Book excerpt: Diabetes, also known as diabetes mellitus (DM), is a disease that occurs when the glucose level in the blood becomes too high (hyperglycaemia). Chronic hyperglycaemia is accompanied by both biochemical and pathological complications such as retinopathy, nephropathy, neuropathy, and cardiovascular diseases. Diabetes mellitus can be classified into two types: type I which results from the destruction of pancreatic β-cells, leading to insulin insufficiency and type II diabetes mellitus, which is mainly related to insulin resistance. Optimal control of blood glucose levels remains the cornerstone of managing DM. To date, the major classes of antidiabetic medications used to treat diabetes include: biguanides, dipeptidyl peptidase 4 (DPP-4) inhibitors, sulfonylureas, meglitinides, thiazolidinediones (TZDs), sodium-glucose cotransporter (SGLT2) inhibitors, α-glucosidase inhibitors, glucose-dependent insulinotropic polypeptide (GIP) receptor and glucagon-like peptide-1 (GLP-1) receptor agonists and the various types of insulin. Several of these drugs may have, besides their antidiabetic properties, also important cardiovascular complications for the patients taking them.

This book was released on 2007-10-28 with total page 645 pages. Available in PDF, EPUB and Kindle. Book excerpt: The cause of diabetes mellitus is metabolic in origin. However, its major clinical manifestations, which result in most of the morbidity and mortality, are a result of its vascular pathology. In fact, the American Heart Association has recently stated that, “from the point of view of cardiovascular medicine, it may be appropriate to say, diabetes is a cardiovascular disease” (1). But diabetic vascular disease is not limited to just the macrovasculature. Diabetes mellitus also affects the microcirculation with devastating results, including nephropathy, neuropathy, and retinopathy. Diabetic nephropathy is the leading cause of end-stage renal disease in the United States, while diabetic retinopathy is the leading cause of new-onset blindness in working-age Americans. The importance of this text on Diabetes and Cardiovascular Disease is evident by the magnitude of the population affected by diabetes mellitus. Over 10 million Americans have been diagnosed with diabetes mellitus, while another 5 million remain undiagnosed. The impact from a public health perspective is huge and increasing. As the population of the United States grows older, more sedentary, and obese, the risk of developing diabetes and its complications will increase. Epidemiological studies have identified diabetes mellitus as a major independent risk factor for cardiovascular disease. Over 65% of patients with diabetes mellitus die from a cardiovascular cause. The prognosis of patients with diabetes mellitus who develop overt clinical cardiovascular disease is much worse than those cardiovascular patients free of diabetes mellitus.

Available in PDF, EPUB and Kindle. Book excerpt: With cardiovascular disease becoming the most common cause of death in people with diabetes, interest in the assessment and treatment of heart disease in these patients has been reawakened. This book examines developing topics from a largely cardiological perspective, covering both pharmacological and non-pharmacological interventions. The DIGAMI study on the use of intravenous insulin infusion at the time of myocardial infarction (MI) has stimulated a large number of discussion papers on the best treatment of MI in the diabetic patient. The UKPDS has shown that treatment of Type 2 diabetes does not reduce cardiovascular end-points significantly, but that aggressive treatment of blood pressure can do so. In addition, sub-group analysis from several large cardiovascular trials has shown that treatment with statins, anti-platelet therapy, ACE inhibitors and other drugs will also reduce cardiovascular events in people with diabetes.

Available in PDF, EPUB and Kindle. Book excerpt: Diabetes and cardiovascular disease together account for the largest portion of health care spending compared to all other diseases in Western society. This work seeks to provide an understanding of the causes of diabetes and its cardiovascular complications. As this understanding becomes more widely appreciated, it will serve as a foundation for evidence-based care and wider acceptance of sound science. The International Conference on Diabetes and Cardiovascular Disease, held in Winnipeg, in June 1999, was organized to bring together a multi-disciplinary group of researchers dedicated to further knowledge amongst researchers, care givers, and the managers of the health system. The invited speakers submitted their works for publication, which serves as the basis for this book. Major themes include: epidemiology of diabetes mellitus, metabolic risk factors in diabetes and cardiovascular disease, hypertension in diabetes mellitus, cardiac function in diabetes, glycemic control and improved cardiovascular function, diabetes management, and endothelial function in diabetes.

Book excerpt: Diabetes mellitus affects approximately 20 million people in the US, or nearly 7% of the population. It is expected to increase by 70% within the next 25 years, and numerous epidemiologic studies have demonstrated that type 2 diabetes increases the risk of cardiovascular morbidity and mortality. It is estimated to cost over $92 billion in health care costs and lost productivity. The increased risk is due to the detrimental vascular effects of prolonged exposure to a hyperglycemic, oxidant-rich environment yielding associated cardiovascular risk factors: atherosclerosis, hypertension and clotting abnormalities. Hypertension and dyslipidemia in diabetic patients produces substantial decreases in cardiovascular and microvascular diseases. Nutritional and Therapeutic Interventions for Diabetes and Metabolic Syndrome provides an overview of the current epidemic, outlines the consequences of this crisis and lays out strategies to forestall and prevent diabetes, obesity and other intricate issues of metabolic syndrome. The contributing experts from around the world give this book relevant and up-to-date global approaches to the critical consequences of metabolic syndrome and make it an important reference for those working with the treatment, evaluation or public health planning for the effects of metabolic syndrome and diabetes. Scientific discussion of the epidemiology and pathophysiology of the relationship between diabetes and metabolic syndrome Includes coverage of Pre-diabetes conditions plus both Type I and Type II Diabetes Presents both prevention and treatment options

Book excerpt: Diabetes is an autoimmune, inflammatory disease affecting many different organ systems and exhibiting both primary and secondary defects. Because diabetes affects a wide range of cellular systems, a multidisciplinary effort has been mounted over the past several decades using a wide range of investigative techniques and methodologies in order to identify molecular mechanisms responsible for cellular dysfunction. Because primary defects at various levels of sub-cellular signaling, intracellular calcium handling, protein expression and energy regulation are often a primary consequence of diabetes. This volume is a compilation of new multidisciplinary research that will broaden our current understanding of diabetes and cardiovascular disease as well as provide the basis for the development of novel therapeutic interventions.
