Quatre services d’urgences psychiatriques indépendants fonctionnant 24 heures sur 24 dans des milieux urbains et suburbains/ruraux comparables sont sélectionnés aux États-Unis et en France: le Centre Intégré d’Urgences Psychiatriques (CPEP) de l'hôpital Bellevue à New York et le Centre Psychiatrique d’Orientation et d’Accueil (CPOA) à l’hôpital Sainte-Anne à Paris en milieu urbain ; le Psychiatric Emergency Service (PES) au Centre Médical de l’Université du Michigan et l’Unité Accueil Urgences du Centre Hospitalier Interdépartemental de Clermont de l’Oise en Picardie en milieu suburbain et rural. Chaque service a répondu à un questionnaire épidémiologique et d’organisation au cours d’une session d’observation de plusieurs jours menée par un opérateur unique. Les deux services américains sont adossés à des services d’urgences médicales; les deux services français sont adossés à des hôpitaux psychiatriques. Les différences d’organisation s’établissent davantage entre services urbains et suburbains/ruraux qu’entre services américains et français. Les deux services urbains prennent en charge une large population de patients sans domicile fixe (40% au CPEP, 30% au CPOA) et consommateurs de toxiques (59% au CPEP, 30% au CPOA). Ils proposent une prise en charge complète avec unité d’hospitalisation de crise, consultation de post-urgence et unité mobile de crise. Les deux services suburbains/ruraux disposent de moyens moins importants mais font preuve de davantage de mobilité: unités satellites dans les services d’urgences médicales avoisinants pour l’UAU, partenariat avec une équipe mobile de crise fonctionnant 24h/24 et 7J/7 pour le PES. Les pratiques professionnelles sont similaires dans les quatre services à l’exception de la prise en charge sociale: les assistants sociaux sont très présents dans les deux services américains et totalement absents dans les deux services français. Les quatre services bénéficient d’un financement public total ou partiel. En revanche les deux services français sont gratuits là où les deux services américains sont payants. Les orientations d’aval sont homogènes dans les quatre services avec des taux d’hospitalisation entre 31% et 43%. La psychiatrie d’urgence est une médecine située au carrefour de la psychopathologie, de la Loi, de l’Ordre et de la vie sociale. Elle est, au cours des dernières années, passée avec succès d’un modèle de triage à un modèle de traitement des malades. Les services d’urgences psychiatriques américains et français sont des services publics universels. Ils s’organisent en fonction de la population de patients qu’ils servent. On retient la gratuité des services français et l’implication des assistants sociaux dans les services américains.

The book examines grassroots building of multi-layered water-rights territories, and State, market and expert networks’ vigorous efforts to reshape these water societies in their own image – seizing resources and/or aligning users, identities and rights systems within dominant frameworks. Distributive and cultural politics entwine. It is shown that attempts to modernize and normalize users through universalized water culture, ‘rational water use’ and de-politicized interventions deepen water security problems rather than alleviating them. However, social struggles negotiate and enforce water rights. User collectives challenge imposed water rights and identities, constructing new ones to strategically acquire water control autonomy and re-moralize their waterscapes. The author shows that battles for material control include the right to culturally define and politically organize water rights and territories. Andean illustrations from Peru, Ecuador, Bolivia and Chile, from peasant-indigenous life stories to international policy-making, highlight open and subsurface hydro-social networks. They reveal how water justice struggles are political projects against indifference, and that engaging in re-distributive policies and defying ‘truth politics,’ extends context-particular water rights definitions and governance forms.
