L'utilisation d'applications médicales mobiles est une avancée pour les médecins et internes en médecine générale dans leur exercice quotidien. Objectif : déterminer la place du Smartphone, à travers les applications médicales mobiles les plus utilisées par les internes en médecine générale (IMG) et les médecins généralistes (MG) d'Île-de-France, et comparer leur utilisation. Méthode : étude d'observation descriptive réalisée du 1er février au 1er mai 2015 à l'aide d'un questionnaire en ligne adressé aux IMG et MG d'Île-de-France. Résultats : les réponses de 169 IMG et 110 MG ont été analysées. Une grande majorité des IMG (99,40 %) et des MG (88,20 %) possèdent un Smartphone, ainsi que des applications médicales mobiles (79,40% des IMG, 72% des MG). Il n'y a pas de différence significative entre l'utilisation des applications médicales mobiles chez les IMG et chez les MG. Les applications les plus utilisées sont les guides thérapeutiques, le calcul de scores, et les fiches pratiques de synthèse. Les IMG et les MG désirent voir développer des applications regroupant les nouvelles recommandations et les dossiers médicaux des patients, et des applications mettant en lien, différents correspondants médicaux. 82,84 % des IMG et 54,54 % des MG considèrent que le Smartphone et les applications médicales mobiles seront une aide importante pour leur pratique médicale future. Conclusion : les IMG et les MG possèdent et utilisent de nombreuses applications médicales mobiles. Ces différentes applications concourent à réaliser une prise en charge la plus globale possible des patients. Il serait pertinent de développer de nouvelles applications médicales mobiles plus adaptées à la demande des professionnels, ainsi qu'une accréditation ou certification de ces applications, dans une démarche de qualité et de transparence.

C'est dans ce contexte qu'une réflexion sur la place du Smartphone a été entreprise, dans deux thèses menées conjointement. Objectif : Explorer la place du Smartphone en soins primaires au cours du suivi des plaies chroniques dans la pratique de l'infirmier libéral. Identifier les modalités et les difficultés de la prise en charge ambulatoire dans un second temps. Méthodes : Étude qualitative exploratoire par entretiens semi-dirigés auprès d'Infirmiers Libéraux Diplômés d'Etat (IDE) exerçant en région Nord-Pas de Calais entre novembre 2013 et février 2014, menée en utilisant une approche par théorisation ancrée. Retranscription puis double codage informatique des données. Résultats : Le Smartphone est bien implanté dans la population des IDE. Sa place en pratique quotidienne dans le suivi des plaies chroniques est plutôt disparate, le Smartphone étant devenu pratiquement indispensable pour certains alors qu'il reste accessoire pour d'autres. Il a aussi des usages diversifiés, allant de l'usage téléphonique simple à un suivi iconographique des plaies réalisé et transmis au médecin généraliste ou à un IDE associé via le Smartphone. L'avantage principal pour les IDE réside dans la possibilité de réaliser un suivi chronologique et iconographique des plaies chroniques avec un outil simple, pratique, rapide et connecté. Des difficultés ont été soulevées concernant la prise en charge de la plaie en elle même, la collaboration avec le médecin généraliste et la relation avec le patient. Conclusion : Les IDE estiment que le Smartphone a sa place dans le suivi des plaies chroniques. Ils l'utilisent déjà entre IDE associés et espèrent pouvoir élargir son champ d'action au médecin généraliste. Cette étude a soulevé un avenir potentiel pour le Smartphone dans la prise en charge des plaies chroniques en ambulatoire.

In Reflexive Translation Studies, Silvia Kadiu investigates the viability of theories that seek to empower translation by making visible its transformative dimension; for example, by championing the visibility of the translating subject, the translator’s right to creativity, the supremacy of human translation or an autonomous study of translation. Inspired by Derrida’s deconstructive thinking, Kadiu presents practical ways of challenging theories that argue reflexivity is the only way of developing an ethical translation. She questions the capacity of reflexivity to counteract the power relations at play in translation (between minor and dominant languages, for example) and problematises affirmative claims about (self-)knowledge by using translation itself as a process of critical reflection.
