Book excerpt: Millions of referrals are made to Child Protective Services a year regarding childhood maltreatment in the United States. Childhood maltreatment has been linked to dysregulation in humans’ stress response systems – the hypothalamic-pituitary-adrenal (HPA) axis and the sympathetic nervous system (SNS). Importantly, dysregulation in these systems has been associated with a variety of adverse physical consequences as well as psychopathology. Difficulties in emotion regulation, an important component of psychopathology, have also been linked with childhood maltreatment. While research has found that childhood maltreatment is associated with dysregulated physiological stress responses, difficulties with emotion regulation, and psychopathology, more research is indicated to link all these variables in an integrated model. This longitudinal study examined a model of the relationship between childhood maltreatment (i.e., physical abuse, sexual abuse, and neglect), emotion regulation style (i.e., reappraisal, suppression, and rumination type), and physiological stress response (i.e., salivary cortisol and alpha amylase levels and reactivity) among a sample of African-American young adult women in low-income, urban communities, hypothesizing that emotion regulation style would act as a mediator.

Book excerpt: The aim of the present study was to examine the mediating roles of emotion dysregulation and cognitive and behavioral avoidance in the relationship between childhood maltreatment and psychological distress. A total of 305 participants from a large public university volunteered to participate in exchange for course credit. Tests for mediation revealed that emotion dysregulation and cognitive and behavioral avoidance partially mediated the effect of childhood emotional maltreatment on psychological distress. When examined in separate analyses, 65% of the effect of maltreatment on distress is mediated through emotion dysregulation, while 39% of the effect is mediated through cognitive and behavioral avoidance. Given previous findings linking maltreatment to an increased risk for problematic emotional functioning, additional work on how to enhance resilience by improving emotion regulation skills and reducing avoidant behaviors among at-risk children and young adults is needed.

The long-term human and fiscal consequences of child abuse and neglect are not relegated to the victims themselves-they also impact their families, future relationships, and society. In 1993, the National Research Council (NRC) issued the report, Under-standing Child Abuse and Neglect, which provided an overview of the research on child abuse and neglect. New Directions in Child Abuse and Neglect Research updates the 1993 report and provides new recommendations to respond to this public health challenge. According to this report, while there has been great progress in child abuse and neglect research, a coordinated, national research infrastructure with high-level federal support needs to be established and implemented immediately. New Directions in Child Abuse and Neglect Research recommends an actionable framework to guide and support future child abuse and neglect research. This report calls for a comprehensive, multidisciplinary approach to child abuse and neglect research that examines factors related to both children and adults across physical, mental, and behavioral health domains-including those in child welfare, economic support, criminal justice, education, and health care systems-and assesses the needs of a variety of subpopulations. It should also clarify the causal pathways related to child abuse and neglect and, more importantly, assess efforts to interrupt these pathways. New Directions in Child Abuse and Neglect Research identifies four areas to look to in developing a coordinated research enterprise: a national strategic plan, a national surveillance system, a new generation of researchers, and changes in the federal and state programmatic and policy response.

Book excerpt: There are elevated rates of childhood maltreatment and posttraumatic stress disorder (PTSD) symptomatology in low-income, abused, suicidal African American women. This investigation aimed to: (1) identify the components of childhood maltreatment in this sample; (2) ascertain whether or not the constructs of childhood maltreatment and PTSD symptomatology were associated in this sample; and (3) examine if maladaptive coping mediated the childhood maltreatment-PTSD symptomatology link and if the magnitude of the mediated relationship was influenced by level of social support (i.e., moderated mediation). Specific types of childhood maltreatment generally loaded onto three components according to a principal components analysis (PCA) of the Childhood Trauma Questionnaire: physical-emotional abuse, sexual-emotional abuse, and neglect. Women who endorsed experiencing higher levels of two of the childhood maltreatment components (physical-emotional abuse and sexual-emotional abuse) reported higher levels of current PTSD symptomatology. However, contrary to the study hypotheses, current level of maladaptive coping did not mediate the relationship between child maltreatment and current PTSD symptomatology. Further, the addition of social support did not change this finding. Results are discussed, clinical implications are explored, and recommendations for future studies are offered.

