Book excerpt: The Female Athlete Triad (Triad), a syndrome of low energy availability, menstrual dysfunction, and low bone mineral density (BMD), affects up to 60% of exercising girls and women. As such, understanding the physiology underlying the Triad is imperative in order to identify therapeutic targets and develop effective treatment strategies to prevent long-term health detriments in afflicted women. Bone health is a major clinical outcome of concern in female athletes and exercising women with the Triad, as bone loss or suboptimal peak bone mass development may predispose women to osteoporosis and related fractures later in life. To garner a better understanding of factors contributing to bone health and to inform guidelines for treatment targets in female athletes and exercising women with the Triad, this purpose of this dissertation was 1) to assess the unique and combined effects of energy status and estrogen status on volumetric BMD (vBMD), bone geometry, and estimated bone strength in exercising women, 2) to assess the unique and combined effects of energy status and estrogen status on bone turnover in exercising women, 3) to evaluate predictors of change in bone turnover dynamics across a 12-month nutritional intervention in women with exercise-associated menstrual disturbances, and 4) to determine if bone turnover dynamics at baseline impact BMD outcomes in women with exercise-associated menstrual disturbances participating in a nutritional intervention. Key findings of the studies that made up this dissertation implicate that energy status is a main driving factor dictating vBMD, bone geometry, estimated bone strength, and bone metabolism in exercising women. The results indicate that improving energetic status will not only improve bone turnover at the cellular level, but that bone turnover favoring bone formation over bone resorption will also predict improved BMD in the following year. The findings support the guidelines put forth by the Female Athlete Triad Coalition that endorse nutritional therapy as the first line of treatment for the Triad, as low energy availability is the root cause of the clinical sequelae, including menstrual dysfunction and compromised bone health.

With the rise of the aging population, the prevalence and cost of osteoporosis is sure to increase as well. The best method of preventing osteoporosis is building strong bones from a young age. Exercise can be used as a factor to build stronger bones at any age. Percussive activity has been shown to lead to increased bone mineral density (BMD), which explains why female athletes tend to have higher BMD than age-matched sedentary controls. This thesis presents a study comparing the BMD of female athletes in hockey and synchronized swimming to sedentary controls. 70 participants, 24 hockey players, 20 synchronized swimmers, and 26 sedentary controls participated in this study. Participants underwent three DXA scans (one total body and two regional scans) and answered a questionnaire about their menstrual history. BMD values were analyzed using ANCOVA controlling for age and weight. Predictors of BMD were also analyzed using step-wise regression. Book excerpt: Background: Much of the nutrition research surrounding bone health in athletes has been limited to the influence of eating disorders, calorie restriction, or poor energy balance in conditions such as the Female Athlete Triad or Relative Energy Deficiency in Sport (RED-S). From other population research, it is purported that dietary factors such as calories, protein, calcium, and vitamin D are critically important to bone health. However, less research has explored the role of these factors in the bone health of adult pre-menopausal athletes. Objective: The purpose of this study was to analyze the relationship of select nutritional variables with BMD at various skeletal sites. Methods: This study recruited a total of 101 adult pre-menopausal female recreational runners. Baseline laboratory visits included anthropometry, dual-x-ray absorptiometry (DXA), a three-day food record, and phlebotomy. Thirty-nine subjects participated in the follow up laboratory visit which included the same procedures except for phlebotomy. The 3-day food record was entered into ESHA Food Processor (SQL version 10.2) to determine average daily intake of calories, calcium, and protein while 25-hydroxyvitamin D levels were obtained from blood. DXA scan sites included non-dominant hip, femoral neck, lumbar spine, distal forearm, and total body. Statistical analyses employed SPSS (version 28) to determine if significant correlations exist between nutritional variables of interest with BMD. Additionally, change variables from calories and dietary protein intake were compared to BMDs from baseline to follow-up. Results: The select nutritional variables were not significantly correlated with BMD. However, calories did show a trend for a small positive correlation with TB (r=0.188;p=0.060). Regression models for prediction of total body (TB) BMD demonstrated lean body mass (LBM), calories, and protein as a reasonable prediction model. Conclusions: Despite poor correlations between nutritional variables and BMD sites, prediction models indicated the importance of LBM, calories, and protein as potential nutritional variables to help target BMD outcomes.

