Book excerpt: "Tuberculosis (TB) affected an estimated 8.7 million people and caused 1.4 million deaths globally in 2011, including 0.5 million women and at least 64 000 children. About 13% of TB occurs among people living with HIV, and TB causes almost a quarter of AIDS deaths. There is evidence of links between TB and noncommunicable diseases such as diabetes mellitus and with determinants of ill-health like tobacco and drug use, alcoholism and malnutrition. TB mostly affects the productive segment of society in their prime. One third of people estimated to have TB are either not reached for diagnosis and treatment by the current health systems or are not being reported. Even in patients who are identified, TB is often diagnosed and treated late. In order to reach the unreached and to find TB patients early in the course of their illness, a wider range of stakeholders already involved in community-based activities needs to be engaged. These include the nongovernmental organizations (NGOs) and other civil society organizations (CSOs) that are active in community-based development, particularly in primary health care, HIV infection and maternal and child health, but have not yet included TB in their priorities and activities. NGOs and other CSOs are non-profit organizations that operate independently from the state and from the private for-profit sector. They include a broad spectrum of entities such as international, national and local NGOs, community-based organizations (CBOs), faith-based organizations (FBOs), patient-based organizations and professional associations. CBOs are membership-based non-profit organizations that are usually self-organized in specific local areas (such as a village) to increase solidarity and mutual support to address specific issues. For example, these include HIV support groups, women's groups, parent-teacher associations and micro-credit village associations. CBO membership is comprised entirely of community members themselves, so these organizations can be considered to represent the community most directly. NGOs and other CSOs engage in activities that range from community mobilization, service delivery, and technical assistance to research and advocacy. The strengths of NGOs and other CSOs active in health care and other development interventions at the community level include their reach and spread and their ability to engage marginalized or remote groups. These organizations have a comparative advantage because of their understanding of the local context. Greater collaboration between NGOs and other CSOs and local and national governments could greatly enhance development outcomes. A more decentralized approach that formally recognizes the critical role of NGOs and other CSOs as partners addressing gaps through support to community-based actions will expand TB prevention, diagnosis, treatment and care activities. Community-based TB activities cover a wide range of activities contributing to prevention, diagnosis, improved treatment adherence and care that positively influence the outcomes of drug-sensitive, drug resistant and HIV-associated TB. The activities also include community mobilization to promote effective communication and participation among community members to generate demand for TB prevention, diagnosis, treatment and care services. While diagnostic tests for TB continue to be performed in clinical settings, for lack of simpler diagnostic methods, community-based TB activities are conducted outside the premises of formal health facilities (e.g. hospitals, health centres and clinics) in community-based structures (e.g. schools, places of worship, congregate settings) and homesteads. Such community-based TB activities could and should be integrated with other community-based activities supporting primary health care services, including those for HIV infection, maternal and child health and noncommunicable diseases to improve synergy and impact. Community-based TB activities utilize community structures and mechanisms through which community members, CBOs and groups interact, coordinate and deliver their responses to the challenges and needs affecting their communities."--Page 1.

Book excerpt: This document has been developed to explain in more detail how the actions outlined in the Operational Guidance can be implemented. The manual has three objectives: * to explain simply how the ENGAGE-TB operational guidance can be implemented by NGOs/CSOs and by NTPs; * to explain simply how community-based TB activities can be integrated into sectors such as PHC, HIV care, MNCH, water and sanitation, agriculture, livelihoods and education; and * to explain the importance of the two community engagement indicators and the need to record community contributions within NTP monitoring systems. The manual also explains how the progress of community-based TB activities may be periodically evaluated.

Book excerpt: Integrating Community-Based Tuberculosis Activities into the Work of NGOs and other CSOs The World Health Organization (WHO) released its operational guidance entitled Engage-TB - Integrating Community-Based Tuberculosis Activities into the Work of NGOs and other CSOs in 2012. In 2013, it produced an accompanying implementation manual for Engage-TB Operational Guidance. This training curriculum and facilitators' guide was developed based on the implementation manual and intended to help train staff of national tuberculosis programs (NTPs) or their equivalents, nongovernmental organizations (NGOs) and other civil society organizations (CSOs) and all other stakeholders in the Engage-TB approach. Such training should increase the ability of these actors to support and integrate community-based tuberculosis (TB) activities into their ongoing work. This document is purposely designed to combine the training curriculum and the facilitators' guide. Part A is the training curriculum, while Part B is the facilitators' guide. The facilitators' guide includes the PowerPoint slides available at http: //www.who.int/tb/people_and_communities/engage_tb_tm/en/ and reproduced in print in Annex 7 of this document. The training curriculum and the facilitators' guide should always be used together with WHO's Engage-TB Operational Guidance and Implementation Manual. The curriculum has five modules, which are divided into units that are logically and sequentially structured to enable participants to acquire unit learning outcomes and competences. The teaching duration, methods, activities and tools presented are only a guide and should be adapted to the facilitators' settings to ensure effective delivery of WHO Engage-TB training. The training curriculum is structured in a modular format to facilitate flexible delivery for different target groups. For example, a trainee knowledgeable about TB such as a national TB program (NTP) staff may not need to use module 2 (TB - the basics). Module 5 unit 5.6 will be applicable mainly to participants intending to integrate TB activities into agricultural programs. Those working in reproductive, maternal, newborn and child health (RMNCH) may not find this unit relevant. The curriculum is a "live" document and should be reviewed, adapted and updated as needed.

