Avec une incidence de 355 000 nouveaux cas par an, en augmentation depuis 30 ans, il s'agit d'un problème de santé publique. Le médecin généraliste possède un rôle majeur dans la prise en charge des patients atteints de cancer et un des objectifs des Plans Cancer est de le renforcer. Cependant les nombreux aspects de cette prise en charge sont parfois à l'origine de difficultés pour le médecin généraliste, par le côté chronophage mais surtout à cause de la lourdeur de la charge morale. Le dosage des marqueurs tumoraux constitue une part de l'activité de cancérologie et est encadré par des recommandations émises par les sociétés savantes. Plusieurs travaux ont démontré qu'en pratique elles n'étaient pas toujours suivies, même en milieu hospitalier spécialisé. Méthode : Nous avons réalisé une étude descriptive transversale par questionnaire auprès des médecins généralistes de la région Nord-Pas-de-Calais. L'objectif principal était d'évaluer les pratiques et besoins des médecins généralistes vis-à-vis du dosage des marqueurs tumoraux. L'objectif secondaire était de créer un document synthétique à l'attention des médecins généralistes regroupant les recommandations de dosage des marqueurs tumoraux émises par l'HAS et l'INCa. Résultats : 42 médecins ont répondu au questionnaire de l'étude. En moyenne les pratiques des médecins généralistes de la région vis-à-vis du dosage des marqueurs tumoraux seraient entre 5 et 70% des cas considérées hors recommandations. 60% des médecins interrogés ont répondu qu'ils ne se sentaient pas à l'aise vis-à-vis des indications de dosages des marqueurs tumoraux et 85% d'entre eux ont déclaré qu'ils utiliseraient un outil synthétique regroupant les recommandations de bonne pratique. Conclusion : Les principales causes évoquées pour expliquer le manque de suivi des recommandations sont d'une part le manque de connaissance et d'utilisation des sources d'informations et d'autre part la difficulté face au patient demandeur, et souvent inquiet, de refuser des examens complémentaires. Pour tenter de palier à cela, nous avons décidé de créer un outil synthétique à l'attention des médecins généralistes regroupant les indications de dosage des marqueurs tumoraux, pas type de cancer, utilisable facilement en pratique courante.
