Contrary to predictions, clients' appraisals of the therapists' interpersonal impacts were congruent with the therapists'. However, there was a significant difference on both dimensions of love and dominance between the mean of the therapists' and observers' appraisals of the therapists' interpersonal impact and the clients' appraisals of the therapists' interpersonal impact. Level of congruence between clients' and therapists' appraisals was significantly related to clients' self-reported symptom severity, but opposite of the predicted direction. Higher congruence between client and therapist appraisals was related to higher client self-reported symptom severity. Clients' symptom severity was also significantly related to the working alliance. Higher self-reported symptom severity was associated with a lower total working alliance score. Level of congruence between clients' and therapists' appraisals was not significantly related to the working alliance.

Section One presents a systematic review of 14 empirical studies, considering how client and therapist intra- and interpersonal variables impact upon the other person's perception of the early therapeutic alliance. The review provides a comprehensive and critical account of the current state of knowledge, indicating that client interpersonal difficulties consistently cause therapists to rate the alliance negatively. In addition, shared expectations within the dyad create a positive alliance, whilst therapeutic ruptures foster negative alliances for both parties. Section Two presents a research study exploring how clinical psychologists experience working with transferred clients. Nine participants were interviewed and the data analysed using interpretative phenomenological analysis (IPA). Four major themes emerged from the transcripts: Developing a boundaried and transparent alliance; Abandonment and raised expectations: the lasting influence of the previous therapist; Following on where somebody else left off: using or not using previous therapeutic work; and Working within the transfer culture. The themes describe the difficulty and frustration experienced within the transfer process, as clinical psychologists attempt to establish a new therapeutic alliance to promote positive change in clients. Section Three provides a critical review of the research study, specifically focusing on reflexivity. The author reflects on the experience of conducting interviews whilst having a dual-role, placing them on 'the inside' of participant accounts. In addition post-hoc reflexivity was used to provide practice guidelines for trainee clinical psychologists managing transfers of care. It is anticipated that the findings presented here will encourage clinical psychologists and associated professionals to consider the 'importance of establishing a solid therapeutic alliance, whilst also recognising the consequences of breaking this bond.

The score from the survey measured the strength of the therapeutic alliance between the therapist and client. There were 38 clients from LifeWorks, a non-profit organization, who completed the survey assigned to 11 different therapists or counseling interns. Client and therapist demographic information were compared to see if higher alliance existed among age, gender, race, ethnic congruence. Findings showed no statistically significant results but clinical significance was exhibited in similar age and ethnic congruence.

Book excerpt: Alliance and client involvement are thought to be important therapy process factors in child psychotherapy; however, few studies have investigated them over the course of treatment. The present study examined change in alliance, client involvement, and the relationship between the two over time in an effectiveness study comparing cognitive behavioral therapy (CBT) and usual clinical care (UC) for child anxiety disorders. The sample included 40 clinically-referred children (57.50%, female, mean age = 10.81, SD = 2.11, 35.00% Caucasian, 32.50% Latino/Hispanic, 5.00% African-American, 7.50% mixed ethnicity, 20.00% not reported) and 39 therapists employed by community clinics. Two doctoral-level students comprised the coding teams for each measure and independently rated alliance and client involvement in all available recorded sessions. Unconditional multilevel growth models indicated alliance and client involvement did not significantly change over time. Findings suggest that when measured by observational coders, initial levels of alliance and client involvement remain relatively stable throughout different treatments for child anxiety in community settings. Existing therapy process models may require further specification based on treatment setting and method of measurement. In practice, these findings suggest strategies to bolster initial alliance and client involvement could help improve the impact and delivery of child anxiety treatment in community settings.
