Pour cela, nous avons recueilli et analysé des enregistrements vocaux ainsi que des données sur la qualité de vie de 6 patients. Des analyses objectives de la qualité vocale ont été réalisées, et un questionnaire d'évaluation de la qualité de vie a été renseigné par les patients eux-mêmes. Les résultats obtenus laissent apparaître l'existence de troubles vocaux avec un léger abaissement de certaines données acoustiques, notamment la Fo. L'analyse des conséquences vocales met en évidence l'importance des évaluations acoustiques de la voix pré et post opératoires. L'analyse des conséquences sociales met en évidence l'importance du Voice Handicap Index et de l'intérêt porté au ressenti du patient. Les conséquences vocales et sociales mettent en avant l'importance de la rééducation orthophonique.

Book excerpt: Objectif - Évaluer la survie et la qualité vocale chez les patients opérés d'un cancer de la thyroïde avec résection de nécessité du nerf laryngé inférieur. Méthodes - Étude rétrospective descriptive incluant tous les patients opérés à l'institut Gustave Roussy de Septembre 2005 à Janvier 2020 d'un cancer thyroïdien avec résection du nerf récurrent. Évaluation de la survie globale et de la survie sans récidive. Évaluation de la qualité vocale et de la déglutition avec l'EORTC Head Neck 35 et le VHI. Les adultes et les enfants ont été étudiés séparément. Résultats - 54 patients ont été inclus (46 adultes dont 39 carcinomes différenciés et 7 carcinomes médullaires, et 8 enfants ayant un carcinome papillaire), avec un âge moyen de 61,2 ans (IC95%{56,4-66,0}) pour les adultes, 12,25 ans (IC95% {9,9- 14,6}) pour les enfants. 31 adultes (66%) et 7 enfants (87%) n'avaient pas de paralysie récurentielle préopératoire. Le suivi moyen était de 51 mois pour les adultes, 71 mois pour les enfants. La survie à 5 ans était de 86% (73-100) pour les adultes présentant un carcinome différencié, aucun enfant n'est décédé au cours du suivi. La survie sans récidive à 5 ans était de 38% {IC95% (23-61)} pour les adultes ayant un carcinome différencié (avec 36% de progression locorégionale et 68% de progression métastatique). La survie des 7 patients atteints d'un carcinome médullaire n'a pas pu être évaluée par la méthode de Kaplan-Meier. La qualité vocale était mesurée subjectivement avec un score VHI moyen de 19,7/40 pour la population adulte. L'évaluation de la qualité de déglutition a retrouvé un score EORTC H&N 35 de 60/130. Seulement 5 patients ont souhaité une médialisation de corde vocale. Conclusion - La résection du nerf récurrent pour raison carcinologique semble permettre un bon contrôle local du cancer sans altération majeure de la qualité de vie en ce qui concerne la voix et la déglutition.

L'évaluation vocale, réalisée 1 mois avant et 3 mois après la chirurgie, était subjective (Voice Handicap Index 10 et GRBAS) et objective (Dysphonia Severity Index). Parmi les 59 patients inclus, aucune altération significative des paramètres vocaux subjectifs et objectifs étudiés n'a été observée après la chirurgie. Le Voice Handicap Index 10 a évolué de 3,39 [0,00-29,00] à 2,90 [0,00-29,00] (p = 0,62) ; le GRBAS de 0,64 [0,00-3,00] à 0,73 [0,00-4,00] (p = 0,57) ; le Dysphonia Severity index de 8,47 [4,10-10,24] à 8,31 [5,59-11,62] (p = 0,42). L'âge supérieur à 65 ans, un score préopératoire supérieur ou égal à 3 au Voice Handicap Index 10 et la réintervention thyroïdienne étaient des facteurs prédictifs d'altération vocale postopératoire. La qualité vocale, analysée de manière objective et subjective, trois mois après une thyroïdectomie, n'était pas altérée par rapport à celle préopératoire.
