Book excerpt: The first laparoscopic cholecystectomy, performed by Prof. Philippe Mouret in 1987 and described by himself in the first chapter of this book, was an event that revolutionized surgery in the past few decades. Although the majority of surgeons today are unfamiliar with the his- ry of early minimally invasive surgery developments, it is important to realize that the advent of laparoscopy led not only to new surgical te- niques, but also to a change in the doctrine of medical care, by streng- ening the concept of minimal invasiveness. This is particularly the case for biliary lithiasis, for which laparoscopy has provided major benefits in terms of both diagnosis and surgical tre- ment. However, our understanding of the etiology of this disease, as well as the availability of effective medical therapies, have remained subst- tially unchanged over time. The application of the innovative approach has not been, in routine practice, without problems: between 1990 and 2000, the rate of serious complications from laparoscopic cholecystectomy were four times higher than those observed with open surgery. Even initial attempts at lapa- scopic treatment of biliary common bile duct stones resulted in excessive complications and associated medico-legal problems. Nonetheless, today, most indications have been stated, techniques have been standardized, and a new aspect to the treatment of biliary lithiasis has appeared.

In addition, the book addresses a number of related issues including thick walled gall bladder, gall bladder cancer with surgical obstructive jaundice, incidental gall bladder cancer, the role and place of common bile duct excision, the Japanese aggressive surgical approach, management of asymptomatic gall stones, etc. An authoritative work that provides detailed insights into various aspects of gall bladder cancer and its management, the book offers a valuable resource for physicians in high-incidence areas and low-incidence areas alike. It is richly illustrated throughout with radiographs (US, CT, MRI, etc.) and operative and specimen photos.

Available in PDF, EPUB and Kindle. Book excerpt: Exciting major changes have occurred in the understanding and treatment of gallstone disease over the last two decades. In bygone years, books about gallstones were often based on postgraduate lectures which the author, usually a surgeon of distinction, had given. More recently, many books dealing with this subject have been based upon national or international conferences. The single-author text has the disadvantage that few authors today can authoritatively encompass a whole field: the reports of symposia, conferences or workshops often lack balance and auth ority. The merits of 'Gallstone disease and its Management' edited by Malcolm Bateson are clear. He has chosen 11 authors, all of whom write on topics relating to their own expertise, and the content of the book has been carefully planned to reflect the most modern ideas about the aetiology and management of cholesterol gallstones. It is worth repeating that we are experiencing rapid developments in the field of gallstone disease. Disappointingly the least progress has been made in identifying the cause (or causes) of the disease notwithstanding the many data implicating a variety of environmental factors. Most of these affect the chemistry of hepatic bile and the contribution of the gallbladder to lithogenesis remains uncertain and unstudied.

After an introductory section on laboratory findings, pathology, and diagnostic methods, it focuses on specific conditions and their management, covering gallbladder stones, acalculous cholecystitis, gallbladder lesions associated with IgG4-related disease and with anomalous pancreaticobiliary ductal union, and dyskinesia of the gallbladder. Moreover, it discusses incidental gallbladder carcinoma in detail, including epidemiology, risk factors, gene mutations, diagnostic imaging methods and treatment modalities. The book also features a section exploring important current issues, such as the diagnosis and therapy of polypoid lesions and gallbladder wall thickening, and the role of prophylactic cholecystectomy in patients with concomitant gallstones following removal of common bile duct stones by ERCP. It also compares endoscopic gallbladder drainage with percutaneous transhepatic gallbladder drainage. Lastly, the book offers stimulating future perspectives.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides detailed insight to the readers into various aspects of bile duct injury. Bile duct injury is a common complication of cholecystectomy to treat gall stone disease prevalent all over the world. Risk of bile duct injury is more during a laparoscopic procedure as compared to open procedure and most cholecystectomies today are performed laparoscopically. Bile duct injury causes major morbidity and may even result in death, additionally it increases the healthcare costs and impairs quality of life. It is therefore important that every surgeon who performs cholecystectomy knows how to suspect, diagnose, manage and prevent bile duct injury. This book provides practical information and offers assistance in managing patients with difficult cases of bile duct injury. It summarises Dr Kapoor’s experience with management of more than 1,000 patients with post-cholecystectomy bile duct injury/ biliary stricture. Chapters cover anatomy, epidemiology, mechanism, pathophysiology, clinical presentation, investigations, classification, diagnosis, management and prevention of bile duct injury. It also includes non-medical issues including health care, socio-economic, costs and quality of life.

Book excerpt: The current medical literature contains ample text addressing the topics of trauma and critical care. However, there are few texts existing that specifically address the common surgical problems encountered by practicing acute care surgeons. Common Problems in Acute Care Surgery provides a comprehensive, evidence-based review of the most common clinical problems encountered by acute care surgeons. Focusing on the clinical care of the patient, this volume explores the general principles of acute care surgery and the specific disease states that are commonly encountered by acute care surgeons. The work also touches upon the ethical issues and systems development behind acute care surgery, including practical considerations for establishing an acute care surgery program, ethical considerations in acute care surgery, and the role of palliative care. Authored by respected experts in the field and illustrated throughout with detailed photographs, Common Problems in Acute Care Surgery is of great value to resident surgeons in training, fellows, and practicing surgeons in acute care surgery.

Available in PDF, EPUB and Kindle. Book excerpt: The nonsurgical management of gallstone disease has drawn widespread clinical interest during the last decade as ultrasound surveys have indicated that cholelithiasis is predom inantly an asymptomatic condition and much more prevalent than previously thought. This book presents an overview of the pathophysiologic and pathobiochemical principles of gallstone formation and the consequences for clinical therapeutic regimens. New information concerning the balance between vesicular and micellar transport of choles terol, early cholesterol nucleation, and the influence of inhibiting and promoting com pounds for cholesterol nucleation as well as information concerning the effects of changes in gallbladder motility and gallbladder mucosal function is reviewed. In order to make further progress in developing treatments which facilitate gallstone dissolution and in preventing disease, it is necessary to integrate this new data into our thinking. Methods of treatment such as systemic litholysis of cholesterol gallbladder stones with bile acid preparations and mechanical fragmentation of stones either by extracorpo ral shock waves or intracorporal laser systems are carefully described and separate discussions of direct contact litholysis of cholesterol stones with ether preparations and the local litholytic treatment of calcified pigment stones are included. New therapeutic applications of HMG-CoA-reductase inhibitors and nonsteroidal anti-inflammatory drugs are also critically reviewed. Finally, for the interested reader an evaluation of prophylactic treatment against stone recurrence after successful conservative treatment and an appraisal of alternative management strategies supplement the information on the conservative treatment of gallstones.
