Book excerpt: With reductions in the age criterion for cochlear implantation, the need for age-appropriate measures of speech perception skills has increased. One recently developed tool that shows great promise for the clinical assessment of auditory speech perception capacity in young children with cochlear implants is the On-Line Imitative Test of Speech-Pattern Contrast Perception (OlimSpac). The OlimSpac requires a child to imitate nonword utterances by providing a verbal response. The child's perceptual abilities are inferred from the child's productions through having a listener, who is masked to the stimulus select the utterance produced by the child in an eight-alternative force-choice task. Although the OlimSpac has the potential for use in children with cochlear implants, the specific role of measured speech production abilities on performance has yet to be systematically examined. Thus, the main objective of the present study was to examine the influence of speech production abilities on OlimSpac performance in an auditory-visual (A-V) and auditory-only (A-O) condition in young children with cochlear implants relative to an articulation-matched sample of normal hearing peers. A secondary objective was to determine whether the presentation modality affected a child's OlimSpac performance. A matched pair design was used to compare the OlimSpac performance of ten children with cochlear implants (ages 29 to 76 months) to normal hearing peers (ages 27 to 73 months). Each child with cochlear implants was matched to a child with normal hearing from a sampled population of 22 normal hearing participants based on word-level articulation skills, as measured by the GFTA-2, accounting for hearing age and gender. The OlimSpac software generated a score for a single contrast and a single presentation modality (A-V or A-O). The score was based on eight binary trials.

Book excerpt: Profound congenital deafness corresponds to a major obstacle in oral communication because deafness has a critical influence on the development of speech and spoken language. The effects of early auditory deprivation and subsequent cochlear implantation on outcomes have been a focus of a body of research throughout the past 30 years. The earliest research produced seminal work on speech perception and articulation skills of children with cochlear implants (CIs). More recent research expanded to include detailed measures of language outcomes including phonological development and auditory working memory capacity. And over the last decade, the scope of research has expanded in several new directions including evaluating these language skills in younger children and also evaluating the benefits of bilateral cochlear implantation (BiCIs) for understanding speech-in-noise. However, there is a new population of CI recipients, toddlers with BiCIs, which has yet to be fully understood. The goal of this work is to better understand this new population by evaluating the auditory abilities of toddlers with BiCIs on both a speech discrimination task and a speech-in-noise task. These ecologically-motivated tasks each address a distinct question and converge to illuminate the auditory abilities of toddlers with BiCIs. These skills are building blocks and are a first step in investigating the effects of early bilateral cochlear implantation on language acquisition. Together, these experiments will advance knowledge on (1) speech discrimination and (2) listening in noisy environments and spatial unmasking. These data are likely to not only contribute to scientific discovery of the emergence of auditory skills in this population, but also have the likelihood of contributing to decisions regarding clinical management of this population.

General cognitive processes, especially analogical reasoning ability, are assumed to lead to the understanding of words and grammatical structures. It has been argued that the time of language or auditory deprivation children with cochlear implants (CIs) often experience early in life, might affect both their language and their cognitive abilities. Children with CIs have disadvantages in terms of language acquisition as they start receiving language input later than peers with typical hearing (TH), and the perceptual quality of the input is reduced. However, they might have additional difficulties in language acquisition if their analogical reasoning ability is negatively influenced by the language or auditory deprivation early in life. The results found in this thesis show that the language delays often seen in children with CIs are not explained by differences in analogical reasoning ability. Results indicate that analogical reasoning ability has a limited influence on language acquisition for the tested age-group. However, language abilities affect the performance on analogical reasoning tasks which are either verbal or can be supported by verbal strategies. This needs to be taken into consideration when comparing analogical reasoning ability of children with CI sand children with TH. In addition, the results from this thesis indicate that the structure of the mental lexicon is associated with the communication skills of children with CIs. The structure of the mental lexicon is most likely influenced by the amount of language input a child receives. The main implication of the results is that emphasis should be put on reducing the duration of language deprivation and to improve the quality of the language input for children with CIs.

In the last decade, advances in auditory science and technology have not only greatly expanded the utility of electric stimulation to other parts of the auditory nervous system in addition to the cochlea, but have also demonstrated drastic changes in the brain in responses to electric stimulation, including changes in language development and music perception. Volume 20 of SHAR focused on basic science and technology underlying the cochlear implant. However, due to the newness of the ideas and technology, the volume did not cover any emerging applications such as bilateral cochlear implants, combined acoustic-electric stimulation, and other types of auditory prostheses, nor did it review brain plasticity in responses to electric stimulation and its perceptual and language consequences. This proposed volume takes off from Volume 20, and expands the examination of implants into new and highly exciting areas. This edited book starts with an overview and introduction by Dr. Fan-Gang Zeng. Chapters 2-9 cover technological development and the advances in treating the full spectrum of ear disorders in the last ten years. Chapters 10-15 discuss brain responses to electric stimulation and their perceptual impact. This volume is particularly exciting because there have been quantum leap from the traditional technology discussed in Volume 20. Thus, this volume is timely and will be of real importance to the SHAR audience.
