Book excerpt: Excerpt from Epidemic Man and His Visitations This Essay is not addressed specially to medical readers, but rather has its purpose for the public, and above all for the Sovereign People wherever they are found. Its aim is to lead them, in their fair rights of sovereignty, to supervise medical policy and practice with regard to epidemics and other events which personally concern them. And, secondly, it offers a theory, very old and very new, of the calamities of disease. This part of the subject will be read and tolerated only by those who believe in God and the Christian religion; in immortality as brought to light in Christ; in man as an organic humanity in the life here and the life hereafter, and in the inseparable mutual connection of these two lives in which mankind wherever extant are members one of another. To others than such believers our theory will be uninteresting, and for the most part unintelligible. We shall now proceed to deal as tenderly as we can with the parasitic hypothesis of the origin of cholera and of diseases generally, which at present blocks the way to higher views. I presume the migration of bacteria from person to person, and from animal to animal, has been made the subject of sufficient observation, and is well ascertained. Book excerpt: Excerpt from Observations Upon the Natural History of Epidemic Diarrhoea The causative agencies from which springs the plentiful harvest of child mortality in large cities may be conceived as a felted mass of rootlets almost inextricably intertwined. Those of poverty, bad housing, bad feeding, and neglect, may be severally recognized, but the extent of their interrelations with actual respiratory and alimentary disease - those to which the greater part of the mortality is referred - can be traced only with difficulty. It is the object Of this work to aid ln the labour of cutting away the matrix and entangling fibres and to lay bare the hidden ramifications of at least one important causative agency - epidemic diarrhoea. This affection - if we accept provisionally the more novel and generally favoured conception. As to its nature - is revealed as something very like an ordinary infectious disease, and one which permeates all classes, while the excessive mortality it gathers round itself in urban centres must be regarded as something superadded, owing to the vicious circle it forms with those baneful conditions of slum life mentioned above. On the other hand, its peculiarly intimate association with the circum stances of domestic life, from the continual faecal pollution of the interior of the household by infants and others, tends to make it more so than other affections of the kind peculiarly a class disease, and an especial scourge of dirty neighbourhoods. Dirty towns may however be saved from excessive mortality by a high percentage of breast feeding. A notable point in the interesting comparison that can be drawn between diarrhoea and typhoid fever is that, in accordance with their peculiarly opposite age-incidence curves, the marked and habitual depositing of infectious excreta within the household in the former disease may make the question of water-closet versus conservancy pan a matter of far less importance than in the latter. Book excerpt: Excerpt from Epidemic Diseases as Dependant Upon Meteorological Influences Certainly those who presume themselves to be right, do not need to hesitate to announce their views at once, and this heralding was at once indulged in, when yellow fever made its appearance in Shreveport. The people were told that yellow fever was due to a specific poison, and that, owing to this agency, the'disease was contagious, and thereforethe rigid quarantaine regulations were urged at once to be put in force; also inducing telegrams from Shreveport to announce the disease to be under control Bun - humanum est errare - hence this proclamation soon proved to be premature, and notwithstanding the supposed controling powers, exercised to the utmost ability, the dis ease increased to spread, and proving even more fatal until meteorological influences caused a diminution in its fre quency as well as in its ultimate extinction. Book excerpt: Excerpt from Notes on Epidemics: For the Use of the Public It seems necessary, in commencing the discussion 'of our subject, to clear up some elementary points with regard to the definition of terms. At the risk, there fore, of appearing to be superfluously minute, we must premise that by epidemic diseases we mean to specify those maladies which, whether they habitually form part of the catalogue of the diseases of any country or not, are at intervals imported afresh from without, and take, more or less suddenly, a greatly increased devel opment, both as to the number of individuals whom they attack and the proportionate mortality. Book excerpt: Excerpt from An Inquiry Into the Nature and Treatment of the Prevailing Epidemic, Called Spotted Fever, &C. &C: In Three Parts Every candid and experienced physician will doubtless allow, that the practice of physic, at the present time, is in a very unsettled and fluctuating condition. Such a change has happened in the nature and treatment of our diseases within a fe'w years, that in many places, in one half or two thirds of thehcases of fever that occur, the physician is without any guide he has merely heard somethin concerning the treatment of this new disease which prevails, or perhaps has read some scrap in the newspaper, or possibly in some periodical publica tion, relating to the same. In these accounts he finds a reat variety of the same disease, occurring in hard y any two places or years alike, With sub divisions into a great number of [grades of the same disease and these descriptions are from physicians who have different views of the disease, and who consequently recommend different and even Opposite modes of treatment. This must tend very much to confuse even experienced and well informed phy sicians, especially in the first cases which they are called to attend. This is too much the case even at the present day. For these reasons, i have attempt ed an Inquiry into the Nature and Treatment of that most formidable disease, called Spotted Fever.
