Book excerpt: La diminution du nombre de gynécologues médicaux va amener les médecins généralistes à prendre en charge de plus en plus de patientes sur le plan gynécologique. La formation à la réalisation des gestes gynécologiques fait partie intégrante de la formation des internes de médecine générale au cours de leur DES. L'objectif de cette étude est de déterminer quels facteurs peuvent influencer leur décision de réaliser ou non ces 3 gestes dans leur pratique future. L'objectif secondaire est de proposer des solutions afin d'améliorer leur formation et de développer leur intérêt pour cette pratique. Matériel et méthode : Il s'agit d'une étude quantitative, descriptive, transversale, réalisée au moyen d'un questionnaire électronique. Le mail d'invitation au questionnaire a été adressé en Décembre 2017 et Juin 2018 à 2 promotions d'internes et à ceux en année de thèse (soit 286 internes). Résultats : Le taux de réponse au questionnaire était de 28,7% (82 internes) dont 75,6% de femmes et 24,4% d'hommes. Le taux d'acquisition des gestes en fin d'internat était de 92,1% pour le FCU, 82,4% pour la pose et retrait d'implant contraceptif et 52,8% pour la pose et retrait de DIU. Les femmes étaient significativement plus nombreuses à souhaiter réaliser des FCU ainsi que des poses et retraits d'implant contraceptif. La réalisation d'un stage en gynécologie hospitalière pendant l'internat était significativement associée à une meilleure acquisition des gestes « pose et retrait d'implant contraceptif et de DIU ». Conclusion : Si la formation des internes de médecine générale à la réalisation des FCU est satisfaisante, leur formation à la pose et au retrait d'implant contraceptif et de DIU est insuffisante. Cet apprentissage nécessite une formation à la fois théorique, grâce aux ateliers de simulation sur mannequin, et pratique lors des stages hospitaliers et ambulatoires. Ces derniers devant être organisés pour y intégrer l'apprentissage systématique des gestes gynécologiques.

Book excerpt: Introduction: A l'issue de l'internat de médecine générale, certains internes expriment leurs réticences à pratiquer la gynécologie dans leur futur exercice. Ce travail a pour but de lister les déterminants de la pratique de la gynécologie de routine et du suivi des femmes enceintes en soins primaires dans l'exercice futur des internes et d'évaluer si leur formation initiale les prépare suffisamment à cela. Matériel et Méthodes: Enquête qualitative par focus group. 3 focus group ont eu lieu de mai à juillet 2013. Les internes interrogés étaient en 3e année de DES voire plus, ayant déjà effectué un stage en gynécologie et éventuellement suivi une fonnation complémentaire en gynécologie-obstétrique. Résultats : Les motivations et les freins de cette pratique sont de plusieurs ordres. La formation universitaire reste incontournable, même si les enquêtés déplorent souvent l'inadaptation des stages hospitaliers; les stages professionnalisant type SASPAS sont, quant à eux., très sollicités, permettant une activité plus proche de ce qui sera vu en médecine ambulatoire. La formation théorique semble insuffisante pour certains; ainsi il existe une réelle volonté d'approfondir leurs connaissances. Les caractéristiques de la médecine générale tendent à orienter l'interne vers cette pratique tout comme la problématique de la démographie médicale. A l'inverse, ils sont freinés par des compétences limités et souvent naturellement attribuées aux spécialistes. par une responsabilité accrue et la complexité des actes. Il persiste aussi des freins d'ordre émotionnel. Enfin, le projet professionnel, propre à chacun, reste un déterminant personnel essentiel à la pratique de cet exercice. Conclusion : La pratique de la gynécologie et du suivi de la femme enceinte est régie par de nombreux déterminants positifs et négatifs. Néanmoins, la majorité des internes interrogés se sentent concernée par cette discipline, même s'ils ne s'estiment pas tous prêts à l'exercer régulièrement et entièrement dans un futur proche

Le stage en surnombre a été créé dans l'optique de favoriser le bon déroulement de la grossesse d'une interne et de préserver l'organisation du service. Il peut être validant ou non validant selon sa durée prévisible. L'objectif principal de cette étude est de déterminer si le stage en surnombre est un facteur protecteur de la survenue d'une MAP chez les internes de médecine générale inscrites en 2016-2017 dans une faculté du G4. MATERIEL ET METHODE : Une étude de cohorte rétrospective a été menée chez les internes de médecine générale du G4 ayant présenté une grossesse menée jusqu'à 22 semaines d'aménorrhée (SA) au moins et s'étant entièrement déroulée pendant l'internat. Le risque relatif de MAP a été calculé suivant le dernier type de stage effectué pendant la grossesse : stage normal, surnombre validant ou non validant. La MAP était définie comme la survenue entre 22 et 36 SA révolues de contractions utérines régulières et douloureuses associées à des modifications cervicales cliniques et/ou échographiques. RESULTATS : Le risque relatif de MAP associé au stage normal est de 1,31 avec IC95%[0,31;6,04]. Celui associé au surnombre validant est de 1,21 avec IC95%[0,22;7,02] et celui associé au surnombre non validant est de 0,62 avec IC95% [0,11;3,17]. CONCLUSION : Selon notre hypothèse et la tendance des résultats obtenus, seul le surnombre non validant pourrait réduire le risque de MAP. L'enjeu de la validation du stage pourrait en partie expliquer la différence de risque mais des études complémentaires sont nécessaires.
