Even in sports that do not permit player contact, incidental contact that can result in concussion often occurs. This dissertation addresses the general hypothesis that sport related concussions have a negative impact on academic performance in high school athletes. Study Design. A systematic review of current literature on the post-acute neurocognitive effects of concussion provides an introduction to the importance of this study. A retrospective cohort of athletes participating in school sports in a public school district is assessed for changes in school attendance and academic performance following concussion compared to ankle or leg injury. Methods. The systematic review included meta-analytic methods. Analysis of the retrospective cohort included longitudinal graphical and multivariable random coefficient methods that characterized changes in academic performance over time in high school athletes with and without concussion. Random coefficient models were used to assess effect modification by predictor variables including gender and age. Results. Researchers have shown that adolescent athletes experience a range of neurocognitive symptoms six or more days following concussion injury. Despite growing awareness of sports-related concussions and campaigns to educate athletes, coaches, physicians, and parents of young athletes about concussion recognition and management, confusion and controversy persist in many areas. Currently, diagnosis is based primarily on the symptoms reported by the individual rather than on objective diagnostic markers, and there is little empirical evidence for the optimal degree and duration of physical rest needed to promote recovery or the best timing and approach for returning to full physical activity. Sports-Related Concussions in Youth: Improving the Science, Changing the Culture reviews the science of sports-related concussions in youth from elementary school through young adulthood, as well as in military personnel and their dependents. This report recommends actions that can be taken by a range of audiences - including research funding agencies, legislatures, state and school superintendents and athletic directors, military organizations, and equipment manufacturers, as well as youth who participate in sports and their parents - to improve what is known about concussions and to reduce their occurrence. Sports-Related Concussions in Youth finds that while some studies provide useful information, much remains unknown about the extent of concussions in youth; how to diagnose, manage, and prevent concussions; and the short- and long-term consequences of concussions as well as repetitive head impacts that do not result in concussion symptoms. The culture of sports negatively influences athletes' self-reporting of concussion symptoms and their adherence to return-to-play guidance. Athletes, their teammates, and, in some cases, coaches and parents may not fully appreciate the health threats posed by concussions. Similarly, military recruits are immersed in a culture that includes devotion to duty and service before self, and the critical nature of concussions may often go unheeded. According to Sports-Related Concussions in Youth, if the youth sports community can adopt the belief that concussions are serious injuries and emphasize care for players with concussions until they are fully recovered, then the culture in which these athletes perform and compete will become much safer. Improving understanding of the extent, causes, effects, and prevention of sports-related concussions is vitally important for the health and well-being of youth athletes. The findings and recommendations in this report set a direction for research to reach this goal.

Book excerpt: While substantial literature exists regarding the neurocognitive sequelae of concussion and return to play determinations for student-athletes who have suffered a concussion, there is a paucity of research that has conceptualized the impact of concussion on common academic outcomes. This study examined these topics in an attempt to evaluate the impact of concussion on GPA and school attendance, the association of preinjury neurocognitive performance to changes in academic achievement following a concussion, as well as the relationship between baseline neurocognitive performance and academic outcomes. The change in monthly assignment GPA and attendance were analyzed for three comparison groups (concussion, other sport-related injuries, control) regarding their differences pre and post injury. Second, whether a student-athlete's performance on a computerized baseline neuropsychological assessment (ImPACT) moderated the change in monthly assignment GPA for a group of concussed student-athletes identified as either short recovery or long recovery was investigated. Lastly, the association between baseline ImPACT scores and cumulative GPA/standardized achievement measures was examined for the entire group of student- athletes. Results of this study indicated no systematic differences between comparison groups (concussion, control, and injury) with the change in pre and post injury monthly assignment GPA and daily attendance rates, although academic declines were evident among all student-athletes. Further analysis revealed that more athletes who experienced concussions evidenced a significant drop in GPA (> .5) than would be expected by chance. Contrary to expectations, baseline ImPACT assessment scores did not moderate the degree of academic decline for concussed student-athletes within the short or long recovery groups. Book excerpt: Mild traumatic brain injuries (mTBI), often called concussions, are a growing concern for children, adolescents, and all athletes participating in contact sports, as studies show that the damage caused by a brain injury can be more serious and have a longer lasting impact than first thought. Recent lawsuits involving professional athletes have resulted in multi-million dollar settlements and new guidelines for returning a concussed athlete to sport (Strachan 2015). However, little attention has been paid to the effect of concussions on a student-athlete's return to academics. This preliminary study of Grade Point Averages (GPA) from a cohort of 26 Penn State Division I varsity student-athletes shows that mTBI has a significant impact (p=0.03) on GPA. Specifically, it was observed that despite a change in study habits and increase in study time, there was an average decrease of 0.25 (on a 4.0 scale) in GPA from the semester prior to mTBI compared to the semester of injury. The student-athletes' GPAs tended to rebound the semester following mTBI, but the trend showed that the GPAs did not quite return to pre-concussive levels. All subjects were cleared to return to play 7-10 days post injury, but it can take more than twice that amount of time to return to school (Master 2012). Hence, return to play does not equate to return to academic rigor, and ignoring this fact with premature return to school could lead to compounding effects, longer lasting impacts, and increased deficits in not only the student-athletes current grades, but also their lives beyond sport.

This study utilized the Rosenbaum Concussion Knowledge and Attitudes Survey - Student Version regarding the knowledge and attitudes high school athletes have towards SRCs to determine if they knew the severity of the possible long-term impacts. The survey was distributed to two female contact sport teams at one high school in Northeast Arkansas. The results of the study showed that high school female athletes possessed greater SRC knowledge but did demonstrate the safest attitudes towards sustaining an SRC. While the study utilized a small sample size, it revealed further research is needed regarding the quality of education high school female athletes receive on SRCs
