Available in PDF, EPUB and Kindle. Book excerpt: The fourth cholera pandemic of the 19th century began in the Ganges Delta of the Bengal region and traveled with Muslim pilgrims to Mecca. In its first year, the epidemic claimed 30,000 of 90,000 pilgrims. Cholera spread throughout the Middle East and was carried to Russia, Europe, Africa and North America, in each case spreading via travelers from port cities and along inland waterways. The pandemic reached Northern Africa in 1865 and spread to sub-Saharan Africa, killing 70,000 in Zanzibar in 186970. Cholera claimed 90,000 lives in Russia in 1866. The epidemic of cholera that spread with the Austro-Prussian War (1866) is estimated to have taken 165,000 lives in the Austrian Empire, including 30,000 each in Hungary and Belgium, and 20,000 in the Netherlands. In June 1866, a localized epidemic in the East End of London claimed 5,596 lives, just as the city was completing construction of its major sewage and water treatment systems; the East End section was not quite complete. It was also caused by the city's overcrowding in the East End, which helped the disease to spread more quickly in the area. Epidemiologist William Farr identified the East London Water Company as the source of the contamination. Farr made use of prior work by John Snow and others, pointing to contaminated drinking water as the likely cause of cholera in an 1854 outbreak. In the same year, the use of contaminated canal water in local water works caused a minor outbreak at Ystalyfera in South Wales. Workers associated with the company, and their families, were most affected, and 119 died. In 1867, Italy lost 113,000 to cholera, and 80,000 died of the disease in Algeria. Outbreaks in North America in the 1870s killed some 50,000 Americans as cholera spread from New Orleans via passengers along the Mississippi River and to ports on its tributaries.

Cholera causes violent cramps, vomiting and diarrhea that are so frequent and serious that the body will quickly dehydrate. A person infected with cholera can die within a few hours because the dehydration can be so severe that the blood coagulates. Cholera was deadly because of several longstanding inaccurate thoughts on its cause: namely, that 'inferior' people with personal failings, or members of a different culture, combined with exposure to environmental filth, were likely to fall victim. While environmental issues are a cause--contaminated drinking water is a major contributor to cholera outbreaks--during the 1800s and onwards, physicians believed that personal characteristics also contributed to cholera. This volume offers a snapshot in time on these beliefs manifested in terms of approach and treatment. Sadly, cholera is far from a dated disease. Yemen is currently facing a major cholera crisis, with smaller outbreaks recently reported in Somalia and Darfur.

What experts did agree on was that the environment played a crucial role in the sites where outbreaks occurred. In this book, Michael Zeheter offers a probing case study of the environmental changes made to fight cholera in two markedly different British colonies: Madras in India and Quebec City in Canada. The colonial state in Quebec aimed to emulate British precedent and develop similar institutions that allowed authorities to prevent cholera by imposing quarantines and controlling the disease through comprehensive change to the urban environment and sanitary improvements. In Madras, however, the provincial government sought to exploit the colony for profit and was reluctant to commit its resources to measures against cholera that would alienate the city's inhabitants. It was only in 1857, after concern rose in Britain over the health of its troops in India, that a civilizing mission of sanitary improvement was begun. As Zeheter shows, complex political and economic factors came to bear on the reshaping of each colony's environment and the urgency placed on disease control.

Medical science failed to see that infected water supplies were the major carriers of the epidemic and failed to acknowledge saline infusion (the basis of successful modern treatment) when it was presented to them by an obscure local surgeon in Leith. The social structure of the medical profession was as much a barrier to scientific advance as the technical limitations of statistical method and microscope. These reactions are explained in terms of the expectations and the understanding of those involved as well as in terms of modern medical knowledge and sociological theory.
