Book excerpt: Dans un contexte économique contraint et de vieillissement de la population, il est nécessaire de valoriser l'activité du pharmacien clinicien chez le sujet âgé. L'outil de valorisation tridimensionnelle Clinique, Economique et Organisationnelle : CLEO à été élaboré pour répondre à ce besoin. L'objectif de cette thèse est d'étudier sa validité dans le domaine du vieillissement. La validation externe a été conduite sur 93 IP de court séjour gériatrique en comparant, pour chaque dimension de l'outil, la cotation des pharmaciens à celle d'un groupe expert. La validation interne a été réalisée sur 247 IP issues 3 types catégories médico-tarifaires : CSG, Hospitalisation de jour (HDJ) et Soins de suite et réadaptation (SSR). Elle a consisté à comparer les cotations entre 2 pharmaciens. La fiabilité externe mesurée entre le groupe expert et le pharmacien ayant le plus d'expérience dans le domaine du vieillissement, est considéré comme modéré pour chacune des 3 dimensions : clinique (kp= 0,46), économique (kp= 0,59) et organisationnelle (kp= 0,51). La fiabilité interne globale est considérée comme mauvaise pour la dimension clinique (kp= 0,24) et bonne pour les dimensions économique et organisationnelle (kp= 0,68 et 0,63). Au vu des résultats, l'utilisation de CLEO paraît possible en CSG par des pharmaciens séniors. Cependant, il existe de grandes divergences, qu'elles soient internes ou externes, dans les cotations d'impacts potentiels notamment cliniques des IP. Il est nécessaire de comparer les cotations d'un plus grand nombre de pharmaciens plus particulièrement en HDJ et SSR, de trouver les sources de divergences avant de pouvoir utiliser CLEO en routine.

En effet, en France, celui-ci ne peut actuellement pas réaliser d'analyses pharmaceutiques supérieures au niveau 1. Notre étude s'est intéressée à l'impact que peut avoir la consultation des résultats de biologie par le pharmacien d'officine. L'étude montre un taux d'intervention pharmaceutique légèrement supérieur à 21%. Le taux d'acceptation des interventions est proche de 80% pour les interventions transmises au prescripteur et égal à 100% lorsque l'intervention concerne directement le patient. Enfin, avec l'outil d'accès aux biologies proposé dans notre étude 15% des pharmaciens demanderaient toujours la biologie et 47% la consulteraient souvent. Enfin, plus que 7% des pharmaciens ne consulteraient jamais la biologie, là où ils sont actuellement 25%.
