In suitable cases more permanent results are secured by divulsion or stretching of the bands than by cutting them. In some cases the vibrations of the ossicular chain are interfered with by an adhesion of the remains of the drum-head to the promontory in such a manner as to bind down the malleus-handle. Under such circumstances division of the adhesion is followed by improved hearing and decreased tinnitus. In most instances divulsion of intra-tympanic bands and adhesions will have to be repeated from time to time in order to secure permanent results; but as the operations are by no means formidable, they may be done during an ordinary office visit when required. In the divulsion of bands and adhesions care should be exercised that sufficient force is not employed to endanger dislocation of one of the ossicles, more especially the stapes.
