Book excerpt: La période de l'adolescence allant de 15 ans-3 mois à 18 ans est une étape charnière dans le développement de tout individu située à la jonction de deux systèmes de santé (enfant/adulte). Cette étude a pour objectif de caractériser la demande de soin de ces adolescents et de tenter de déterminer l'intérêt d'un accueil pédiatrique. Il s'agit d'une enquête rétrospective effectuée aux urgences pédiatriques d'un hôpital de banlieue parisienne acceptant depuis peu les patients jusqu'à 18 ans. Sur une durée de 6 mois consécutifs, 930 dossiers de patients ont été analysés. Les accidents sont à l'origine de la majorité des passages aux urgences (53,5%) et sont pris en charge en traumatologie. On trouve ensuite les pathologies somatiques aigues (39,2%), généralement bénignes, et les pathologies psychiatriques ou psychologiques (6,8%). Les résultats de notre étude concordent avec ceux des travaux antérieurs. Ces adolescents ont du mal à se situer dans le système de santé et ont tendance à utiliser les urgences comme source de soins primaire (horaires et jours de passages, motifs de consultation...). Leur prise en charge nécessite des compétences multidisciplinaires et requiert une approche globale du patient. Il serait judicieux d'aligner les services d'urgences pédiatriques français sur une limite de prise en charge de 18 ans non seulement pour assurer une certaine conformité européenne mais aussi car le pédiatre, plus sensibilisé à une approche médicale et psychique du patient que son homologue urgentiste, semble être le plus à même de répondre aux demandes des adolescents. En ville, proposer le choix d'un médecin référent dès 15 ans, pédiatre ou généraliste, favoriserait peut être qu'il soit reconnu par l'adolescent comme le premier maillon de la chaîne de soins. Il faut améliorer la formation du personnel soignant à l'accueil des adolescents.

Un questionnaire rempli d'après les dossiers archivés recherchait des données administratives, médicales et sociales. Nous avons étudié 1247 dossiers, moyenne d'âge de 16,6 +- 0,8 ans, 51% de filles. Les principaux motifs de passage étaient: traumatologie (54,7%); infectieux (11,8%) et digestifs (9,4%). Les conduites à risque ont été recherchées dans 4,3% des dossiers. 5,8% des patients présentaient des problèmes psychologiques. Les pathologies et la prise en charge de l'adolescent ne variaient pas selon le sexe, le secteur d'admission, excepté un plus fort taux de garçon en traumatologie. Les facteurs de risque devraient être plus recherchés chez l'adolescent dès les urgences afin de réduire les morbidité et mortalité qui y sont liées
