African-Americans are more likely to suffer from diabetes-related complications and experience a higher diabetes-related mortality rate than Caucasians. There is a definite need to better understand the relationship of psychosocial variables to diabetes control in these two racial groups. The purpose of this two-group, comparative, descriptive study was to examine the relationships of social support, self-efficacy, and outcome expectations (beliefs that diabetes-related behaviors will lead to certain outcomes) to self-care behaviors and glycemic control in Caucasian and African-American adults with type 2 diabetes. A convenience sample of 91 Caucasian and African-American subjects receiving health care at three outpatient facilities participated in the study. All subjects completed four self-report measures: Social Support Questionnaire, Self-Efficacy Questionnaire, Outcome Expectancy Questionnaire, and the Diabetes Activities Questionnaire at the time of the outpatient visit. Long-term glycemic control was assessed by glycosylated hemoglobin analyses at the time of the outpatient visit. Two-sample t-tests revealed no significant differences between the two racial groups in regards to age (p = .81), duration of diabetes (p = .69), and glycosylated hemoglobin (p = .85). Book excerpt: Type 2 diabetes is a chronic metabolic disease that can be maintained with adjustments in lifestyle and medical intervention that has been proven to minimize complications that occur over time if the disease is not controlled. The benchmark of determining diabetic status is maintenance of glycemic control, which frequently involves radical lifestyle changes, medication management, and stress reduction. Incidence of Type 2 diabetes in underserved minority populations is increasing rapidly; subsequently, those lacking financial resources, medical insurance coverage, education, and family support have even lower rates of glycemic control. Psychological, social, and physical stressors contribute negatively to treatment adherence and overall disease outcome. Uninsured/underinsured minorities have more difficulty achieving and maintaining glycemic control and therefore have poorer overall diabetes outcomes when compared with diabetics who have adequate resources. In order to improve long-term treatment prognosis in this population, current methods of assessing psychosocial factors should be tailored to meet the special needs of this population. The purpose of the current study aimed to investigate the relationship between control of Type 2 diabetes and psychosocial factors in a group of uninsured/underinsured diabetic individuals receiving care at an urban public hospital. Medical records, demographic data, and information obtained from self-report instruments were collected at initial outpatient visits. The current study involved analysis of three self-report instruments designed to measure emotional, psychiatric, and general difficulties involved in maintaining the recommended diabetic lifestyle. From the 62 participants who met the inclusion criteria, completed the questionnaires and self-reports instruments, a total of 50 participants were randomly assigned to either a control or experimental group. These 50 participants were assigned to either a single session of Motivational Interviewing (MI) or psychiatric intake interview/treatment as usual (TAU) in order to determine the effect of brief MI on glycemic control and readiness to change health related behaviors. Information gathered from the remaining 12 participants was included in all but the MI/TAU data analyses. All participants completed questionnaires and self-report instruments of the Problem Areas in Diabetes (PAID), Brief Symptom Inventory (BSI), Stages of Change (SOC), and Diabetes Knowledge Test (DKT). Data was examined using regression and correlation models to investigate the predictive value of psychosocial factors on glycemic control. Results of this study did not successfully demonstrate a significant relationship between glycemic control (HbAlc) emotional stress; psychiatric symptoms and general problems related to diabetes in the population sampled nor did they illustrate a significant relationship between glycemic control and readiness to change health related behaviors or additional variables such as employment status, and activity level. Cultural factors affecting the current findings are discussed along with the importance of exploring ethnic variables related to lifestyle changes and limitations of current treatment protocols. Recommendations for future research including underserved minority populations are addressed.

In this work we are showing importance of social support for improving the quality of life for a diabetic patients.Aim: Determine the importance of different types of social support to improve the quality of life of diabetic patients using the new tool DIASSQL 23/20 (Diabetes Social Support Quality of Life 23/20) drawn up for the implementation of the milestones of the research.Method: In the first part we are describing epidemiologic course of diabetes mellitus. In the second part we are determining emotional, informational, instrumental and evaluating social support, provision, admission, perception, availability, obtainability of social support, level of social support in context of social needs. In the third part we are determining quality of life, aspects of quality of life and benefit of groups in system of quality of life. In fourth part we are focusing on research and the seven individual aims of the research and five hypothesis. We specify of the research, research models, methods of research, organization and analysis of the research in statistical program Statistical Package for Social Science SPSS. In the survey section, DIASSQL 23/20, we used the method of qualitative research in the form of a combined system of an anonymous questionnaire. Survey sample DIASSQL 23/20 completed of 160 respondents, 143 respondents participated in the survey. DIASSQL 23/20 contains 23 social support items, 20 qualiy of life items and 9 sociodemographic survey questions, which we have obtained together 17 303 informations. The results we processed by the software program Microsoft Excel, statistical software program Statistical Package for the Social Science SPSS and descriptive and inductively statistics into tables and graphs summarize in the discussion and suggested recommendations for practice.Results: We are evaluating the results of the research, appointing practical advantages and recommendations for the daily practice within the modern health education and application of the social work. We could show importance of social support in improving quality of life of diabetic patients. Social support is essential for improving the quality of life for diabetic patients and compensation of diabetes mellitus. New instrument DIASSQL 23/20 is suitable for use in the context of diabetology evidence-based and diabetes education. Will provide modern diabetology other options like even more to help diabetic patients who really need it.Discussion: Chronic disease causes significant interference in the daily lives of individuals. A diabetes mellitus for its pandemic character not only a crisis, stress and stress situation, but as a civilization, a chronic, metabolic, non-communicable diseases with long-term course, economic, psychological, social and health consequences of the deterioration of the overall quality of life and global problem. Namely negative impact on the life of individuals, families, groups, communities and of society.
