Book excerpt: In today's evolving health care system, effective nursing leadership is crucial to the advancement of the nursing profession. The style of management and leadership behaviors have been shown to affect work environments, including the level of nurse job satisfaction and nurse turnover rates. Increasing body of evidence shows that many nurses perceive being unsupported by nursing leaders, with complaints ranging from feeling undervalued and disempowered to hostile communication behaviors. Decreased level of nurse job satisfaction can have many detrimental consequences, including burnout, disengagement, poorer performance, and increased turnover rates, all of which compromise the ability of health care organizations to provide safe and effective patient care. Because the level of job satisfaction among staff nurses has been shown to affect patient outcomes, knowing what improves the level of satisfaction is significant in any health care organization. Therefore, there is an obvious need for health care organizations to undergo transformation by redefining effective leadership attributes and functions as well as education about and development of leadership skills and behaviors. The transformational leadership approach has been shown to have a positive impact on job satisfaction of staff nurses, resulting in improved relationships between staff nurses and managers and a strong desire to maintain membership in the organization. Leaders who use this approach have been shown to create healthy working environments that foster staff nurse trust in management, which has an impact on both the quality of nurses09́ work life and the quality of patient care. This proposal will include an implementation plan for the integration of transformational leadership in the current work flow health carehcare organization by way of increasing awareness of transformational leadership qualities and attributes among staff nurses work flowe managers.

Book excerpt: The lack of an effective leadership style has been recognized as a leading contributor of reduced job satisfaction and high employee turnover. In the health care sector, this has been shown to lead to negative patient outcomes. The purpose of this DPI project was to establish the relationship between leadership style and the nurses' job satisfaction and retention, and the patient outcome of falls reduction in a long-term care unit in Chicago. The clinical questions sought to determine how the transformational leadership (TL) style, through the intervention of education/training impacted the other variables. The independent variable was the implementation of TL, while the dependent variables included the nurses' job satisfaction, retention, and the patient outcome of reduction in falls. The project employed two theories; Jean Watson's theory of human caring and Patricia Benner's novice to expert nurse theory. The sample consisted of 30 nurses (N=30) from the facility who completed the pre and post-test surveys on the NWI-R questionnaire. Using the Statistical Package for Social Sciences (SPSS) for data analysis, the findings indicated that the perception of nurses towards their leaders and working conditions improved with TL. In the three months after implementation, the number of reported patient falls reduced from 38 to 13. The test for significance was p=0.888, indicating a positive correlation between the variables. The project recommended that health care organizational leadership should understand and adopt the characteristics of TL since it enhances nurses' morale and performance leading to job satisfaction, which may lead to an increase in retention, and may lead to improved patient outcomes.

Recent data implies that job satisfaction and nurse retention is a direct correlation with nursing leadership styles. Current challenges like the nursing shortage require the modern day leader to diagnose the situation and determine appropriate strategies to foster a creative and autonomous working environment through empowerment. Between the healthcare reform and the imminent nursing shortage, precautions are being taken to balance innovation without incurring financial ruin (Carver and Candela, 2008). The average age of a nurse at the bedside is rising yearly, and estimates indicate that 50% of the current practicing nurses will leave the workforce in the next 5 to 10 years (Carver and Candela, 2008). The objective when reviewing the research was to assess effective leadership styles, training and impact on staff satisfaction. Upon review of the literature, a clear distinction emerged suggesting leadership styles such as transformational and transactional had a direct and positive association with staff satisfaction and retention through empowerment, motivation, shared vision and respect. A broad-based assessment system was introduced to nursing staff members in a busy emergency department as a pilot approach to create a transformational leadership style of management throughout the unit. The introduction of the Hogan survey provides each participant within an individual assessment report describing strengths, weaknesses, and details where each person is likely to find significant satisfaction, dissatisfaction, and opportunities for growth. The finalized results of this ongoing pilot will provide leadership with the means to understand potential within each staff member, see growth areas, and find future leaders within the existing ranks of staff nurses. Further benefits include an understanding of the motivations for each member of the staff so that managers can provide incremental transformational coaching based NURSE LEADERSHIP'S INFLUENCE OVER STAFF 3 on the needs of each staff member within their leadership structure. The benefits to the highest levels of leadership include a clear succession planning picture within existing leaders, as well as a reduction in staff turnover rates. At the end of the implementation phase, the pre-and post job satisfaction survey will be applied to all staff to determine if the engagement of staff has increased, along with the turnover percentages for the Department. These metrics will provide a tangible sense of success for the pilot.

Given these statistics, it is essential that healthcare organizations develop strategies and engage in leadership practices, which will address the complexity of healthcare processes and ensure that care is provided in a consistent, reliable manner in order to achieve the desired outcomes (Frankel, Gandhi & Bates, 2003). It is equally vital that leaders create supportive practice environments that promote a non-punitive culture of learning, continuous improvement, inter professional collaboration, and professional autonomy, thus engaging nurses in safe practice aimed at improving patient outcomes (Aiken, 2008; Pronovost et al., 2003). In order to understand how nursing leadership affects outcomes, it is important to assess what leadership behaviours are most effective in promoting a patient safety culture. This study tested a hypothetical model which predicted the influence of nurse manager Transformational Leadership behaviour on staff nurse perceptions of supportive practice environments, organizational citizenship behaviours, patient safety culture, job satisfaction and objective measures of selected nurse sensitive outcomes. Findings supported the hypothesized model 2 (df = 22) = 40.72, p = .008; CFI = .958; TLI = .916; RMSEA = .079; SRMR = .045 linking transformational leadership to nurse and patient outcomes through supportive practice environments, organizational citizenship behaviours, safety culture and job satisfaction. Book excerpt: Abstract : The problem addressed was the low job satisfaction levels of nurses and subsequent nurses' decision to leave the organization. The quantitative correlational survey study involved determining whether a relationship exists between nurses’ perceptions of nurse managers’ leadership style and nurses’ job satisfaction. Eighty-three fulltime medical surgical intensive care nurses in two hospitals in Phoenix, Arizona, completed the Job Description Index for Jobs in General (JID/JIG) and the Multifactor Leadership Questionnaire (MLQ, Form 5X). The results suggest a significant, positive correlation between job satisfaction and perceptions of nurse managers' leadership style by nurses. Nurses with the highest satisfaction scores in three of the six subscales perceived their managers used the transformational leadership style. The mean score for nurses whose managers were rated as transactional was higher than the mean score for nurses whose managers were rated as passive-avoidant. The promotion and supervision subscales and the job in general scale showed a significant relationship with transformational leadership. Implications for healthcare administrative leaders include hiring transformational managers to increase job satisfaction in nurses and offer nurses opportunities for promotion and training.
