Available in PDF, EPUB and Kindle. Book excerpt: Directional vacuum assisted breast biopsy (VAB) is a modified and improved technique of large core breast biopsy. It allows faster acquisition of contiguous, multiple cores with single needle insertion. A 10-fold greater tissue volume is obtained per core with VAB compared with core needle biopsy. VAB can be used under stereotactic, ultrasound or MRI guidance. Large samples make VAB almost equivalent to surgical biopsy. VAB is an accurate and reliable technique to obtain histologic diagnosis of malignant or benign breast lesions and assists therapeutic decisions. We conducted a study to assess the role of VAB in patients with previously equivocal biopsies for breast masses suspicious for cancer.

Available in PDF, EPUB and Kindle. Book excerpt: Directional, vacuum-assisted breast biopsy is a procedure for undertaking biopsy for breast lesions including microcalcifications, masses, spiculated masses, asymmetric densities and multifocal disease. The procedure may be assisted by imaging guidance such as ultrasound and X-ray. It is diagnostic procedure and is not, at this time, intended for therapeutic use. The evidence available shows that directional, vacuum-assisted biopsy is safe with a low minor complication rate and absence of major complications. When compared to core biopsy, directional, vacuum-assisted breast biopsy has a higher success rate for the removal of microcalfications, is able to obtain larger numbers of specimens at biopsy, and has increased sensitivity in the detection of ductal carcinoma in situ and atypical ductal hyperplasia. No cost-effectiveness analysis was undertaken. The disposals used for directional vacuum-assisted breast biopsy are, however, more expensive than those used for core biopsy.

Book excerpt: "This project was commissioned by the National Breast Cancer Centre (NBCC). The objectives of the project, as set out in the call for expressions of interest, were to determine:> The costs associated with the introduction and use of directional vacuum-assisted breast biopsy (DVA breast biopsy) in Australia; and> Whether directional vacuum-assisted breast biopsy used for diagnostic purposes is cost-effective in Australia when compared to core biopsy. The motivation for commissioning the project was an assessment of directional vacuum-assisted breast biopsy conducted by the Medical Services Advisory Committee (MSAC) which concluded that the procedure is safe and more effective than core biopsy. Although a cost-effectiveness analysis was not conducted as part of the MSAC study, MSAC recommended that the costs associated with the procedure be investigated and that, pending a review of costs, the procedure receive interim Medicare funding at a higher level than was previously available"--Foreword.

Available in PDF, EPUB and Kindle. Book excerpt: Modern imaging methods have made it possible to detect breast cancer at an earlier stage than in the past. Nevertheless, a large majority of suspicious findings at screening subsequently prove to be benign. It is therefore important to be able to identify benign lesions in a manner that is reliable, tissue sparing, patient friendly, and cost-effective. More than 70% of breast biopsies can now be performed using minimally invasive procedures that meet these criteria. This book examines in detail vacuum-assisted minimally invasive breast biopsy systems (ATEC, EnCor, Intact, Mammotome and Vacora), stereotactic systems, MRI-guided procedures, and ductoscopy. Further chapters are devoted to the pathology of the breast tissue obtained using these procedures, their limitations, the implications of recent advances in breast imaging, and the results of cost-benefit analyses. The closing chapter provides a systematic review and meta-analysis of recent data.

Emergency medicine is a specialty where time and knowledge are critical factors in deciding appropriate management which could otherwise result in loss of life or limb. The challenge often is to have lucid management plans, whilst standing at the bedside of the patient. In order to address this challenge, a manuscript is needed which aims to enhance the clinical skills of the emergency physician. The objective of this book is to compile a road map for practitioners of emergency medicine, which would guide them through algorithm-based pathways. This format is distinctive by nature for its concise presentation, which facilitates easy reading and early application. Written by global experts, this book aims to be a truly international representation of emergency physicians who have come together to deliver contemporary concepts in emergency patient care.

Book excerpt: A practical guide for the diagnostic surgical pathologist, this new edition of Biopsy Interpretation of the Breast presents the diverse spectrum of pathologic alterations that occur in the breast in a manner analogous to that in which they are encountered in daily practice. Lesions are grouped together according to their histologic patterns rather than by the traditional benign-malignant categorization in order to simulate the way pathologists face these lesions as they examine microscopic slides on a daily basis. The role of adjunctive studies in solving diagnostic problems in breast pathology is emphasized where appropriate. In addition, the clinical significance and impact on patient management of the various diagnoses are discussed and key clinical and management points highlighted.

Book excerpt: Each volume in this richly illustrated series, published in association with the Papanicolaou Society of Cytopathology, provides an organ-based approach to the cytological and histological diagnosis of small tissue samples. Benign, pre-malignant and malignant entities are presented in a well-organized and standardized format, with high-resolution color photomicrographs, tables, and lists of key specific morphologic criteria. Example vignettes allow the reader to assimilate the diagnostic principles in a case-based format. This volume provides comprehensive coverage of both surgical pathology and cytopathology of breast lesions. With a focus on malignant tumors, the full spectrum of inflammatory disorders, benign lesions, and hyperplasias are also covered in detail. Advantages and disadvantages of aspiration and core biopsy are discussed, as well as ancillary testing such as hormonal and molecular markers. With over 200 printed photomicrographs and a DVD-ROM offering all images in a downloadable format, this is an important resource for all anatomic pathologists.
