Book excerpt: Introduction : les pratiques d'automédication dépendent de nombreux déterminants autour de la santé dont la représentation qu'a l'individu de l'objet médicament. La France reste le premier pays consommateur de médicaments en Europe mais paradoxalement, l'engouement pour cet objet diminue. A l'inverse, la vente de médicaments non allopathiques est en plein essor. L'objectif de ce travail est donc d'observer les représentations qu'ont les individus des médicaments et des médecines alternatives et complémentaires et d'évaluer les facteurs influençant ces représentations. Matériels et méthodes : il s'agit de l'analyse secondaire d'une étude qualitative par entretiens semi-dirigés couplés à des journaux de santé de 44 patients de Loire-Atlantique et de Vendée. Résultats : l'objet médicament suscite de nombreuses controverses. Considéré indispensable au soulagement ou à la guérison pour certains, il est également jugé toxique, voire destructeur pour d'autres. Parmi les réticences à la prise médicamenteuse, on retrouve la crainte de l'inefficacité, de la toxicité, ou encore de l'accoutumance. Les MAC sont consommées avant tout dans un but préventif, elles s'approprient une prise en charge holistique, centrée sur la personne. En se rapprochant du naturel, elles ne sont pas considérées comme des médicaments et comportent moins d'effets secondaires. Selon ces représentations, les individus vont choisir l'une de ces solutions ou combiner différentes options, dans un désir d'autonomie. Ce « bricolage thérapeutique » est influencé par l'entourage familial et amical, les médias et les représentations personnelles du milieu médical. Les patients ont des réticences à se confier à leur médecin traitant sur ce pluralisme thérapeutique. Conclusion : les représentations d'un individu du médicament et des MAC influencent son comportement en termes d'automédication et l'incitent à utiliser l'une ou l'autre de ces thérapies. Les professionnels de santé devraient questionner leurs patients sur ces représentations pour personnaliser leur prise en charge, la coordonner et remettre le patient au centre du soin.

Résultats : Les patients sont en demande de références en termes de connaissances et de conseils d'utilisations des produits de santé alternatifs, et souhaitent une intégration de ces médecines alternative en complément de la médecine conventionnelle qui leur est proposée. Conclusion : Le médecin généraliste, de part son statut est au cœur de l'évaluation de ces pratiques de soins alternatives, il lui appartient de conseiller ou mettre en garde les patients concernant les médecines alternatives. Un complément de formation dans ces domaines permettrait au médecin généraliste de répondre au mieux à sa mission de prévention et de soins attendue par les patients.

Les médecins sont souvent réticents à utiliser ces pratiques en dépit de l'engouement des patients. L'objectif de notre étude est de définir d'une part le taux de consommation de MAC chez les patients consultant en cabinet de médecine-générale en Franche-Comté, et d'autre part de déterminer leurs profils et de comprendre leurs motivations, et leurs attentes.Méthode : Nous avons réalisé 2 études : une étude observationnelle descriptive transversale par auto-questionnaire remis aux patients consultant dans un échantillon de cabinets de médecine générale franc-comtois tirés au sort, et 'une étude qualitative inspirée de la théorisation ancrée par 10 entretiens semi-dirigés.Résultats : Partie quantitative : 802 questionnaires ont été analysés. 74,9% des patients affirment avoir déjà eu recours aux MAC. Les patients consommateurs de MAC étaient plus volontairement des femmes, entre 30 et 60 ans, cadres et professions intellectuelles supérieures, employées, ou professions intermédiaires et vivant en milieu rural. Les MAC les plus citées étaient l'ostéopathie l'homéopathie et l'acupuncture.Conclusion : Nos résultats confirment l'engouement des patients pour les MAC. Ils restent attachés à la médecine conventionnelle mais ont une volonté d'ouverture. Ils revendiquent une évolution de la médecine vers la médecine intégrative.
