Book excerpt: Développés il y a 25 ans sur le principe du déconditionnement des lombalgiques chroniques, les programmes de réentraînement à l'effort (RAE) ont fait la preuve de leur efficacité. L'objectif était de recenser les centres de RAE en France en 2011 et décrire leur prise en charge. Sur les 62 centres recensés, 35 ont répondu à l'étude et 39 programmes ont pu être décrits. Les établissements proposaient des programmes de RAE depuis 10,1 ans en moyenne (écart-type ± 4,1) et le nombre moyen de patients traités était de 83,9 (±73,5) patients par an. Les médecins intervenants étaient plus souvent rééducateurs (64%) que rhumatologues (10,2%). Les équipes comprenaient toujours kinésithérapeutes et ergothérapeutes, souvent un professeur d'activité physique adaptée et plus rarement un psychomotricien. Le stage se déroulait sur une durée moyenne de 3,8 (±1,2) semaines avec 5,8 (±1) heures/j de rééducation et un volume horaire global de 111,1 (±37,8) heures. La musculation isocinétique du tronc était utilisée dans 10 programmes, une approche cognitivo-comportementale était possible pour 13 programmes. La problématique professionnelle était abordée plus spécifiquement dans 10 programmes, fréquemment dans le cadre d'un réseau. L'évaluation physique comportait : tests isométriques du tronc (37/39), tests isocinétiques (18/39), PILE test (32/39) et épreuve d'effort (18/39). En moyenne, les patients remplissaient 3,9 (±2) questionnaires en début de programme, évaluant le plus souvent le handicap (79,5% des cas), la qualité de vie (82,1%), l'anxiété et la dépression (74,4%) et la notion d'évitement (64,1%). Une standardisation des pratiques et des évaluations semble nécessaire.

Les programmes de restauration fonctionnelle (PRF) sont des programmes multidisciplinaires dont le but est de permettre la réinsertion socio-professionnelle de ces patients. L'objectif de ce travail est de déterminer l'efficacité à long terme d'un PRF dans une population française de patients lombalgiques chroniques et de déterminer les facteurs prédictifs de la reprise du travail. Des patients lombalgiques chroniques en arrêt de travail depuis au moins un mois ont été inclus dans un PRF. La douleur, le statut professionnel, le niveau d'invalidité, la dépression et l'anxiété et les craintes et croyances des patients ont été évalués à l'inclusion, à 1 an et à la fin du suivi. Le critère principal d'efficacité était la reprise du travail. Cent cinq des 162 patients inclus dans le PRF ont été évalués. L'age moyen des patients à l'inclusion était de 44 ans, leur durée moyenne d'arrêt de travail était de 15 mois. La durée moyenne de suivi était de 3,5 ans. Dans cette population de patients lombalgiques chroniques gravement invalidés, l’efficacité du PRF en terme de reprise du travail était maintenue à long terme (55% de reprise du travail à long terme). Les paramètres concernant la fonction, l'état psychologique et les craintes et croyances des patients étaient tous améliorés de façon significative et prolongée. En analyse multivariée, les facteurs prédictifs de reprise du travail à long terme étaient la durée d’arrêt de travail, la durée des douleurs et la pratique de sport à l’inclusion dans le PRF.

L'objectif de cette étude était d'évaluer l'impact d'un protocole de restauration fonctionnelle du rachis sur les répercussions fonctionnelles de la douleur. Matériel et méthodes : les dossiers médicaux de tous les patients inclus dans le protocole de restauration fonctionnelle du rachis du CRMPR ont été analysés. L'échelle de Québec et l'évaluation du niveau de la douleur étaient réalisées en début (M0) et fin de stage (M1). Les données des patients avec discopathie active Modic 1 ont été relevées. Ils ont été contactés par téléphone afin de compléter un questionnaire de suivi à distance (Mx). Résultats : Quarante-quatre patients avec discopathie active ont été inclus. Il y avait une diminution significative du score de l'échelle de Québec à M1, de 49,8/100 ± 12,7 à 37,1/100 ± 14,7. Il n'y avait pas d'évolution du niveau de la douleur. Vingt patients ont répondu au questionnaire, avec une durée moyenne de suivi de 29,5 mois ± 19,3. L'échelle de Québec était évaluée dans cet échantillon à 48,2 ± 13,2 à M0, 36,7 ± 15,8 à M1, et 37,0 ± 18,6 à Mx. Il y avait une différence significative entre M0 et M1 (p = 0,0004) et entre M0 et Mx (p = 0,007). Il n'y avait pas de différence du niveau de la douleur entre M0, M1 et Mx. Conclusion : L'inclusion des patients avec discopathie active dans les protocoles de restauration fonctionnelle du rachis est encore débattue. Cette étude semble confirmer l'intérêt d'une prise en charge active et psycho-sociale dans cette indication. Elle montrait un effet significatif d'une prise en charge globale sur les répercussions fonctionnelles de la lombalgie chronique.

He went on to write many books and film scripts.
