The diagnosis of oral cancer at an early stage has a good prognosis as the survival rate is high (around 80%). However, the majority of oral cancer cases are diagnosed at a later stage with a considerably poor 5-year survival rate of 50% according to World Health Organization statistics. Thus, an effective management strategy for oral cancer will depend on its early identification and intervention which would pave the way for superior prognosis. Despite the obvious advantage of earlier diagnosis of oral cancer, no approach has yet proven to be a reliably successful in diagnosis of oral cancer at an early stage. Currently; the primary line of screening of oral cancer is performed by visual inspection, which is a subjective examination. Among the screening tests or diagnostic aids now available for oral cancer, few (toluidine blue, brush biopsy, salivary and serum bio-markers) have been utilised and studied for many years while others have recently become commercially available. The authors in the present book review all the modalities of screening aids used in oral cancer detection and provide an update on the latest screening tools used in oral cancer detection.

Identification and detection of oral potentially malignant disorders (OPMD) allows delivery of interventions to reduce the evolution of these disorders to malignancy. A variety of new and emerging diagnostic aids and adjunctive techniques are currently available to potentially assist in the detection of OPMD. The accuracy of adjunctive tests namely, autofluorescence, chemiluminescence, toluidine blue staining was evaluated in relation to conventional oral examination and surgical biopsy. DNA ploidy was performed and results were correlated with adjunctive test results and pathology (dysplasia). -- A total of 126 patients, 70 males and 56 females (mean age 58.5 ±11.9 years) who presented to the Oral Medicine Clinics at King's and Guy's Hospitals, London with oral white and red patches suspicious of OPMD were enrolled. Sixty-one patients were current smokers, 28 ex-smokers, while 92 were alcohol users. In a detailed investigation these patients underwent all adjunctive tests followed by surgical and brush biopsy. -- Based on the clinical diagnosis, 70 patients had oral leukoplakia/erythroplakia, 32 had oral lichen planus, 9 chronic hyperplastic candidiasis and the rest frictional keratoses (13) or oral submucous fibrosis (2). Of 126 lesions, 105 (83%) showed loss of fluorescence, 95 (75.4%) were positive with ViziLite and 55 (44%) retained toluidine blue. Following biopsy 44 had oral epithelial dysplasia (29 mild, 8 moderate and 7 severe). Ploidy analysis was performed in two laboratories on 85 specimens (38 dysplasia, 47 non-dysplasia). Ten (43.5%) of 23 dysplasia cases were aneuploid while only 1 (2.6%) was aneuploid in the non-dysplasia group.
