Available in PDF, EPUB and Kindle. Book excerpt: A comprehensive text of intensive care would readily fill the equivalent of the Shorter Oxford English Dictionary. This is because the diseases treated are both numerous and varied; thus the patient can be medical, surgical, trauma or obstetric. It follows that the sum total of knowledge which needs to be available is truly encyclopaedic. This compact volume represents only a fragment of such information. The contributors were chosen because of their experience and because their methods were well-tried. The text therefore summarizes the best of current therapy and includes the controversial. The contributors come from four countries, adding an international flavour. One topic - The Recovery Room - outside the confines of intensive care has been included for two reasons. The recovery room is an important but neglected aspect of care, and it also seems important to define its relationships with the intensive care unit. It is hoped that the book will help the nurses and doctors involved in intensive care and, therefore, the patient. ERIC SHERWOOD JONES Vll Contributors P. M. ASHWORTH K. Available in PDF, EPUB and Kindle. Book excerpt: The increasing number of pacemaker patients correlates with the number of pacemaker meetings that attract physicians and medical engineers entering this expanding branch of medicine. The Pacemaker Colloquium organised in Brussels April 21st and 22nd 1977 was such a meeting. It was different from previous meetings as it had no surveys or summaries on patient series but instead it focused on controversial subjects that have emerged from pacing in the past and on new developments for the future. Besides controversial papers on indications for temporary and long-term pacing, new technical developments were presented in Brussels, where for the first time the important lithium battery manufacturers demonstrated their data. The surveys on electrodes, especially the fixation mechanisms and the atrial electrodes, initiated an animated discussion. This all converged in a Round Table on the choice of the electrode-pacemaker combination. Pacemaker electrocar diography and the application of cardiac stimulation in various types oftachyar rhythmias formed two clinically oriented sessions. Mter a survey ofthe various follow-up systems a series of presentations deals for the first time with the possibilities and difficulties of the application of com puters in cardiac pacing. A Round Table on the organisation of the Pacemaker clinic closed this friendly and animated meeting. It is our sincere hope that the publication of the presented papers and dis cussions in these Proceedings will help all those active in cardiac pacing to answer some of their problems and to improve the treatment of pacemaker patients.

Available in PDF, EPUB and Kindle. Book excerpt: The Pace of Fiction redefines the literary history of the novel by analyzing its most elaborate feature: its pace. It moves from the rise of the novel to realism and modernism. It starts by tracing the evolution of two narrative units: scenes (shown slowly) and summaries (told swiftly). These units emerge from the conflict of epic and drama, gain shape in the commentaries of Fielding and Goethe, and become dynamically opposed in nineteenth-century realism. In Middlemarch, they rotate in regular sequence: summaries move swiftly until scenes slow them down; scenes play out dramatically until summaries sweep them forward; their movement imitates the conflict of fate and free will. Over the course of the nineteenth century, however, scenic impulses overtake summary storytelling. The reader sees the tendency already in Austen's dialogues, Hawthorne's tableaux, or Balzac's battering drama, and finds it in Jane Eyre's placement of summaries in private scenes. When Flaubert extends scenic vividness to all of his summaries, and when Henry James subordinates his summaries to scenic consciousness, the extreme pressure of scene upon summary brings the opposition of realist pacing to collapse. But other oppositions arise in the modernisms that follow. In the alternation of stasis and kinesis, of drifting thoughts and everyday actions, of stories and acts of storytelling--in Proust, Joyce, Woolf, Mann, Hemingway--pace gathers and creates meaning in new ways.

Book excerpt: Children and adolescents with moderate and severe disabilities often have communication challenges that lead them to use problem behavior to convey their desires. This is the most comprehensive contemporary volume on functional communication training (FCT)--the individualized instructional approach that teaches a child socially acceptable communicative alternatives to aggression, tantrums, self-injury, and other unconventional behaviors. The expert authors provide accessible, empirically based guidelines for implementing FCT, and tips for overcoming obstacles. Grounded in the principles of applied behavior analysis, the book includes detailed strategies for developing a support plan, together with illustrative case examples. ÿ

Book excerpt: "A pitch-black global thriller that is nevertheless supremely intimate." -- Kirkus Review A mysterious Front, originating in Scotland and slowly expanding outward, threatens humanity's existence. In defiance of the known laws of physics, it only kills humans and is otherwise undetectable. Panic-stricken nations struggle against both the advancing menace and a tide of civil unrest. Desperate individuals must find their own paths, and find them quickly. Although very slow, the Front can be deceptively difficult to escape. Mankind has only a few years to pool its rapidly dwindling resources and save itself from extinction.
