Book excerpt: Osteoclasts and osteoblasts play critical roles in remodelling alveolar bone during orthodontic tooth movement (OTM). The objectives of this study were (i) To characterize the spatial/temporal pattern of osteoblast and osteoclast differentiation in the periodontal ligament (PDL) during OTM, and (ii) To determine if OTM acceleration could occur with PDL administration of an osteoclastic differentiation factor, Macrophage-Colony Stimulating Factor (M-CSF). Rat/mouse OTM models were used to characterize the expression of osteoblast and osteoclast differentiation markers. Expression of markers involved in recruiting osteoclast precursors and markers found on preosteoclasts, but not monocytes, were found in the PDL during early stages of OTM. We found up-regulated expression of genes downstream of M-CSF and an increase in OTM rate after M-CSF administration. Taken together, our results showed that improved knowledge of the molecular basis of cellular activities is critical towards the development and testing of pharmacological strategies as a means to accelerate OTM.

Little is known about the biological basis of relapse. Proper characterizations and quantification of osteoblast differentiation in a mouse model of orthodontic relapse have yet to be demonstrated. It is the goal of this study to characterize and quantify osteoblast differentiation in orthodontic relapse in an in vivo transgenic mouse model. Transgenic mice with GFP bound to Bone Sialo Protein (BSP) promoter marker, a protein expressed by early osteoblast lineage cells were used. It is hypothesized that GFP-BSP will be decreased on the compression side of the furcation of the mouse maxillary first molar when compared with the contralateral control molar in both orthodontic tooth movement and relapse. Nineteen 10-12 week old mice were divided into two groups: 1.) 2 weeks of OTM applied by spring, 2.) 10 days OTM followed by spring removal and 4 days of relapse movement. TUNEL staining was performed to evaluate apoptosis. Results show a marked decrease in BSP-GFP on the compression side and tension side after 2 weeks of OTM. A significant decrease was also seen on the compression side after 10 days of OTM and 4 days relapse. There was a significant increase in apoptosis on the compression side after OTM but not after relapse.

A multidisciplinary approach is often necessary to treat and prevent dental problems in a patient. Orthodontics and periodontics are interrelated in a variety of ways. Orthodontic treatment is based on the principle that if prolonged pressure is applied to a tooth, tooth movement will occur as the bone around the tooth remodels. Bone is selectively removed in same areas and added in others. In essence, the tooth moves through the bone carrying its attachment apparatus with it, as the socket of the tooth migrates. Since the bony response is mediated by the periodontal ligament, tooth movement is primarily a periodontal ligament phenomenon. Orthodontic tooth movement, which is the basis of the orthodontic treatment, is possible because of the inherent nature of the periodontium. Many advances have been made in understanding the mechanisms involved in the process of tooth movement. However, there are many pathologic conditions affecting the periodontium which may affect these mechanisms of tooth movement and alter the end result of the orthodontic treatment. Similarly awareness of the pathologic changes or other undesirable changes which can occur in the periodontium as a result of ideal or less than ideal orthodontic procedures would help in better treatment procedure and management of the patients. Altered tooth position may affect periodontal health. Placement of orthodontic appliances causes microbiological changes as well as changes in the periodontium. With the success, in the recent decades of the fixed multi banded appliance, orthodontic treatment in adults has continuously grown in the United States and Europe. It is in connection with adult orthodontics that periodontal factors are becoming more important to the orthodontist. Orthodontics in the adult patient, specially the periodontally compromised patient requires adequate periodontal considerations to maintain the periodontium in a healthy condition during and after treatment. Minor periodontal surgery may be required to prevent relapse after orthodontic treatment. In addition, since periodontal diseases can secondarily cause malocclusion, very often orthodontic treatment would be an essential adjunct if the periodontal therapy is to succeed.