Book excerpt: The results of several recent research studies have questioned the benefit of high levels of running and its influence on bone mineral density. Elite endurance athletes often experience high physiological stress, hormonal imbalances, and nutritional inadequacies during competitive seasons as a result of training and competing. The effect of these factors on bone mineral density is not well understood. Purpose: To examine changes in body bone mineral density (BMD) in male and female collegiate cross country runners over the period of a competitive season (pre- post CC). Methods: Subjects were 23 (11 male and 12 female) Division I collegiate cross country runners ages 18-22 yr. BMD was measured using dual x-ray absorptiometry (DXA) by the GE Lunar iDXA Bone Densitometer. BMD was measured in the total body, spine, hips, and non-dominant forearm. Body fat percentages were measured using DXA and standardized skin fold measures. Maximal oxygen uptake (VO2 max) was measured using a maximum treadmill test. Caloric intake was calculated from a 7-day food log and running mileage quantified using self-reported training logs. Results: Due to lack of adherence to study requirements only 8 males and 8 females were evaluated for changes from pre-post CC. As expected, at pre- CC males were taller, heavier, running more miles, and had a higher VO2 maximum than females. From pre- to post CC season males did not have any changes in body composition whereas females had an increase of 2.7 ± 1.7 lbs or 2.1% of total mass. From pre- to post season males did not have any significant changes in BMD whereas females had a significant decrease in BMD of L1, L1-2, and L1-3 vertebrae. As a group, males and females did not have any BMD value that significantly differed from the reference population but 4 males and 4 females had total BMD that were lower than "normal". At pre- CC male total body BMD and forearm BMD positively correlated with lean body mass whereas hip BMD negatively correlated with running mileage. At pre- CC female hip BMD was negatively correlated with % body fat. From pre- to post CC males showed significant decreases in vitamin D levels whereas females showed significant increases in iron and magnesium levels. Conclusion: Despite limitations, this study adds substantially to our understanding of BMD levels of competitive collegiate CC runners. The results of this study suggest that a competitive CC season has minimal negative or positive impact on BMD on collegiate CC athletes. However, approximately 50 % of the male and female athletes in this study have BMD levels below their age-matched normal levels. The low BMD levels seen in these distance runners may increase their risk of stress fractures and/or osteoporosis later in life. Further investigation of this important topic is warranted and should be performed on a larger and more diverse group of collegiate distance runners.

Physical activity (PA) is an important step in preventing osteoporosis and is one of the major determinants of peak bone mass during adolescence. However, type of PA plays an important role when examining the effects of PA on bone mineral density (BMD). PURPOSE: 1) To determine the differences in BMD among adolescent female tennis players (TP) and non-tennis players (NTP); 2) to determine the differences in body composition (BC) between adolescent female TP and NTP; 3) to determine whether BC variables (weight, fat mass, and lean mass) were predictive of differences in BMD; and 4) to assess osteoporosis knowledge among female high school adolescents. METHODS: Nineteen female TP and 19 female NTP, aged 14 to 18 years, participated in this study. Lumbar spine, total hip, femoral neck, and left and right forearm BMD, and BC were assessed using dual-energy x-ray absorptiometry. Total osteoporosis knowledge (TOK) was assessed by the Osteoporosis Knowledge Test. One-way ANOVA was used to assess differences between TP and NTP for BMD, BC, and OKT . Multiple regression analysis was used to identify significant predictors of BMD. RESULTS: TP had significantly greater femoral neck BMD than NTP (p = 0.017). In addition, when data were analyzed with an outlier excluded, TP also had significantly greater total hip BMD than NTP (p = 0.02). Although TP had greater BMD for lumbar spine, and dominant arm measurements, these differences were not significantly greater than NTP. There were no significant differences for total body percent fat (p = 0.72), total body lean mass (p = 0.07) or total body fat mass (p = 0.59) between the groups. Collectively, body composition variables (body weight, total body lean mass and total body fat mass) significantly predicted BMD at the femoral neck and hip with lean mass being the best predictor among the three independent variables. A moderate level of knowledge related to osteoporosis was found among female adolescents. In addition, there were no significant differences between TP and NTP for TOK. CONCLUSION: This study suggests that adolescent female TP have greater femoral neck and total hip BMD than NTP. This difference might play an important role in preventing osteoporosis and decreasing the risk of fractures at the hip later in life.
