Qu'elle soit spontanée ou induite. par la rééducation orthophonique, la récupération sollicite la participation des deux hémisphères cérébraux. La prise en charge orthophonique, remédiation essentielle à la récupération de l'aphasie, peut engendrer des modifications de l'organisation cérébrale. Ainsi, la· Thérapie Mélodique et Rythmée, qui utilise et accentue les paramètres prosodiques du langage, amène-t-elle à une réorganisation corticale. Ces modifications des réseaux neuronaux sollicitant préférentiellement l'un ou l'autre des hémisphères font débat dans la littérature. Nous avons donc cherché, dans une étude en IRM fonctionnelle, à mettre en évidence les substrats neuronaux sous-tendant le traitement de la prosodie dans une cohorte de sujets non pathologiques puis nous avons proposé une rééducation en TMR à un patient aphasique afin d'étudier l'impact des paramètres de la TMR en clinique. Le traitement prosodique semble supporté par un réseau bihémisphérique, majoré dans l'hémisphère droit. L'étude de cas clinique ne nous permet pas d'incomber les progrès du patient au recours hémisphérique droit, néanmoins, nous avons montré que la TMR peut être efficace chez un sujet aphasique ne présentant pas les caractéristiques lésionnelles retrouvées dans les études sur cette méthode de rééducation.
