la grille Cochrane Risk of Bias. La revue a été réalisée selon les recommandations PRISMA 2020. Résultats : quinze études ont été retenues dans la revue de littérature, dont trois études portant spécifiquement sur une population d'âge moyen égal ou supérieur à 65 ans. La compilation des données sur l'amélioration de l'Échelle Visuelle Analogique de douleur permet d'objectiver une différence moyenne significative de 0.8 points en faveur de la kétamine (p=0.02). Trois études montrent que les patients traités par kétamine ont significativement moins recours à un antalgique de secours par rapport à un comparateur. Les résultats concernant la survenue d'effets secondaires sont hétérogènes et de faible niveau de preuve. Discussion : cette étude est la première revue systématique de littérature s'intéressant à la kétamine dans la douleur aiguë du sujet âgé. Notre revue permet de conclure à un bon niveau de preuve de l'efficacité de la kétamine dans la douleur aiguë du sujet âgé malgré le faible nombre et l'hétérogénéité des études, mais la généralisation des résultats à une population gériatrique constituée de patients présentant des troubles cognitifs ne peut être attestée. Des recherches futures s'intéressant aux effets secondaires spécifiques du sujet âgé permettraient d'assurer une meilleure généralisation des résultats et potentiellement de mieux mettre en valeur l'apport de la kétamine dans cette population fragile.
