Book excerpt: Excerpt from Essentials Physical Diagnosis of the Thorax The first edition of this book, published under the title "Outline of Physical Diagnosis of the Thorax," was chiefly intended to meet the immediate wants of my classes. From its rapid distribution it has seemed to have reached a wider field. The present edition under the new title, as published by Mr. Saunders, is a revision of the original text, with an added section setting forth the signs found in each disease of the chest. In the preparation of this synopsis I have availed myself of the works of the best writers upon Diagnosis, General Medicine, Physiology, and Anatomy, from which I have endeavored to cull the essentials of the subject in hand. To Drs. Wm. R. Parkes and John Edwin Rhodes I desire to express my thanks for their valued services rendered in the reading of the proof. Book excerpt: Excerpt from The Essentials of Physical Diagnosis of the Chest and Abdomen Last summer, when 1 bad occasion to take my Ward Tutorial Class under my own care, I had a difficulty in referring the Junior Students to a simple concise work on Physical Diagnosis, that would take the place, to some extent, of the Clinical Lecture, and leave me more time for Practical Demonstration. I was induced for this reason to write the present text-book, in the hope that those beginning the study of Medicine might find it of some service. The paragraphs in smaller type may be read at a later stage, as they deal chiefly with what is matter of opinion. Book excerpt: Excerpt from Essentials of the Physical Diagnosis of Thoracic Diseases On the right side in this region the thin sharp border of the lung very nearly corresponds to the inferior bound ary. But below the lower border of fifth rib the thin portion of lung overlies the liver. The liver is sometimes pushed up into fourth intercostal space. On the left side a section is taken out of lung to afford space for the heart. The lung extends in front as far as the fourth sterno-costal articulation, where the anterior border passes outward to the outer end of the fifth cartilage, then obliquely inward to lower border of outer end of the sixth costal cartilage, then outward. Book excerpt: Excerpt from Diseases of the Chest and the Principles of Physical Diagnosis To write a practical book on the physical diagnosis of the heart and lungs in health and disease, has been the aim of the authors. We have attempted to omit everything not of practical diagnostic use, and to condense methods of secondary importance. More than is the custom has been written on the subject of diagnostic acoustics, because we believe that only through the comprehension of the laws of sound production and transmission can the results of percussion and auscultation be intelligently interpreted. We have endeavored to teach as much as possible by means of illustrations. Of these, many are photographs of frozen sections from the cadaver, previously hardened in formalin, so that the anatomic relations of the tissues remain as during life. We have found these specimens invaluable in our own teaching; and the book has been written in the hope that the photographic reproductions of our sections may be useful to others. We take pleasure in acknowledging our great indebtedness to Dr. George Fetterolf for the frozen sections; to Dr. J. Claxton Gittings for assistance in the preparation of the Section on Pediatrics; to Dr. Edward B. Krumbhaar for the Chapter dealing with the electrocardiogram; to Professor Richard Geigel, and to Dr. Charles M. Montgomery, from whose articles upon acoustics in diagnosis, much information has been gleaned. We are also indebted to Miss Eleanor A. Cantner for the production of some of our drawings. We are further under obligations to Professor William M. L. Coplin for permission to photograph numerous specimens in the Museum of Pathology of the Jefferson Medical College, as well as to Dr. David R. Bowen and to Dr. Henry K. Pancoast for our radiograms. Book excerpt: Excerpt from A Handbook of Physical Diagnosis: Comprising the Throat, Thorax, and Abdomen The design of this handbook is to present a concise description of the various methods pursued in the clinical examination of the thoracic and abdominal organs in health and disease and an estimate of the diagnostic value of the results so obtained; the examination of the Larynx is treated of in an Appendix. The general plan of the work, both as regards the order in which the different methods of exploring each organ are discussed and the account of the conditions likely to be met with, is naturally that of a systematic, scientific inquiry. Our description, however, particularly when dealing with the purely physical methods of examination, will be found to vary considerably in form and fulness, according as groups of similar phenomena, belonging possibly to different diseases, do or do not admit of being explained by com, ?non causes; in the latter case a detailed account of the symptoms has been given. Whenever the nature of the subject permitted, I have endeavored to preserve a certain uniform order also in the arrangement of details, taking up first the signs obtained by each method of exploration, then their causes, and lastly the physiological and pathological conditions, and the diseases, in which they occur. In this way only was it practicable to include within our relatively narrow limits the large number of pathological phenomena elicited by the various modes of clinical examination. That an account of this kind, indeed, should not be absolutely complete, that it should fail to take (cognizance of every deviation of the symptoms from the fundamental pathological type, is unavoidable. Book excerpt: Excerpt from Physical Diagnosis of Diseases of the Chest This book is intended for students and, so far as I am aware, contains nothing original. I have written it because I have not been able to find any small work upon the subject which does not contain glaring errors. The correct books are too large; the small books are out of date and repeat such well-worn myths as that the aortic second sound is normally louder than the pulmonic second, that aortic regurgitant murmurs are usually best heard in the sec ond right interspace, that a hypertrophied left auricle can produce dulness and pulsation near the left sternal border, that systolic re traction at the cardiac apex means adherent pericardium, that epi gastric pulsation denotes hypertrophy of the right ventricle, e'tc. Further, none of the smaller text-books contains any adequate ac count of muscle sounds, of pulmonary atelectasis, or of adherent pericardium. To record the well-known but often forgotten truth on such matters as these has seemed to me of importance in small books as well as in encyclopaedic treatises. The diagrams illustrating respiratory types are modifications of those used by Wylie and Sahli. I am indebted to Mr. Eliot Alden, of the Harvard Medical School, for his kind assistance in the preparation of the illustra tions and to Drs. E. C. Bradford and R. W. Lovett for permission to use three cuts from their well-known work on orthopedic sur gery. I am also indebted to the editor of the Archives of the Rontgen Ray for permission to use two radiographs from that journal. This book was released on 2010 with total page 728 pages. Available in PDF, EPUB and Kindle. Book excerpt: This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.
