Yet, little is currently known on the role of health insurance in facilitating access to care for children. Exploiting a regression discontinuity design made possible through a policy to provide health insurance to pre-school aged children in Vietnam, this paper evaluates the impact of health insurance on the health care utilization outcomes of children at the eligibility threshold of six years. Using three rounds of the Vietnam Household Living Standards Survey, the study finds a positive impact on inpatient and outpatient visits and no significant impact on expenditures per visit at public facilities. We find moderately high use of private outpatient services and no evidence of a switch from private to covered public facilities under insurance. Results suggest that adopting public health insurance programs for children under age 6 may be an important vehicle to improving service utilization in a low- and middle-income country context. Challenges remain in providing adequate protections from the costs and other barriers to care.

Book excerpt: The Children's Health Insurance Program was crafted in a period of intense partisan and ideological controversy over health care entitlements to provide -creditable coverage- for American children below 200 percent of the Federal Poverty Level. This objective was widely supported, though achieved only by a compromise between the structural alternatives of a block grant, similar to the Maternal and Child Health Block Grant or an entitlement resembling Medicaid. According to David G. Smith, the CHIP compromise has been a successful experiment that far exceeded expectations, both in identifying and enrolling -targeted low-income children- and in earning political capital. He argues that beyond this core mission, the reauthorization of CHIPRA (Children's Health Insurance Program Reauthorization Act of 2009) invites a larger mission: going beyond enrollment of children to include assuring access, improving quality, and containing costs of health care for them. Extending this thrust, the author notes that CHIP could be used to establish children's health as a niche--much like care for the elderly--within the larger scheme of health care insurance for all. Several areas of successful performance needed for the program to be adjudged a success as well as its limitations are discussed in the book. These areas include initial implementation, enrolling kids, federal-state relations, and the uses and misuses of waivers to modify the program. A description of changes made by the CHIPRA reauthorization and the new Patient Protection Affordable Care Act (PPACA) is also included. This is followed by a consideration of lessons learned from CHIP's evolution and recommendations for future development. In short, this is a valuable and readable account for those interested in the current and future trends of health care for the young.

Book excerpt: Public health insurance programs comprise a large share of federal and state government expenditure, and these programs are due to be expanded as part of the 2010 Affordable Care Act. Despite a large literature on the effects of these programs on health care utilization and health outcomes, little prior work has examined the long-term effects of these programs and resultant health improvements on important outcomes, such as educational attainment. We contribute to filling this gap in the literature by examining the effects of the public insurance expansions among children in the 1980s and 1990s on their future educational attainment. Our findings indicate that expanding health insurance coverage for low-income children has large effects on high school completion, college attendance and college completion. These estimates are robust to only using federal Medicaid expansions, and they are mostly due to expansions that occur when the children are older (i.e., not newborns). We present suggestive evidence that better health is one of the mechanisms driving our results by showing that Medicaid eligibility when young translated into better teen health. Overall, our results indicate that the long-run benefits of public health insurance are substantial.

Book excerpt: The goal of my research is to explore ways to improve the welfare of populations that are targeted by public programs. In particular, my thesis consists of three chapters on health insurance and education. The first chapter looks at Medicaid take-up decisions among poor children. Medicaid is public health insurance that is available to low income individuals, and it is provided freely by the government. However, there is a puzzling observation that many low-income children are uninsured despite their eligibility for Medicaid. As one possible explanation, I propose that the low level of access to health care that Medicaid provides can explain incomplete take-up. Existing literature suggests that the low level of Medicaid fee payments to physicians reduces their willingness to see Medicaid patients, creating an access-to-care problem for these patients. Using variation in the timing of the changes in Medicaid payments across states, I find that improving Medicaid generosity increases the take-up rate and reduces the uninsured rate among poor children. These findings provide a partial explanation for why Medicaid-eligible children in poverty remain uninsured. While my first chapter focuses on traditional means-tested public health insurance which targets mainly low income families, the second chapter explores the issues with a more recent intervention that extends beyond low income families. In recent years, several states have allowed young adults as old as 30 to remain covered under their parents' employer-provided health insurance. For those who qualify for these benefits, the expansion of parental coverage partially reduces the value of being employed by a firm that provides health insurance since adult children can now get health insurance through another channel. We employ quasi-experimental variation in the timing and generosity of states' eligibility rules to identify the effect of the policy change on young adults' labor market choices. Our results suggest that the expansion of parental coverage increases the group coverage rate and reduces labor supply among young adults, particularly in full-time employment. The third chapter analyzes the effect of educational tracking by decomposing it into the separate roles of peer effects and coursework. The practice of tracking often results not only in grouping students by different ability, but also in providing different types of coursework for students. For instance, the advanced track may have both higher achieving peers and higher level coursework. Using detailed panel data from the San Diego Unified School district, I find that having high achieving peer is beneficial, while I do not find convincing evidence that taking more advanced math coursework predicts student's test score.

Book excerpt: The State Children's Health Insurance Program (SCHIP) was established by Congress to provide health insurance to uninsured children whose family income was too high for Medicaid coverage but too low to allow the family to obtain private health insurance coverage. The enabling legislation for SCHIP, included in the Balanced Budget Act of 1997, made available to states (and the District of Columbia) almost $40 billion over a 10-year period for this program. Like Medicaid, SCHIP is a joint federal-state program, with funding from both sources, but it is implemented by the states. Thus, there are SCHIP programs in all of the states and the District of Columbia. The National Research Council, through the Committee on National Statistics, was asked to explore some of the ways in which data analysis could be used to promote achievement of the SCHIP goal of expanding health insurance coverage for uninsured children from low-income families. To inform its work, the panel for this project held a workshop to bring together state SCHIP officials and researchers to share findings and methods that would inform the design, implementation, and evaluation of SCHIP at the state and national levels. In keeping with this charge, this report is limited to discussions at the workshop. It does not attempt to provide a summary of all the state programs nor a comprehensive review of the literature. Data Needs for the State Children's Health Insurance Program concludes that data are insufficient in the individual states to provide a clear picture of the impact of SCHIP on the number of children who are eligible for the program, the rate at which eligible children are enrolled in the program, and the rate at which they are retained in the program once enrolled. This situation is due, in part, to the fact that sample sizes in national surveys are too small to provide detailed data for individual states. In addition, the great amount of movement of children among health insurance categories-Medicaid, SCHIP, private insurance, or no insurance at all-makes it difficult for states to count the number of children in specific categories at a particular point in time. The panel specifies a number of practices that could be implemented to improve the overall functioning of SCHIP and the ability of policy makers to evaluate the program. Foremost among these are: (1) developing more uniform ways of estimating eligibility and health insurance coverage among the states; (2) sharing among the states effective methods for outreach; (3) taking qualitative information into account, in addition to quantitative information, in assessing variation among states in enrollment and disenrollment; and (4) implementing longitudinal studies to track the movement of children among the various insurance statuses.
