Book excerpt: In spite of old vintage and 200 years of clinical use, digitalis remains an interesting therapeutic agent, to clinicians as well as to the pharmacologist, the biochemist, and colleagues in other diciplines of theoretic medicine. When a drug, however, has so many attractive facets, it seems proper and aJvis able for the success of a scientific meeting to focus on a number of well-defined aspects. This symposium was devoted to pharmacokinetics, drug metabolism, analytic procedures, blood level determinations, and their interpretation both for thera peutic and toxic situations. Considerable progress has been made during the last years in this area of digitalis research. The time was suitable for a critical reap praisal of facts and theories and for future planning. Our main intention was to relate analytic data and biochemical findings to clinical problems and questions. Despite the undoubtedly basic character of clinical pharmacology, it is neverthe less an applied science which should help to develop the rational basis of thera peutics. We are particularly grateful to the active participants who bore the burden of preparing presentations and - even worse - manuscripts. At the same time we are well-aware that many other active research groups would have been able to contribute in this way, but our program was limited because of the short time available. Their knowledge is included in the discussion parts of the meeting, so we hope a well-balanced description of the present state of affairs emerged in this volume.

However, in recent years acute heart failure has become a growing area of study and this is the first book to cover extensively the diagnosis and management of this complex condition. The book reflects the considerable amounts of new data reported and many new concepts which have been proposed in the last 3-4 years looking at the epidemiology, diagnostic and treatment of acute heart failure.

The most widely used methods to analyse drugs in biological fluids. i.e. chromatographic methods, CE and immunoassays are described in detail. For important drugs, an overview about the methods available and a comparison of the techniques should be given to enable the reader to choose the right method depending on laboratory equipment, staff, the aim of the investigation etc. Other general aspects important for conducting therapeutic drug monitoring or pharmacokinetics studies are also covered, i.e. sample preparation, validation of the analytical methods and pharmacokinetic methods for interpreting the data. Areas where therapeutic drug monitoring is used frequently such as antibiotics, immunosuppressant drugs, antipsychotic and anticancer drugs will be discussed in detail. In addition, the important field of phenotyping and genotyping for therapy optimisation with special focus on real-life applications is also covered. The book contains important information for analyst working on drug analysis in clinical chemistry, hospital pharmacists involved in therapeutic drug monitoring, other pharmacists, chemists or physicians working on pharmacokinetic studies in industry or academia. In contrast to other books in this field, this book provides up-to-date information regarding both methodology and clinical applications. Book excerpt: The purpose of this book is not to discuss in depth various topics in medicine nor to describe in detail all possible controversial subjects in car diology. The primary intention is to describe common cardiac problems with significant controversial viewpoints frequently encountered in our daily practice. This book presents 19 chapters, including the Coronary Arteries in Fatal Coronary Events, Prophylactic Antiarrhythmic Therapy in Acute Myocar dial Infarction, Mobile Coronary Care-Is It Really Needed?, The Use of Artificial Pacemakers in Acute Myocardial Infarction, Treatment of Car diogenic Shock, Serum Digitalis Level-Practical Value, Factors Modifying the Efficacy of Digitalis, Hyperlipidemia and Vascular Disease, Antianginal Agents for Coronary Heart Disease, Anticoagulation Therapy for Coronary Heart Disease, When to Operate on Congenital Heart Diseases, Indications for Coronary Artery Surgery and Patient Selection, Car diomyopathy: Diagnostic Criteria and Classification, Therapeutic Approach to Idiopathic Hypertrophic Sub aortic Stenosis, Current Concepts of Hemiblocks, Physical Activity and Coronary Heart Disease, His Bundle Electrocardiography-Its Clinical Value, Computerized Electrocar diography-Its Practical Value, and Echocardiography-Its Practical Value. As the title of the book indicates, the "pro and con" viewpoints are described in each chapter, and the authors final conclusions are expressed at the end of each. It is hoped that the book will be provocative as well as educational and practical. The contents are intended to be clinical, concise, and practical, so that this book will provide all physicians with up-to-date materials that will assist them directly in the daily care of their patients with common cardiac problems.