Book excerpt: This study examined the relationships among self-reported childhood physical or psychological abuse history, psychological distress, and difficulties in emotion regulation in a sample of young adults. Specifically, the study examined whether young adults who report a history of childhood physical or psychological abuse experience higher levels of emotional avoidance, emotion dysregulation and alexithymia than non-abused counterparts. To examine these issues, 253 participants (138 females, 113 males) completed measures assessing the above mentioned variables. Participants who self-reported childhood physical abuse histories experienced greater psychological distress, emotion dysregulation and alexithymia than non-abused individuals. Self-reported psychological abuse was positively correlated with psychological distress variables, emotional avoidance, emotion dysregulation, and alexithymia. Emotional avoidance mediated the relationship between psychological abuse history and psychological distress. Alexithymia mediated the relationship between both physical and psychological abuse history and psychological distress. Clinical and treatment implications of these findings are discussed.

Book excerpt: Approximately 60% of adults report experiencing an adverse childhood event (e.g., abuse, neglect, household dysfunction) prior to 19 years of age. Child maltreatment-related chronic stress is associated with brain volume reductions in pre-frontal, cerebellar, and cortico-limbic regions. These changes likely correspond to difficulties in inhibitory control and emotion regulation and associated psychopathology common among maltreatment survivors. However, few studies examined functional brain activity related to these constructs in adults with maltreatment history. The goal of this study was to examine functional brain differences in inhibitory control and emotion regulation between young adults with (maltreated), and without (controls) a history of adverse childhood events, including abuse, neglect, and parental domestic violence. The ERP and brain correlates of response inhibition under normal and frustrating conditions were examined during a Go/No-Go task. Potential confounding variables including gender, IQ, PTSD symptoms, and number of adverse childhood experiences were controlled for in all analyses. Results indicated that maltreated participants showed poorer accuracy to No-go trials during baseline, but not frustration conditions, indicating behavioral differences in inhibitory control but not emotion regulation. Brain activity in the pre-supplementary motor area and rostral anterior cingulate cortex increased between the baseline and frustration conditions for all participants, but only returned to baseline levels for the maltreatment group. Brain activity in the orbital frontal cortex was unrelated to child maltreatment. This study replicated previous findings indicating that child maltreatment is related to poorer inhibitory control in adulthood. Additionally, brain areas involved in motor (pre-SMA) and emotion (rACC) modulation showed differential patterns of activation for maltreated participants following frustration, providing evidence that child maltreatment is associated with differences in emotion processing in adulthood. These differences in inhibitory control and emotion processing likely play a role in the adverse social and psychological outcomes observed among adults with maltreatment history, and may also impact cognition in everyday life.

Book excerpt: This study investigated links between childhood maltreatment (CM), perceived stress, coping strategies, and physical health problems among adult women. There is mounting evidence to suggest that perceived stress and coping strategies help to explain the association between CM and physical health outcomes. However, research has yet to clarify the precise mechanisms through which stress and coping independently, and in combination, predict the health concerns of victimized women. Through the use of structural equation modeling (SEM), support was found for a model in which perceived stress partially mediated the association between CM and physical health problems. While emotion-focused coping was also found to partially mediate the CM-health relationship, problem-focused and avoidance coping did not. A moderated mediation model revealed that each coping strategy moderated the impact of maltreatment, but not of perceived stress, on physical health. Multi-mediation model testing indicated that emotion-focused coping and perceived stress better explain the relationship between CM and health than either variable on its own, and that this coping strategy fully accounted for the link between CM and subsequent stress. Finally, multivariate regression analyses revealed that child physical abuse was uniquely associated with greater physical symptoms, and child psychological maltreatment had a unique link with functional impairment; however, no form of abuse uniquely explained health care utilization. Findings suggest that child maltreatment is a risk factor for adverse health outcomes in later life and that stress and coping strategies are important mechanisms in this relationship. Implications for clinicians, medical professionals, and researchers are discussed.
