En France, la prévention est depuis près de 30 ans un axe prioritaire de santé publique. Cependant, cette orientation soulève plusieurs questions, notamment quant à la mise en oeuvre des programmes de prévention précoces. En effet, non seulement l'efficacité des ac-tions menées n'est aujourd'hui que très rarement évaluée, mais de plus, ces pro-grammes ne ciblent pas le public le plus à risque de développer des difficultés. Dans ce contexte, l'objectif de notre étude était celui d'évaluer les effets d'une intervention de prévention précoce axée sur le langage oral et la communication, proposée à des parents d'enfants âgés entre 0 et 3 ans et usagers d'une Protection Maternelle et In-fantile d'une zone sensible. Pour tenter de mesurer les effets de notre action, nous avons analysé les réponses aux questionnaires pré et post-intervention présentés aux participants de l'étude. Nos résultats montrent une augmentation des connaissances parentales sur le langage oral et la communication, mais pas d'augmentation ni d'amé-lioration des interactions avec leur enfant. Ainsi, il apparait des plus important d'évaluer l'efficacité des programmes de prévention, mais aussi de les cibler aux populations les plus à risque.

La vulnérabilité sociale constitue un ensemble de facteurs de risque pour le développement langagier de l’enfant et pourrait avoir des conséquences sur son intégration sociale et sa réussite scolaire. Il est nécessaire et primordial d’agir en amont de ces difficultés et de proposer des actions de prévention sous forme de Programmes d’Intervention en Orthophonie (PRIO). Dans le cadre d’une intervention auprès d’un enfant, il est pertinent d’intégrer le parent, interlocuteur privilégié de l’enfant. A travers différents ateliers de stimulation des attitudes de communication et la co-construction d’outils permettant d’intégrer les apprentissages dans les routines quotidiennes, les PRIO améliorent la qualité pragmatique des interactions parent/enfant. Le langage de l’enfant serait par conséquent davantage étayé et stimulé et il serait possible d’observer une amélioration de sa compétence langagière. En collaboration avec le Programme de Réussite Educative de Vénissieux, nous avons proposé un PRIO destiné aux parents d’enfants de Grande Section de Maternelle présentant des fragilités langagières. Le cas de deux familles est étudié dans ce projet de recherche. Nous avons procédé à plusieurs évaluations permettant une comparaison qualitative et quantitative entre le pré et le post-PRIO. Une situation de lecture partagée entre le parent et son enfant a été proposée afin d’évaluer les attitudes de communication du parent, tandis que l’analyse des sessions et d’un questionnaire nous ont permis de qualifier leur sentiment de compétence. Nous avons également réalisé une évaluation langagière des enfants afin de pouvoir quantifier l’amélioration de leur compétence langagière. Notre étude met en évidence une augmentation du sentiment de compétence chez les parents, et l’apparition d’attitudes de communication facilitatrices. Cependant, nous ne notons pas d’amélioration de la compétence langagière des enfants. Notre PRIO, projet pilote, permet de confirmer la pertinence de l’orthophonie dans les politiques de santé publique dans le champ de la prévention

Pour sa part, la mission pour la protection maternelle et infantile des Hautes-Pyréennées a décidé de mettre en place un dépistage assez précoce (3-4 ans) des troubles du langage oral. Cette action a soulevé de nombreuses questions auxquelles le présent mémoire tente de répondre, après avoir rappelé le développement du langage de l'enfant et les troubles qui peuvent survenir au cours de celui-ci. Les résultats de l'étude vont permettre de constater un pourcentage non négligeable d'enfants dépistés pour lesquels le trouble a suscité une intervention thérapeutique. Nous verrons cependant que les médecins de l'équi de PMI ont dû faire face à diverses contraintes et adaptés leur méthode et pratique en conséquence. D'autre part, nous pourrons observer l'intérêt manifeste des parents, médecins et enseignants pour ce type de prévention.