It provides a comprehensive and up-to-date assessment of the TB epidemic and progress in implementing and financing TB prevention care and control at global regional and country levels using data reported by almost 200 countries that account for over 99% of the world's TB cases. Two years before the 2015 deadline for achievement of global TB targets the 2013 report includes a special supplement that assesses progress towards the 2015 targets and the actions needed to accelerate towards or move beyond them. The report has 8 main chapters. The introductory chapter provides general background on TB as well as an explanation of global targets for TB control the WHO's Stop TB Strategy that covers the period 2006-2015 and the development of a post-2015 global TB strategy. The remaining seven chapters cover the disease burden caused by TB (incidence prevalence mortality); TB case notifications and treatment outcomes; drug resistance surveillance among TB patients and the programmatic response in detecting and providing treatment for multidrug-resistant TB; diagnostics and laboratory strengthening for TB; addressing the co-epidemics of TB and HIV; financing TB care and control; and research and development for new TB diagnostics drugs and vaccines. The four annexes of the report include a thorough explanation of methods used to estimate the burden of disease caused by TB one-page profiles for 22 high TB-burden countries and tables of data on key indicators for all countries organized by WHO region.

Book excerpt: This facilitators' guide for training community health workers (CHWs) and community volunteers (CVs) is intended for training in tuberculosis (TB) and integration of TB prevention and care services into community-based activities. The training lasts 3 days. It includes a PowerPoint slide set which is an integral part of the guide. Together these allow the facilitator to progress carefully from one idea to the next. This document is not for use directly by CHWs or CVs. The training includes six training modules. It starts with introductions objectives and norms followed by a brief presentation of the ENGAGE-TB approach which will enable CHWs and CVs to integrate TB activities into their existing work. The third module allows substantial time for CHWs and CVs to understand the basics of TB: its signs and symptoms and how it can be prevented and treated. The fourth module deals with integration of community-based TB services into community work. This is followed by a field visit to a TB clinic so that CHWs understand how clinical and laboratory aspects are handled. Finally the CHWs and CVs reflect on what they have learnt and describe how they will integrate TB services into their work on their return home.

Book excerpt: The importance of community and civil society engagement to end TB has been highlighted in various strategies of global commitments. The WHO End TB Strategy, aligned with the United Nations Sustainable Development Goals, emphasizes the role of communities and civil society in ending the TB epidemic by 2030. Furthermore, the political declaration of the 2081 United Nations High-level Meeting on TB Highlights the need to develop integrated, people-centred, community-based, gender-responsive health services. This guidance was developed in collaboration with civil society and other partners, in order to further strengthen engagement and leverage capacities of communities' and civil society in line with the End TB Strategy. The guidance emphasizes the complementarity of health systems and community systems; the key roles that people affected by TB should play in planning, decision-making, implementation and monitoring; and the role of ministries of health and their NTPs. It underlines the importance of fair, sustainable financing and of policy environment for community and civil society engagement. Its purpose is to provide guidance for communities and for all stakeholders in the health system for working together to end TB and strengthening people-centred care. Stakeholders in national TB responses include ministries of health, other government ministries, the private sector, civil society and affected communities, academic and research institutions, and technical and funding partners.

Book excerpt: The Operational guidance on adaptation and implementation of WHO’s Multisectoral Accountability Framework to end TB (MAF-TB) provides practical advice on key approaches and interventions needed to establish the MAF-TB at the national (and local) levels with concrete country examples, best practices and case studies under each suggested approach and interventions. It is intended for use by all stakeholders involved in their national TB response, including ministries of health and other relevant government ministries and bodies, national TB programmes (or their equivalents in ministries of health), other relevant national programmes, parliamentarians, the private sector, international organizations, nongovernmental and civil society organizations and TB-affected communities involved in planning, implementation, monitoring and evaluation and review of the TB response. The stakeholders involved may vary across countries depending on several factors, including the epidemiology and determinants of TB, the institutional arrangements for TB care and prevention and the degree of devolution of the political and health governance system.
