Book excerpt: This book is the result of an informal association between the editors which extends back to 1974. At the beginning, it was a tenuous alliance between physicians in quite different medical subspecialties-urology and psychiatry. As the alliance was forged, subspecialty rivalries and mistrust were replaced by a common clinical interest in the diagnosis and treatment of erectile problems. We quickly became aware of the high prevalence of such disorders, how poorly prepared we were to make accurate and re sponsible diagnosis and treatment plans, and how complicated an etiol ogical diagnosis could prove to be. A variety of biological and psychological influences bear on sexual function, and in many clinical contexts, diagnosis and treatment planning involves consideration of complex interactive var iables. The need for an expanded multidisciplinary team became obvious. The requisite knowledge base extended across too many subspecialty boundaries, and the necessary information was not available in a conve nient source. As we began accumulating the information base, we became aware that this information might be of value to other physicians. The authors enjoy the luxury of an academic setting in which special expertise can be readily assembled. Such resources are often unavailable to the physician in practice. Oral phosphodiesterase type 5 (PDE–5) inhibitors are the first-line treatment options offered to patients with ED. This systematic review of the recent evidence on clinical benefits and harms associated with different pharmaceutical treatments used in the management of male ED is to clarify uncertainties in the field, as well as to identify existing knowledge gaps and directions for future research. The purpose of this evidence report was to review systematically the literature on the diagnosis and pharmaceutical treatments of erectile dysfunction (ED) and to address the following objectives put forth by the Agency for Healthcare Research and Quality (AHRQ) and the American College of Physicians (ACP). The primary objectives of this evidence report were: KQ 1. To determine the clinical utility of routine blood tests - testosterone, prolactin, luteinizing hormone (LH), follicle stimulating hormone (FSH) - in identifying and affecting therapeutic outcomes for treatable causes of ED. KQ 2. To determine the benefits of pharmaceutical treatments for patients with ED. KQ 3. To determine the harms of pharmaceutical treatments for patients with ED. The secondary objectives of this evidence report were: KQ 2a. To explore how patient-specific characteristics (e.g. specific symptoms, age, comorbid conditions) may affect prognosis and treatment success for ED patients. KQ 2b. To determine if the likelihood of treatment success varies by underlying cause of ED. KQ 3a. To identify specific harms, such as nonarteritic ischemic optic neuropathy (NAION) and penile fibrosis of pharmaceutical treatments in patients with ED.

In a UK-based study, 32% of British men had difficulty obtaining an erection, 20% with maintaining an erection. In recent years the physiology and pathophysiology of ED have changed our understanding of what ED is from a purely psychological-based disorder to a multifactorial one, with neurological, endocrinological, psychological factors and the role of the vascular system. Recently identified risk factors include diabetes, cardiovascular disease, spinal cord injury, smoking, depression, atherosclerosis, hypertension, pelvic surgery and trauma, pharmacological medications, arthritis, peripheral vascular disease, substance abuse, endocrine abnormalities and peptic ulcer disease. Many ED patients have a combination of these risk factors, thus exacerbating ED further. The treatment of ED was revolutionized in 1998 with the introduction of sildenafil. Beside these oral treatments, newer injectable agents are being investigated as are topical preparations. However, not all patients are tolerant of oral agents, many patients still require penile reconstruction with penile implants, an option with low complications and morbidity.
