Since we have hundreds of miRNAs, the major challenge is now to understand their specific biological function. In fact the experimental evidence suggests that signaling pathways could be ideal candidates for miRNA-mediated regulation. Several studies suggest that miRNAs affect the responsiveness of cells to signaling molecules such as WNT, Notch, TGF-β and EGFR. Altered expression of particular miRNAs has been implicated in the onset and development of cancer and could be used as potential biomarkers for the disease. Recently, many studies have found miRNAs have crucial regulatory roles in Cancer stem cells (CSCs) a kind of tumor initiating cells (TICs) and dormancy. Findings also suggest that DNA methylation may be important in regulating the expression of many miRNAs in several cancer initiating cells. Several miRNAs are known to either upregulated or downregulated in CSCs when compared to non-cancerous cells from the same tissues. CSCs are a small subpopulation of cells identified in a variety of tumors and involve in self-renewal, differentiation, chemoresistance and tumorigenesis. Evolving technologies and the increasing availability of “multiomics” data are leading to the selection of numerous potential biomarkers, based on DNA, RNA, miRNA, protein, and metabolic alterations within cancer cells or tumor microenvironment, that may be combined with clinical and pathological data to greatly improve the prediction of both cancer progression and therapeutic treatment responses. However, in recent years, few biomarkers have progressed from discovery to become validated tools to be used in clinical practice. Such dysregulation is caused by multiple mechanisms, and exposure to different carcinogens causes dysregulated epigenetic changes and defects in the miRNA biogenesis machinery. Cancer cells with abnormal miRNA expression evolve the capability to sustain proliferative signaling, evade growth suppressors, resist cell death, activate invasion and metastasis, and induce angiogenesis. Genome-wide profiling demonstrates that miRNA expression signatures are associated with tumor type, tumor grade and clinical outcomes, so miRNAs could be potential candidates for diagnostic biomarkers, prognostic biomarkers, therapeutic targets and preventive screening programs. Although miRNAs have multiple targets, their function in tumorigenesis is due to their regulation of a few specific targets. After the first detection of altered miRNA in leukemia, microRNAs have been demonstrated to be constantly altered in all cancer. More recently, microRNA has been shown to be altered by exposure to environmental carcinogens, thus driving the whole process of carcinogenesis. Our aim is to provide a rigorous peer review and publish cutting-edge research on the role of microRNA in cancer prevention therapy to educate and inspire the scientific community worldwide.

MicroRNAs (miRNAs) are short noncoding RNA fragments, which involve in posttranscriptional regulation of gene expression, and they are shown to involve in tumorigenesis either targeting oncogenes or tumor suppressor genes. Based on the current studies, miRNAs are now suggested as potential biomarkers for CRC diagnosis, prognosis, and therapeutic responses. In this chapter, the latest findings on the role of miRNA in CRC in many aspects are reviewed: diagnosis (role of circular miRNAs in blood and miRNAs from tissue biopsies and their potential role in pathophysiology and diagnosis of CRC), prognosis (miRNAs related with metastasis, recurrence, and survival rates in CRC), and therapeutic responses (role of miRNAs both in chemotherapies and/or in targeted therapies in CRC). In conclusion, miRNAs are promising molecules for diagnosis, prognosis, and therapeutic responses of CRC.
